Pets, Gardening and Home Hygiene During Treatment — Most Families Do Not Need to Rehome the Dog

Yes, for most patients. A healthy, vaccinated household pet can stay. Checkpoint-inhibitor immunotherapy does not suppress your bone marrow the way chemotherapy can, so blood counts usually stay normal. The precautions are narrow: someone else handles the litter tray, no licks on broken skin, and any bite is seen the same day.

This page exists because the advice families are given is usually wrong in one direction. A relative says the dog has to go. A neighbour says stop touching the plants. Both are said kindly, and both cost something real. Immunotherapy runs for months. Rehoming a pet and giving up a garden for a year removes two of the few things that make treatment liveable.

The confusion has an honest source. Almost all household advice for cancer patients was written for chemotherapy, where blood counts drop and infection risk rises with them. Checkpoint-inhibitor immunotherapy behaves differently. Your neutrophil count usually stays where it was. That single difference is why this list is short.

• Someone else empties the litter tray, every day. Cat faeces can carry toxoplasma, and the parasite needs about a day or more in the tray before it turns infectious. A tray emptied daily by another adult is a far smaller problem than one left standing.
• If nobody else can do it, wear disposable gloves and a well-fitting mask, empty the tray outdoors, and wash your hands with soap afterwards.
• No licking of your face, of any open wound, or of the skin around a drip line or port site.
• Keep the pet's own vaccination and deworming schedule current with your vet, and treat for fleas and ticks on time.
• Do not switch a pet to raw meat during your treatment. Raw diets shed bacteria into the kitchen and on to the animal's coat.
• Keep claws trimmed, and stop using hands and feet as play toys. Most cat scratches during treatment are avoidable ones.
• Hand over bird cages, fish tanks and rabbit hutches. The dust and the tank water are the risk, not the animal.
• Reptiles, turtles, baby chicks and ducklings are the exception. They carry salmonella routinely while looking perfectly healthy, so keep them at arm's length for now.
• This is not the year to bring home a new puppy, kitten or reptile. Young and newly bought animals shed more of everything.

Nothing here replaces the instruction your own oncologist gives you. Your counts, your steroid dose and your other medical conditions can all move these lines.

Yes, with gloves. Soil, compost and potting mix are not sterile. They carry tetanus bacteria and fungal spores as a matter of course. The risk reaches you through a cut or through dust you breathe in, never through the plants. Wear gloves and closed shoes, keep dry compost away from your face, and wash your hands afterwards.

• Gloves for every session, even ten minutes of weeding on the balcony.
• Closed shoes, not slippers. A large share of garden wounds land on the foot.
• Open compost, manure and potting-mix bags outdoors, at arm's length, and dampen the surface before turning it. Dry material throws spores into the air you are standing in.
• Wear a well-fitting mask for turning compost, sweeping dry leaves, or any work where the soil is visibly dusty.
• Skip the garden on infusion day and on any day you feel unwell. Immunotherapy at CION is given as day care, and nothing in the garden is urgent.
• Wash your hands with soap immediately afterwards, before touching your face, your eyes or food.
• Clean and cover any cut the same day, and look at it again tomorrow. A garden cut that is redder on day two is a phone call, not a wait-and-see.
• Empty standing water weekly — pot saucers, old tyres, cooler trays. In Telangana a mosquito-borne fever during treatment is a genuinely avoidable problem.
• Hand over the heavy digging. Fatigue and wounds are the two real limits here, not the plants.

This table follows the general position taken in routine infection-prevention guidance for patients on cancer treatment, including NCCN's. It is a starting point, not a decision for your case — your team may set it tighter or looser depending on your counts and your steroid dose.

Neither, exactly. Checkpoint-inhibitor immunotherapy takes a brake off the immune system rather than suppressing it. Blood counts usually stay in the normal range. You are not neutropenic in the way a patient on chemotherapy may be. What can lower your immunity is the steroid treatment given for an immune side effect.

This is the single most useful thing to understand before you rearrange your house. Chemotherapy lowers the white cells that fight infection. Immunotherapy does not work that way. Your defences against a scratch, a mouthful of soil dust or a cough in the next room are broadly the ones you had before treatment started.

The catch is that the same released immune system can turn on healthy organs. Those reactions are called immune-related adverse events, and they are treated with steroids. Steroids genuinely do suppress immunity. So your risk is not a fixed setting. It moves with your current prescription, which is why the table above has two columns instead of one.

The stricter list starts on day one of the steroids. They do suppress immunity, and they can blunt fever and pain, so an infection shows itself late. Hand over the litter tray, the compost and the cage cleaning completely. Report a symptom even when the thermometer reads normal.

   Litter trays, bird cages, hutches, fish tanks, compost and manure. Not with gloves. Not once a week. Someone else does them until your team confirms the steroid course is finished.

   Watch symptoms, not the thermometer

   Steroids can hold a fever down. Breathlessness, a new cough, burning on passing urine, a spreading patch of skin, or simply feeling wrong is enough to call the helpline.

   Tighten the food and water rules

   Boiled or filtered water only. Freshly cooked, hot food. This is the phase where skipping outside food is genuinely worth the inconvenience.

   Keep the steroid dose written down and on you

   Any doctor who sees you for an infection needs to know the dose and how long you have been taking it. Steroids are never stopped suddenly on your own.

   Ask about any vaccine before, not after

   Live vaccines stay off the list during immunotherapy, and steroids make that firmer. Non-live vaccines can still be given, but your body may respond less well while the dose is high.

Two vaccine points matter for anyone with pets. Live vaccines are avoided during immunotherapy — that group includes the oral polio drops handed out at public camps, oral typhoid capsules, BCG, yellow fever, chickenpox and measles-mumps-rubella. The rabies vaccine given after an animal bite is not a live vaccine. It is never delayed because of cancer treatment, because rabies cannot be treated once symptoms appear. Take it, then tell your oncology team the same day.

Handwashing, safe water, and keeping unwell people at a distance. Those three do more than deep-cleaning the whole flat. Concentrate on the kitchen, the bathroom and anyone in the house who is coughing. Disinfecting the sofa and the curtains is not a cancer precaution, and it exhausts the caretaker.

• Soap and water, twenty seconds — before every meal, and after every contact with an animal, with soil, or with the bathroom.
• Boiled or filtered drinking water. Keep the storage pot covered, and empty it fully before refilling rather than topping it up.
• Keep raw and cooked food apart, use separate boards, and cook meat and eggs through.
• Towels are personal during treatment, and the bathroom gets cleaned more often than usual.
• Change bedlinen weekly, and sooner if you have been sweating at night.
• Anyone with fever, cough or loose motions keeps their distance and wears a mask until they are better. That includes visiting relatives, and it includes household help.
• Ask visitors to come when they are well. A crowded drawing room on a Sunday is a far bigger exposure than the dog is.
• Empty every source of standing water weekly — cooler trays, plant saucers, buckets, the terrace.
