Clothing, saree and dressing with a stoma | CION Cancer Clinics
Most people wear the clothes they wore before surgery with a stoma bag, including sarees, salwar suits, lungis and trousers. Modern bags lie flat and are hard to see under clothing. The main thing to plan is the waistline: keep drawstrings, belts and tight bands above or below the stoma. This page covers each everyday garment, dressing during recovery, and special occasions. CION Cancer Clinics’ surgical oncologists in Hyderabad can talk this through with you.

The short answer
Can you wear a saree and your usual clothes with a stoma bag?
Yes. Most people go back to the clothes they wore before surgery, including sarees, salwar suits, lungis and trousers. A bag lies flat under clothing, and a few small changes to where a waistband or knot sits usually make it invisible and comfortable.
Why the bag is less visible than you fear
Modern bags are thin, flexible and made to lie close to the body. When the bag is emptied before it gets too full, it does not bulge. Most people who meet you will not know you have a stoma unless you tell them.
The one thing to plan around
The waistline. A tight band, drawstring or belt that presses directly across the stoma can stop output flowing into the bag, cause leaks and rub the stoma. The answer is to move the band above or below it, or to loosen it at that point.
How the stoma site helps
Before planned surgery, a stoma nurse often marks the right spot on the tummy while you sit, stand and bend. Part of that choice is keeping the stoma away from the usual line of a waistband or saree petticoat.
If you had emergency surgery, the stoma may sit on the waistline. It is still possible to dress comfortably. Ask your stoma nurse for ideas.Garment by garment
How do you wear everyday Indian clothes with a stoma?
Small adjustments to what you already own work better than buying a new wardrobe.
Saree
The petticoat drawstring is the main issue. Tie it above or below the stoma, or move the knot to the side. The pleats and pallu fall naturally over the bag.
Also try
- An elastic-waist or wide-band petticoat
- Tucking pleats on the side away from the stoma
Salwar, churidar and kurta
A kurta hides the bag completely. Swap a tight nada for elastic, or wear the salwar a little higher or lower than before.
Lungi, dhoti and panche
Fold or tuck the top edge above or below the stoma rather than across it. A thin belt worn well away from the stoma can help keep it in place.
Trousers and jeans
Higher-waisted trousers often sit above a low stoma. Braces instead of a belt remove pressure altogether. Very stiff, tight jeans can press on the bag.
Nightwear and innerwear
High-waisted underwear holds the bag close and flat. Loose cotton nightwear is comfortable while the wound is still healing.

Ask an oncologistAfter surgery
What can you wear as you recover?
-
In hospital
A hospital gown or a loose nightie or kurta that opens easily, so nurses can check the wound and the stoma. Bring soft, loose clothes for going home.
-
The first weeks at home
The tummy is sore and the stoma is still swollen. Choose loose clothes with soft elastic or no waistband. Avoid anything that presses on the wound.
-
When the wound has healed
Start trying your usual clothes again. Put them on and sit, bend and stand to check nothing presses on the stoma. This is a good time to ask about support underwear.
-
Back to your usual wardrobe
Once the stoma has settled to its final size, most people wear what they wore before. Your surgeon will tell you when lifting and fitted support wear are safe.
Small things that help
What makes dressing easier day to day?
- Empty the bag before dressing and before going out
- High-waisted underwear or a stoma support band to hold it flat
- A fabric bag cover to stop plastic rustling or sticking to skin
- Darker or patterned fabrics if you feel self-conscious at first
- No safety pins or brooches pinned near the bag
- A spare bag and set of clothes in your handbag or bike box

Commonly believed
What do people wrongly believe about clothes and a stoma?
Many women wear sarees every day with a stoma. Moving the petticoat knot away from the stoma is usually all it takes. The drape of a saree hides a bag very well.
A flat, emptied bag under ordinary clothes is very hard to spot. The feeling that people are staring is common in the first months and usually fades as confidence returns.
Loose clothes help while the wound heals. After that, fitted clothes are fine as long as nothing tight presses directly on the stoma itself.
Pressure over the stoma blocks output from entering the bag and can push it under the baseplate, causing more leaks. A proper stoma support band is designed differently.
Occasions
How do you dress for weddings, temple and prayer?
Being straight with you
When is dressing harder, and what can this page not tell you?
Some people find it harder to hide the bag or keep it comfortable. That is not a failure, and there is usually a fix worth asking about.
When the stoma sits on the waistline
After emergency surgery, or when weight has changed, the stoma may sit exactly where waistbands fall. Higher-waisted clothes, braces, dresses and kurtas often work better than tucking.
When there is a hernia or prolapse
A bulge beside the stoma, or a stoma that sticks out, changes the shape under clothes. Support wear helps, but it needs to be measured and fitted by a stoma nurse so it does not press on the stoma.
What this page cannot tell you
It cannot tell you what your own stoma needs, or when your wound is healed enough for fitted clothes or support wear. Your stoma nurse and surgeon can.
Feeling like yourself again
Wearing what you love is part of recovery. If worry about the bag is stopping you going out, tell your team. Talking to a counsellor or a stoma support group often helps.
Clothes that suit a stoma are sold in ordinary shops. You do not need to buy an expensive range. Take the bag with you when you shop, try clothes on sitting down, and bring a family member whose opinion you trust.Questions we are asked
Common questions about clothing with a stoma
Can I tie the saree petticoat string over the stoma bag?
It is better not to. A drawstring tied across the stoma can block output and cause leaks. Tie it above or below the stoma, move the knot to the side, or switch to an elastic-waist petticoat that sits softly over the bag.
Do I need special stoma clothing?
Most people do not. Everyday clothes with small changes work well. Support underwear and bands made for stomas are useful for some people, especially when lifting or if a hernia is a concern. Ask your stoma nurse whether they suit you.
Can I wear jeans and a belt?
Often yes, once you have healed. Choose a waist that sits above or below the stoma. If the belt line crosses it, braces are a comfortable alternative. Very stiff denim that presses on the bag is worth avoiding.
Will the bag show under a fitted kurta or blouse?
Rarely, if the bag is emptied regularly and held flat by high-waisted underwear. Patterned or slightly thicker fabric can help while you get used to it. Try the outfit on at home and look in the mirror while sitting and standing.
Can I wear gold waist chains or a vaddanam?
A decorative waist belt is usually fine if it sits above or below the stoma and does not press on the bag. Wear it loosely and avoid it while the wound is still healing.
What should I wear for a long bus or train journey?
Something with a soft, loose waist, and a top that covers the bag when you are seated. Empty the bag before you leave. Keep a spare bag, wipes and a change of clothes in your hand luggage.
How do I stop the bag rustling under clothes?
A soft fabric bag cover quietens the plastic and stops it sticking to warm skin. Many bags now come with a cloth backing. Your supplier or stoma nurse can suggest one.
My father refuses to wear his usual lungi. How can we help?
Let him try it at home first, with the top folded above or below the stoma. Reassure him that nobody can see the bag. Worry about being noticed is common early on. If it continues, a stoma nurse or counsellor can talk it through with him.
