After losing an arm or a leg, you can apply for a disability certificate and a UDID card, the national disability identity card. A government medical board assesses the disability as a percentage. At 40% or above it is a benchmark disability, which opens concessions in travel, tax, jobs, education and pensions. A major amputation is usually assessed above that line. The short answer
Can you get a disability certificate after an amputation?
Yes. A person who has lost an arm or a leg can apply for a disability certificate and a UDID card, the national disability identity card. A medical board at a government hospital assesses how much the amputation affects you, and that assessment decides which benefits you can claim.
Why the percentage matters
The certificate records disability as a percentage. Under the Rights of Persons with Disabilities Act, 2016, a disability of 40% or more is a benchmark disability. Most of the concessions in travel, jobs, education and pensions depend on reaching that line. A major amputation of an arm or leg is usually assessed above it, but the board decides, not the hospital that did the operation.
Is it worth applying during cancer treatment?
Often, yes. Treatment and recovery are expensive, and some of the benefits, such as tax relief and travel concessions, help straight away. The application does not interrupt treatment. The main cost is time: a visit to the board and some paperwork. A family member can do most of the online part.

How do you apply for the certificate and UDID card?
Register online
Apply on the national UDID portal, Swavlamban Card, with a photo, Aadhaar, proof of address and the medical documents. A family member can fill the form in for you.
Find the right board
In Telangana, assessment has run through the state SADAREM system at government hospitals, linked to the UDID card. Ask the district welfare office or hospital which board or camp to attend.
The medical board visit
Doctors examine the amputation and review the reports. It is an assessment, not treatment. Bring the originals and copies, and a family member to help you move around.
Certificate and card
The certificate is issued with the percentage recorded, and the UDID card follows. Track the application online with the enrolment number you were given.

What should you take to the medical board?
- Aadhaar card and a second proof of address
- Recent passport-size photographs
- The discharge summary from the amputation
- The operation note and pathology report
- The UDID enrolment number or printout
- Photocopies of everything, as well as originals
- Crutches, wheelchair or limb you normally use
- A family member to help with queues and forms
What it opens
What entitlements can the certificate open?
Each benefit has its own conditions. The certificate is usually the first thing every office asks for.
Travel
Indian Railways gives fare concessions to people with an orthopaedic disability, usually where the certificate says an escort is needed. State bus concessions may also apply.
Income tax
Section 80U gives a deduction to the person with the disability. Section 80DD gives one to a family member who supports them. A tax adviser can check which fits.
Pension
The Telangana government pays a monthly pension to persons with a benchmark disability who meet the income and other conditions. Apply through the district office.
Jobs and education
Government posts and seats in higher education reserve a share for persons with benchmark disabilities.

What do the words on the forms mean?
- Benchmark disability
- A disability assessed at or above the level set by law. Most concessions are open only at this level.
- Locomotor disability
- The category that covers loss of an arm or leg, and difficulty moving the body or limbs.
- UDID
- Unique Disability ID. A single national card meant to replace separate certificates for different offices.
- Permanent or temporary
- Whether the certificate needs renewing. A healed amputation is usually recorded as permanent.
- Medical board
- A panel of government doctors authorised to assess disability and sign the certificate.
- Reasonable accommodation
- Changes an employer or institution makes so a person with a disability can take part on equal terms.
Commonly believed
Are these beliefs about the disability certificate true?
The law protects employees with disabilities from being treated unfairly because of it. Whether to share the certificate with an employer is your choice, and it is often what unlocks adjustments.
Only an authorised government medical board can. Your treating hospital provides the reports the board needs to see.
The application itself is free and can be made online. Be wary of anyone asking for money to speed it up. The district welfare office can help if you are stuck.
An amputation does not change once healed, so there is often no reason to wait. Applying earlier lets tax and travel benefits help during treatment, when the expense is highest.

What can this page not tell you?
This page explains how the system usually works. It cannot tell you what percentage the board will record for you, or whether you meet the conditions for any single benefit.
Why your own case may differ
The percentage depends on the level of the amputation, the other arm or leg, and any other conditions the board takes into account. Pension and scheme rules include income limits and other conditions that change from time to time. Railway rules depend on what the certificate says about travelling with an escort. None of this can be judged from a web page.
Where to get reliable answers
For the certificate and card, use the national UDID portal and the district welfare office. For tax, speak to a tax adviser. For pensions, ask the district office responsible for welfare of persons with disabilities. If a relative is managing the paperwork, make sure they carry an authorisation letter and copies of your ID.

Common questions about disability certificates after amputation
How soon after the amputation can we apply?
You can usually register online at any time. The board generally examines you once the wound has healed, because an amputation does not change after that. Ask your surgical team for the discharge summary and operation note first, since the board will want to see both.
What percentage will a leg amputation get?
It depends on the level and on the board's assessment. A major amputation of a leg or arm is usually assessed above the benchmark line, and higher amputations are assessed higher. Nobody but the board can give you the actual figure, so treat any number you hear in advance as a guess.
Can a family member apply on the patient's behalf?
A family member can fill in the online form and collect documents. The person with the amputation normally has to attend the board in person to be examined. If travel is very difficult, ask the district office whether a home visit or nearby camp is possible.
Does the certificate need to be renewed?
For a healed amputation, the certificate is usually recorded as permanent and does not need renewing. Some other conditions are certified for a set period. Check what your own certificate says, and keep several copies, because many offices keep one.
Will the certificate help with the cost of the artificial limb?
It can. The ADIP scheme and some state and charitable programmes help eligible people with artificial limbs and mobility aids, often with income conditions. The certificate is normally needed to apply. Ask the district welfare office or the limb centre what is currently available.
Is there a disability pension in Telangana?
Yes. The Telangana government pays a monthly pension to persons with a benchmark disability who meet its conditions, including income limits. The amount and rules are revised from time to time. Apply through the district office with your certificate, Aadhaar and bank details.
Does taking the certificate affect Aarogyasri or insurance?
It should not remove any cover you already have. Aarogyasri, CGHS, ECHS and EHS are based on their own eligibility rules. Insurance policies have their own terms, so read yours or ask the insurer. Call the helpline if you need help understanding what cover applies to treatment.
What if the board's percentage seems too low?
You can usually appeal to a higher authority named in the rules. Ask the office that issued the certificate how to appeal and within what time. Take a letter from your treating doctor explaining how the amputation affects your daily life.
