Clopidogrel is often paused before a major cancer operation, but only when your heart doctor, surgeon and anaesthetist agree. There is no fixed number of days that suits everyone. If you have a heart stent, stopping it on your own can make the stent block suddenly. The short answer
Does clopidogrel have to be stopped before surgery?
Often it is paused before a major cancer operation, but only when your heart doctor and surgical team agree it is safe. How many days before is their decision, not a fixed rule. If you have a heart stent, stopping clopidogrel on your own can cause the stent to block suddenly.
What clopidogrel does
Clopidogrel, sold in India as Clopilet, Deplatt and other names, stops platelets from clumping together. Platelets are the tiny blood cells that start a clot. That protects a stent or a narrowed artery. It also makes a surgical wound bleed more than aspirin alone does, and the effect lasts after the last tablet until the body makes fresh platelets.
Why the number of days varies
The team weighs the bleeding risk of your operation against the risk to your heart or brain if the tablet stops. When the stent was placed, what type it was, whether you also take aspirin, and how urgent the cancer operation is all move that balance. Because cancer surgery often cannot wait long, these conversations need to start early.

One thing that cannot wait
If you have a stent and get chest pain, pressure or tightness, pain spreading to the arm or jaw, sudden breathlessness or heavy sweating, go to the nearest emergency department straight away. This matters most while clopidogrel is paused. Say that you have a stent and that your blood thinner was stopped for surgery. Do not wait to see if it settles, and do not drive yourself.

A recent stent
This is the highest-risk group. The stent is still settling into the artery wall and depends on the tablets most. Your cardiologist will be central to any decision.

An older stent
The risk of a gap is usually lower. Clopidogrel may be paused while aspirin continues, but only after your heart doctor has seen the stent details.
After a stroke
Clopidogrel may be your only blood thinner. The neurologist or physician who started it is often asked whether a pause is acceptable, and whether aspirin can cover the gap.
On a combination strip
Some strips combine clopidogrel with aspirin. People often do not realise they are taking two blood thinners. Bring the actual strip so the team can read every ingredient.

How is a clopidogrel plan put together?

You tell the surgeon early
Mention clopidogrel at the very first surgical visit, with your stent card or angiography report. The earlier it is known, the less likely the operation date has to move.

The surgeon contacts your heart doctor
They discuss when the stent was placed and how much bleeding risk the operation carries. You may be asked to see your cardiologist again.

The anaesthetist reviews it
Clopidogrel affects whether a spinal or epidural injection is safe, so the anaesthetist has a say in the timing as well.

You receive one written plan
It names the date of your last tablet, whether aspirin continues, and who to call if something changes. Keep it on your phone.

Restart after the operation
The surgeon decides when bleeding risk has settled enough. For people with a stent, restarting is treated as a priority.

Words you may hear
What do the words in the heart doctor's letter mean?
- DAPT
- Dual antiplatelet therapy. Taking two platelet medicines together, usually aspirin and clopidogrel, most often after a stent.
- Drug-eluting stent
- A stent coated with a medicine that stops the artery narrowing again. These usually need the tablets for longer.
- Stent thrombosis
- A clot forming inside the stent and blocking it. It causes a heart attack and is the main reason gaps are planned so carefully.
- Neuraxial anaesthesia
- A spinal or epidural injection. Blood thinners change when it is safe to give one.
Commonly believed
What do families often believe about clopidogrel and surgery?
"The surgeon said stop it, so the heart doctor does not need to know."
If you have a stent, your cardiologist should always know about a planned pause. If nobody has contacted them, ask the surgical team to do so, or take the plan to your cardiologist yourself.
"Clopidogrel and aspirin are the same, so I can stop either one."
They work in different ways and are planned separately. Often one is paused while the other continues. Swapping which one you stop can leave a stent without the protection it needs.
"A cancer operation is urgent, so the stent will have to take its chances."
Neither risk is ignored. The team looks for a timing that protects both the heart and the cancer plan, which is why the heart doctor, surgeon and anaesthetist decide together.
"If there is bleeding, nothing can be done on clopidogrel."
Surgical teams have ways of managing bleeding in people on platelet medicines, including platelet transfusion when needed. This is part of why continuing or pausing is a planned decision, not a guess.
Being straight with you
What can this page not tell you?
This page cannot tell you when to take your last clopidogrel tablet, or whether you should stop it at all. It does not know your stent date, the type of stent, your operation or your cardiologist's view. Those decide the plan.
Who needs the most careful plan
People with a stent placed recently, people who have had a stent block before, people on clopidogrel plus another blood thinner, and people with kidney disease need a plan built specifically for them. If that describes your parent, expect more than one appointment before the date is confirmed.
What to ask your team
Ask who has spoken to the cardiologist and what they agreed. Ask whether aspirin continues. Ask what to do if the operation is postponed after the tablet has already stopped. Ask who to call at night if chest pain starts at home.

Common questions about clopidogrel before surgery
How many days before surgery do you stop clopidogrel?
Your surgeon, anaesthetist and heart doctor set this together, and it differs from person to person. It depends on your stent, whether you also take aspirin and the bleeding risk of the operation. This page does not give a number because a wrong number can be dangerous. Follow only your written plan.
What if the operation is postponed after I have stopped it?
Call your surgical team the same day you hear of the delay. They may ask you to restart clopidogrel while you wait, and then plan a fresh pause. Do not restart it on your own, and do not stay off it for weeks without anyone deciding that is safe.
Can I keep taking aspirin while clopidogrel is paused?
Often the plan is exactly that, especially for people with a stent, but not always. Some operations carry a bleeding risk high enough that the team decides differently. Your written plan should mention both tablets by name. If it mentions only one, ask about the other.
Is Deplatt the same as Clopilet?
Both contain clopidogrel, made by different companies. Some versions of each also contain aspirin or a cholesterol medicine, and the name on the strip may only differ by a letter. Always bring the strip itself so the team reads the full ingredient list rather than the brand name.
Can I have a spinal anaesthetic while on clopidogrel?
A spinal or epidural injection is usually avoided until clopidogrel has been paused for long enough, because of the small risk of bleeding near the spinal cord. The anaesthetist decides. If the tablet cannot be paused, a general anaesthetic may be planned instead.
I take ticagrelor or prasugrel instead. Is it the same?
They belong to the same family and raise similar questions, but each wears off at its own pace, so the plan is not copied from clopidogrel. Tell the team exactly which one you take and bring the strip. The heart doctor's view matters just as much.
Will I need a heart test before surgery?
Possibly. If you have heart symptoms, a stent or your heart doctor has not reviewed you recently, the team may ask for an ECG, an echo or a cardiology visit. This is about making the operation safer, not a sign that surgery has been ruled out.
When does clopidogrel restart after the operation?
When the surgeon judges the bleeding risk has settled, which depends on the operation and how you recover. For people with a stent it is not left to drift. Make sure the restart plan is on the discharge summary, and ask whom to call if you are unsure at home.
