What is IMRT (Intensity-Modulated Radiation Therapy)?

IMRT is an advanced form of 3D-conformal radiotherapy where the intensity of each radiation beam is modulated dynamically using multi-leaf collimators (MLCs) — thin tungsten leaves inside the Linac that slide in and out hundreds of times per session to shape the dose.

The result is a dose distribution that wraps tightly around the tumour while curving around critical organs — the spinal cord, parotid glands, brainstem, rectum, bowel, and others depending on the site.

Side effects depend heavily on the site treated. They build over the course and typically resolve 4–10 weeks after the last session. Long-term effects are rare with modern IMRT planning.

  • Skin redness / dryness over the treated area, especially in the last 2 weeks.
  • Fatigue that accumulates gradually; usually mild-to-moderate.
  • Head & neck: mucositis, taste changes, dry mouth (much milder than 3DCRT).
  • Pelvis: mild loose stools, urinary frequency, irritation; usually settles in 6–8 weeks.
  • Brain: mild headache, hair thinning in the radiation field.
