What is VMAT (Volumetric Modulated Arc Therapy)?

VMAT is a step beyond IMRT. Instead of stopping the Linac gantry at 5–9 fixed angles, VMAT delivers radiation continuously as the gantry rotates 360° around the patient. Three things change simultaneously during that arc: the position of the multi-leaf collimator (MLC) leaves, the rate at which radiation is delivered (dose rate), and the speed at which the gantry rotates.

The result is a "volumetric" sculpting of the dose — the radiation wraps around the tumour in three dimensions, conforming tightly to complex shapes and concavities that fixed-angle IMRT struggles with. The treatment also runs 3–5× faster per session, with most VMAT fractions completed in 2–4 minutes of beam-on time vs 12–18 minutes for IMRT.

Is RapidArc the same as VMAT?

RapidArc is Varian's commercial name for VMAT. Other vendors call their version VMAT, SmartArc or Elekta VMAT. Clinically they are the same technique — arc-based intensity-modulated delivery. RapidArc on the latest Varian Linacs is slightly faster and slightly costlier.
