The short answer
What is a rituximab biosimilar, and why does it cost less?
A rituximab biosimilar is a version of rituximab made by another company after the original patent ended. It is approved as working the same way in the body, and it usually costs noticeably less, which is why it is widely used for lymphoma in India.
Why it is not called a generic
Rituximab is an antibody, a large protein grown in living cells. No two batches of a living product are exactly identical, even from the original maker. So a copy cannot be called a generic in the way a simple tablet can. Instead, the company has to show through laboratory tests and studies in patients that its version behaves in the same way.
Why the price is lower
The company making a biosimilar does not repeat the whole early research programme. Several makers now sell rituximab in India, and that competition brings prices down. In India, biosimilars are approved by the national drug regulator before they can be sold.
Who rituximab is for
Rituximab works on lymphoma cells that carry a marker called CD20. That mainly means B-cell lymphomas such as diffuse large B-cell and follicular lymphoma. It is not part of the usual plan for Hodgkin lymphoma or T-cell lymphomas, so a biosimilar will not lower the cost of those plans.

What decides how much rituximab costs you over the whole course?
The price of one vial is only part of it. These are the things that change the total.
The number of treatments
Your plan sets how many times rituximab is given. A short first course costs far less than a plan that adds a longer period of maintenance treatment afterwards.
Your body size
The amount given into a vein is worked out from your height and weight. A larger person needs more of the medicine at each visit, so the cost per visit is higher.
Vein or under the skin
Rituximab can be given as a drip into a vein or, in some cases, as an injection under the skin. The two forms are priced differently and take different amounts of day-care time.

Each treatment includes a bed, nursing time, medicines given beforehand to prevent a reaction, and blood tests. These add to the medicine cost on every visit.

What happens on a rituximab day in day-care?
Checks before treatment
Your blood counts are checked and the doctor asks how you have been since the last visit. Before the very first dose, a blood test for past hepatitis B infection is standard, because rituximab can wake up a hidden infection.
Medicines to prevent a reaction
You are usually given medicines beforehand to lower the chance of fever, chills or a rash during the drip. The nurse explains what to report.
The drip itself
The first treatment is given slowly and can take most of the day. Later treatments are often quicker if the first one went smoothly. A nurse checks you regularly throughout.
Going home
Most people go home the same day. You are told which symptoms mean you should call the team, and when your next visit is.

Is a cheaper biosimilar a weaker medicine?
"A cheaper version must be a weaker copy."
A biosimilar has to show that it works the same way as the original before it is approved. The lower price comes from skipped research costs and competition between makers. Price on its own tells you nothing about how well a medicine suits your lymphoma.
"We should buy whichever version is cheapest this month."
Changing between versions should be a decision your haematologist makes and records, not one made at the pharmacy counter. The team keeps track of which product you received on each visit, so that any reaction can be linked to the right one.
"Rituximab is chemotherapy, so it will cause the same hair loss."
Rituximab is an antibody, not chemotherapy, and hair loss is not one of its usual effects. Where hair loss happens, it usually comes from the chemotherapy given alongside it. Reactions during the drip are the more common problem.
"If the original brand is available, the biosimilar is second best."
For most people with CD20-positive lymphoma, a biosimilar is a standard choice. The right product is the one your haematologist is comfortable with and that you can continue for the whole planned course without a break.

Who does rituximab not suit, and what can this page not tell you?
Rituximab, original or biosimilar, is not right for everyone with lymphoma. It helps only where the lymphoma cells carry CD20, and your biopsy report will say whether they do.
When the team may be more careful
People with a past hepatitis B infection need extra checks and sometimes extra medicine to protect the liver. People with serious heart or lung problems, or an active infection, may need the timing changed. Pregnancy also changes the plan. Tell the team about every illness and medicine before the first dose.
What a price list cannot tell you
No price list can tell you how well rituximab will work for you. It cannot tell you how many treatments you will end up needing, or whether your plan will change after the response scan. It cannot predict an admission for an infection, which can cost more than the medicine itself.

Take your biopsy report and treatment plan to the billing or scheme desk. Ask for a written estimate that shows the medicine, the day-care visits and what your cover pays. If the figure worries you, say so to your haematologist before treatment starts, not halfway through.

What do the words on a rituximab prescription or bill mean?
- Biosimilar
- An approved version of an antibody medicine that works the same way as the original.
- CD20
- A marker on the surface of many B-cell lymphoma cells. Rituximab attaches to it.
- Infusion
- A drip of medicine into a vein, given slowly in day-care.
- Maintenance
- Rituximab given on its own at spaced-out visits after the main treatment, for some slow-growing lymphomas.
- R-CHOP
- A common plan that combines rituximab with a set of chemotherapy medicines. The R stands for rituximab.

Common questions about rituximab biosimilars and cost
How much cheaper is a rituximab biosimilar than the original?
It is usually noticeably cheaper, but the gap changes with the maker, the hospital and current prices. Because the total depends on how many treatments you need and your body size, ask for a written estimate for your whole plan using the product the hospital will actually use.
Does a biosimilar work as well for lymphoma?
Biosimilars are approved only after showing they work the same way as the original in the body. They are widely used for CD20-positive lymphoma. How well treatment works for you depends on the type and stage of your lymphoma and your general health, not on the price of the product.
Can we bring our own rituximab from outside to save money?
Ask the hospital first. Antibody medicines must be kept cold the whole way, and a break in storage can spoil them without any visible sign. Many hospitals give only products they have stored themselves, both for safety and so that the record of what you received is reliable.
Can I switch from the original to a biosimilar partway through?
Sometimes, but only with your haematologist's agreement. The team needs to record the change so any reaction can be linked to the right product. Do not switch on your own because a pharmacy offers a lower price. Raise cost worries at a clinic visit so the team can plan with you.
Does Aarogyasri or insurance pay for rituximab?
Aarogyasri, PM-JAY, CGHS, ECHS, EHS and many insurers cover rituximab where it is part of an approved lymphoma plan. Packages may cover a specific product or set limits, and rules change. Ask the scheme desk or your insurer to confirm cover before the first treatment.
Is the injection under the skin cheaper than the drip?
Not always. The two forms are priced differently, and the injection may not be available as a biosimilar everywhere. It can shorten your time in day-care, which matters if you travel far. Your haematologist will say whether it suits you, since the first treatment is normally given as a drip.
What side effects add to the cost?
A reaction during the drip may lengthen the day-care visit. Infections are the side effect most likely to add cost, especially when rituximab is given with chemotherapy, because they can need admission. If you get a fever or feel very unwell at home, contact the team or go to the nearest emergency department straight away.
Why do I need a hepatitis B test before rituximab?
Rituximab can let an old, silent hepatitis B infection become active again and harm the liver. A simple blood test before the first dose shows whether you carry it. If you do, the team can protect you with extra medicine and checks. The test is a small cost that prevents a serious problem.
