Go to an emergency department today if
You are breathless at rest, cannot lie flat because of breathlessness, have noisy or whistling breathing, or notice swelling of the face, neck or arms with swollen veins on the chest. Call 108 or go to the nearest emergency department now. A mass in the chest can press on the windpipe or on the large vein that drains the head and arms. Tell the team if a scan has already shown a chest mass, and do not wait for a clinic appointment.
The short answer
Can lymphoma in the chest cause a cough and breathlessness?
Yes. Lymphoma can grow as a mass in the middle of the chest, and as it grows it presses on the windpipe, the lungs and nearby veins. A dry cough that will not settle, breathlessness and chest pressure are the most common results.
What the mediastinum is
The mediastinum is the space in the middle of the chest, between the two lungs. It holds the heart, the windpipe, the food pipe, large blood vessels and a group of lymph nodes. A mediastinal mass is anything growing in that space. Because the space is tight, even a mass that causes no pain can press on the structures around it.
Who this tends to affect
Lymphoma in this space is found most often in teenagers and young adults. The two types seen most are Hodgkin lymphoma and a type of non-Hodgkin lymphoma called primary mediastinal B-cell lymphoma. In older adults, a mass here is more often from other causes, including lung cancer, thymus tumours, thyroid swelling and TB.
Why it is often missed at first
A young person with a cough is usually given cough syrup, then antibiotics, then perhaps an inhaler. Each step is reasonable on its own. The problem comes when weeks pass and nobody orders a chest X-ray. If the cough has not improved after treatment, or you also have night sweats, weight loss, itching or a lump in the neck, ask for a chest X-ray.

Which chest symptoms can a mediastinal mass cause?
Symptoms often creep up over weeks and are first put down to a chest infection or asthma.
A cough that will not settle
Usually dry, often worse lying down, and not better after antibiotics or cough syrup. Pressure on the windpipe keeps setting it off.
Breathlessness
First on stairs or walking fast, then with less effort. Many people find it worse when lying flat and sleep propped up without realising why.
Chest pressure or pain
A heavy or tight feeling behind the breastbone. Some people notice pain there after drinking alcohol.
Swelling of the face and neck
Pressure on the large vein can make the face puffy, especially in the morning, with a full feeling in the head.

Finding the cause
What tests follow if a chest mass is suspected?

A chest X-ray
Often the first test. It can show a widened area in the middle of the chest, but cannot say what is causing it.

A CT scan of the chest
Shows the size of the mass, what it is pressing on, and whether nodes elsewhere are enlarged. It also helps plan how to get a sample.

A biopsy
If a neck or collarbone node is swollen, that node is usually sampled because it is easier to reach. Otherwise tissue is taken from the chest mass with a CT-guided needle or a small operation.

Blood tests and a PET-CT
Once lymphoma is confirmed, blood tests and a PET-CT scan show how far it has spread. This is called staging and it shapes the treatment plan.

Tests of the heart and breathing are often done before treatment, because some treatments affect them.
Commonly believed
What do families often assume about a chest mass?
"He is young and does not smoke, so it cannot be serious."
Lymphoma in the chest is most common in young adults, including non-smokers. A cough that lasts in a young person deserves the same attention as in an older one.
"A mass in the chest means lung cancer."
Many masses in the middle of the chest are not lung cancer. Lymphoma, thymus growths, thyroid swelling and TB nodes are all possible. The biopsy report tells you which.
"It is TB, so we can start TB medicines."
TB and lymphoma can look alike on a scan. TB medicines should start only once TB is shown on a sample. Treating on a guess can delay the right treatment while the mass keeps growing.
"We can use steroids to shrink it while we decide."
Steroids can shrink lymphoma quickly and make the biopsy much harder to read. They should be given only by the treating team, who will weigh this carefully.

What can this page not tell you?
This page cannot tell you what is in your chest. Symptoms and even a CT scan cannot separate lymphoma from other causes with confidence. The biopsy report does that.
It cannot tell you the outlook
If lymphoma is confirmed, the outlook depends on its exact type, how far it has spread and your general health. The size of the mass alone does not decide it. A haematologist explains your own situation once the biopsy and staging scans are complete.
One thing to tell every doctor
If you know you have a chest mass, say so before any procedure that needs sedation or anaesthesia. Lying flat and being sedated can make breathing harder when a mass is pressing on the airway, so the team may plan the procedure differently. The same applies to dental work or any minor operation arranged before the diagnosis is complete.

Common questions about chest masses and lymphoma
Can lymphoma in the chest show up without a neck lump?
Yes. The mass can be the only place lymphoma is found at first, with no lump you can feel. In that case chest symptoms, or a chance finding on an X-ray, are the first sign. A CT scan and a biopsy of the mass are then needed to find the cause.
Why is my breathlessness worse at night?
When you lie flat, a mass in the middle of the chest can press harder on the windpipe and blood vessels. Many people start sleeping on extra pillows without noticing. If you cannot lie flat at all, or wake up struggling for breath, go to an emergency department the same day.
Will a normal chest X-ray rule out a mass?
Not always. A small mass can hide behind the heart and large vessels on an X-ray. If symptoms continue despite a normal X-ray, especially with night sweats, weight loss or a neck lump, ask whether a CT scan of the chest is needed.
Is a mediastinal mass always cancer?
No. It can be an enlarged thyroid, a cyst, a harmless thymus change, TB nodes or other infections. Some causes are cancers, including lymphoma. The type of mass, your age and the scan appearance guide the doctor, but a biopsy is usually needed for a firm answer.
How is the biopsy done if the mass is deep in the chest?
Where possible, a swollen node in the neck is sampled instead. Otherwise a needle is guided into the mass using a CT scan, or a surgeon takes tissue through a small cut. Your team chooses the safest route that gives enough tissue to name the type.
Does the cough go away once treatment starts?
In many people the cough and breathlessness ease as the mass shrinks with treatment, sometimes within the first weeks. This varies. Tell your team if breathing gets worse at any point, rather than waiting for the next appointment.
Can a chest mass affect the voice or swallowing?
Yes. A mass can press on the nerve that controls the voice box, causing hoarseness, or on the food pipe, making swallowing harder. Tell the doctor about any voice change or food sticking, as it helps show what the mass is pressing on.
