The short answer
How much does CML medicine cost in India, and do generics help?
The cost of CML treatment depends mostly on which TKI you take and whether a generic version is available. Generic imatinib costs a small fraction of the original brand in India, while the newest TKIs usually cost far more each month.
Why the tablet is the main cost
Most people with chronic phase CML are treated as outpatients. There is no long hospital stay. The biggest expense is the daily tablet, taken every month for many years. Small monthly differences add up to a large sum over a decade, which is why the choice of product matters to families.
The other costs to budget for
Regular BCR-ABL tests, blood counts, consultations and travel to your centre come on top of the tablet. Extra tests such as mutation testing, heart checks or scans are needed at certain points. If CML progresses to an advanced phase, costs rise sharply because hospital treatment may be needed.
What this page cannot tell you
It does not give a quote. Prices change often, differ between manufacturers and pharmacies, and depend on your TKI and your scheme or insurance. Your treating team and pharmacy can give you a current figure for your own prescription.

Why do some TKIs cost so much more than others?
The price mainly reflects how long a medicine has been available and whether other companies are allowed to make it.
Imatinib
The first TKI, used for many years. Several Indian manufacturers make generic imatinib, so it is usually the least expensive option. For many people in chronic phase it works well as a first treatment.
Second-generation TKIs
Dasatinib, nilotinib and bosutinib are used first-line in some people, or after imatinib stops working or causes problems. Generic versions of some are available in India, which has lowered their cost.
Third-generation and newer TKIs
Ponatinib and asciminib are kept for specific situations, such as certain mutations or resistance to earlier TKIs. They are generally the most expensive, and cheaper versions are more limited.

Tests and follow-up
The BCR-ABL test is repeated at intervals for as long as you are treated. Ask whether it is reported on the International Scale, so results stay comparable wherever you are tested.

Practical steps
How can you lower the cost without risking your treatment?

Ask your haematologist about a generic
Say openly that cost is a worry. Your doctor can tell you whether a generic of your TKI is suitable and which products they are comfortable with. Stay with one manufacturer where possible.

Check every scheme you may qualify for
Aarogyasri, PM-JAY, CGHS, ECHS, EHS and employer or cashless insurance may cover tests, consultations or tablets. Coverage for outpatient tablets varies, and rules change, so check your current entitlement.

Ask about patient assistance programmes
Some manufacturers and charities run programmes that provide TKIs at reduced cost or free for eligible patients. Your treating centre can often help with the paperwork.

Buy through a reliable source
Use a licensed pharmacy and keep the bills. Avoid unlabelled strips or imports sold without a prescription. Fake or poorly stored medicine can lose its effect.

Keep taking it steadily
Missed tablets can lead to resistance, which may mean a costlier TKI later. Steady use is the surest way to keep long-term costs down.

What do families get wrong about the cost of CML tablets?
"A cheap tablet cannot possibly work as well."
Price in India reflects competition between manufacturers, not how strong the medicine is. Quality generics contain the same active medicine. Your BCR-ABL result is the real check of how well your treatment is working.
"We can take the tablet on alternate days to make a strip last longer."
This is one of the most harmful ways to save money. It can let the leukaemia grow and become resistant. Tell your team if you cannot afford the full supply, so they can help you find support.
"The newest and most expensive TKI is always the right choice."
Newer does not mean better for everyone. Many people do very well on imatinib for years. The right TKI depends on your risk, other illnesses and response, not on price.
"Insurance never covers tablets taken at home."
Some policies and schemes do cover oral cancer medicines, and others do not. Read the policy wording or ask the insurer directly, with your prescription in hand.

What documents should you keep ready for schemes and insurance?
- The bone marrow or blood report confirming CML
- A current prescription naming the generic medicine
- Your BCR-ABL reports in date order
- Pharmacy bills for every purchase
- Aadhaar, ration card or scheme card
- Insurance policy and any earlier claim letters
Planning for years
How do you plan for a medicine you may take for decades?
Think in years, not months. Work out what a steady monthly supply costs you after any scheme, and plan for that as a regular household expense, in the same way you would plan rent or school fees.
Tax relief and employer support
Section 80DDB of the Income Tax Act allows a deduction for treatment of certain illnesses, including cancer, subject to conditions. Some employers also reimburse long-term medicines. Check current limits with a tax adviser or your HR department.
Who this advice does not fit
People in accelerated or blast phase, or waiting for a transplant, face very different and much higher costs. Their plan needs a separate estimate from the treating team.

Common questions about CML drug cost and generics
Is generic imatinib as effective as the original?
Approved generics contain the same active medicine and must meet quality standards. Many people in India are treated with generic imatinib and monitored in the usual way. Your BCR-ABL test is the real check. If you change product, your team may repeat the test sooner to confirm the response holds.
Can I switch between generic brands to save money?
Talk to your haematologist before switching. Frequent changes between manufacturers can make it harder to tell whether a change in your result is due to the product. Once you find a reliable, affordable option your team approves, try to stay with it.
Does Aarogyasri or PM-JAY cover CML tablets?
Coverage for CML care under these schemes depends on the current package rules and on the hospital. Some parts of treatment may be covered while outpatient tablets may not. Rules change, so ask the scheme desk at your hospital with your card and diagnosis report.
What is a patient assistance programme?
It is a programme run by a manufacturer or charity that provides a medicine at reduced cost, or free, to people who meet its conditions. Eligibility usually depends on income, diagnosis and prescription. Your treating centre can tell you which programmes exist for your TKI and help with forms.
Why does my newer TKI cost so much?
Newer TKIs are often still protected by patents or have fewer manufacturers, so there is less price competition. They are usually prescribed for a specific reason, such as resistance or side effects. Ask your doctor whether a generic exists yet and whether an assistance programme is available.
Is the BCR-ABL test expensive?
The test is more costly than a routine blood count because it looks for the gene change at a very deep level. It is needed regularly, so ask your hospital for its current price and whether your scheme or insurer covers it. Using the same laboratory keeps your results comparable.
Can I buy TKIs online?
Only from a licensed pharmacy that asks for your prescription and gives a proper bill. Avoid sellers offering tablets without a prescription or at prices that seem too low. Fake or badly stored medicines can fail to control CML, and you may not know until your result rises.
What if we simply cannot afford the tablets this month?
Tell your haematologist straight away rather than stretching the supply. The team may be able to suggest a generic, a scheme, an assistance programme or a charity. Running out quietly or taking fewer tablets can harm your response and lead to much higher costs later.
