Ecosprin and aspirin before cancer surgery | CION Cancer Clinics
Ecosprin is not always stopped before cancer surgery. If you take it for a heart stent or after a stroke, your team may ask you to keep taking it, because a gap can cause a heart attack or stroke. If you take it only for prevention, it is more often paused. Your surgeon, anaesthetist and prescribing doctor decide together. Keep taking it until they give you a written plan. The short answer
Do you have to stop Ecosprin before cancer surgery?
Not always. Many people who take Ecosprin for a heart stent or a past stroke are asked to keep taking it through surgery, because stopping it raises the chance of a heart attack or stroke. Others are asked to pause it. The decision is made by your surgeon, anaesthetist and the doctor who prescribed it, never by you alone.
What Ecosprin actually does
Ecosprin is a low-dose form of aspirin. It makes platelets, the tiny blood cells that start a clot, less sticky. That protects a narrowed heart artery or a stent from blocking. It also means a cut bleeds a little more than usual, and the effect lasts for some time after the last tablet, because each platelet stays affected for its whole life.
Why there is no single rule
Two things are weighed against each other. One is how dangerous a small amount of extra bleeding would be in your particular operation. The other is how dangerous a gap in the tablet would be for your heart or brain. A person with a recent stent and a person who started Ecosprin on their own for "prevention" sit at opposite ends of that balance.

Why does the reason you take Ecosprin change the plan?
Tell the team which of these describes you. If you are not sure, bring the prescription or the name of the doctor who started it.
After a heart stent or bypass
This is the group where stopping carries the most danger. A stent can block suddenly without its protection. The surgical team will usually speak to your cardiologist before any change is made.
After a stroke or mini-stroke
Ecosprin lowers the chance of another one. Many teams prefer to continue it where the operation allows. Your neurologist or physician may be asked for an opinion.
Taken only for prevention
Many people take it without any heart or stroke history, sometimes on a friend's or pharmacist's suggestion. Here the balance often tips towards pausing it, but the team still decides.
As part of a combination strip
Some strips sold under the Ecosprin name also contain a cholesterol medicine or clopidogrel, another blood thinner. The plan for those is different.

Carry on exactly as prescribed until a doctor who knows about the operation tells you otherwise. Stopping early on your own is the most common mistake.
Tell the surgical team why
Say who started Ecosprin and for what. Bring any angiography or stent report, and any discharge summary from a heart or stroke admission.
Get the plan in writing
The plan should say whether to continue or pause, from which date, and who agreed it. If your cardiologist was consulted, their name should be on it.
Ask about restarting
If it is paused, ask before discharge when it will restart. Do not simply pick up the old strip once you are home.

What do people often believe about Ecosprin and surgery?
"Ecosprin is just a mild tablet, so it does not matter."
It is a low dose, but it still affects every platelet in your blood. For someone with a stent it is one of the most important tablets they take. Always mention it, however ordinary it seems.
"Any blood thinner means the operation will bleed badly."
For many operations, surgeons operate safely on people who continue aspirin. The team plans for it. What causes trouble is a blood thinner nobody was told about.
"If I stop it early, I am being extra careful."
For a person with a stent, stopping without a plan can cause the stent to block, which is a heart attack. Being careful means following the written plan, not doing more than it says.
Did you know
Common painkillers bought from a chemist, such as ibuprofen and diclofenac, also affect bleeding. If you take them alongside Ecosprin for joint or back pain, tell the surgical team about both.

What can this page not tell you?
This page cannot tell you whether to continue or pause your own Ecosprin, or for how long. It does not know your heart history, your operation or what your cardiologist has said. Those facts decide the plan, and only your team has all of them.
Who general advice suits least
People with a stent placed recently, people on two blood thinners at once, people with kidney or liver disease and people having surgery near the brain, spine or eye all need a plan made specifically for them. If that is you or your parent, expect more questions and possibly a call between doctors before a date is fixed.
If you are managing a parent's tablets
Older parents often take Ecosprin alongside several other strips, and it is easy to lose track. Photograph every strip, front and back. Keep the written plan on your phone. If the operation is postponed, ask again whether the Ecosprin plan still stands, because a paused tablet may need to restart while you wait.
What to ask your team
Ask whether your cardiologist has been contacted, and what they said. Ask whether Ecosprin continues or pauses, and whether any other tablet changes with it. Ask what to do on the morning of the operation. Ask who to call if chest pain, weakness on one side of the body or unusual bleeding starts at home. Write the answers down, because it is easy to forget them once you leave the clinic.

Common questions about Ecosprin before surgery
How many days before surgery should I stop Ecosprin?
There is no single number that suits everyone, and many people are not asked to stop it at all. The timing depends on why you take it and on the operation. Your surgeon and anaesthetist set it, often after speaking to your cardiologist. Follow the date on your own written plan and nothing else.
I stopped Ecosprin on my own. What now?
Tell your surgical team today and give them the date of your last tablet. If you have a stent or have had a stroke, also call the doctor who prescribed it, because the gap itself may be a risk. Do not restart or stop anything else until one of them has advised you.
Can I take Ecosprin on the morning of the operation?
Only if your written plan says so. Where it is being continued, you will usually be told to take it with a small sip of water. Where it is paused, you should not take it. If the plan does not mention the morning dose, ask the anaesthetist before the day.
Is Ecosprin the same as ordinary aspirin?
It contains the same medicine, in a lower dose and a coating that protects the stomach. For surgery planning, the team treats it as aspirin. Some Ecosprin combination strips contain other medicines too, so bring the strip so the team can read every ingredient listed on it.
Will I bleed more if I keep taking it?
A little more oozing is possible, and the surgeon plans for it. For many operations this is manageable. The team weighs that small extra bleeding against the danger of a heart attack or stroke if the tablet is stopped, and chooses the safer side for you.
Can I have spinal or epidural anaesthesia on Ecosprin?
Often yes when aspirin is the only blood thinner, but the anaesthetist decides after checking everything else you take. The answer can change if you are also on clopidogrel, another blood thinner or regular painkillers. Mention all of them at the pre-anaesthesia check.
My parent takes Ecosprin but nobody knows why. What should we do?
This is common. Look for the old prescription or ask the family doctor who first wrote it. Tell the surgical team that the reason is unclear. They may ask for a heart check before deciding. Keep giving it as usual until they do.
When will Ecosprin restart after surgery?
If it was paused, the surgeon decides when it is safe to restart, based on bleeding and how the wound looks. For people with a stent this is usually treated as a priority. Make sure the restart plan is written on the discharge summary before you leave the ward.
