CION Cancer Clinics
Clothing and saree with a urostomy bag | CION Cancer Clinics
You can wear a saree with a urostomy bag, and almost everything else you wore before. The only real change is where a cord, waistband or belt sits: above or below the stoma, never across it. This page goes garment by garment, shows how to drape a saree over the bag, explains the belts and bands stoma suppliers sell, and covers weddings, the gym and sleep. CION Cancer Clinics’ surgical oncologists in Hyderabad can talk this through with you.

The short answer
Can I wear a saree with a urostomy bag?
Yes. A saree is one of the easier garments to wear with a urostomy, because the pleats and pallu fall over the bag and nothing presses on it. The one thing to change is where the petticoat cord sits. It must not cross the stoma.
Where the stoma usually sits
A urostomy is normally placed on the right side of the belly, below the navel and a little to the side, on a flat patch of skin the surgeon marks before the operation. That is exactly the height at which most women tie a petticoat and most men wear a belt. So the question is never whether you can wear your usual clothes. It is only how you fasten them.
The two rules that cover almost everything
First, nothing tight should press directly on the stoma or on the bag's tap. A waistband can sit above the stoma or below it, not on it. Second, the bag needs room to fill. Urine flows all the time, and a bag squeezed flat under a tight cord will push urine back against the seal and leak. Everything else on this page follows from those two rules.
Nothing here replaces a fitting with a stoma nurse. If a garment is leaking every time, the fit needs looking at, not the garment.Garment by garment
What works, and what needs a small change?
Most people keep their whole wardrobe. A few habits change.
Saree and petticoat
Tie the petticoat cord above the stoma, at the natural waist, or switch to a petticoat with a wide soft band or elastic. Drape the pleats so they fall over the bag.
Salwar, churidar and leggings
A drawstring salwar is easy: tie it loosely above the stoma. Fitted leggings and churidars with a narrow elastic band press right on the bag, so choose a high-waist pair with a wide soft band, or wear them rolled below the stoma with the kameez covering everything.
Lungi, dhoti and trousers
A lungi or dhoti tied high, at the navel, sits above most stomas and works well. Trousers are the awkward one. A belt on the stoma is the commonest cause of a daytime leak in men. Wear them a little lower, use braces, or go a size up and use a soft belt.
Underwear and support
Underwear can go over the bag or under it, whichever you find holds the bag steadier. A stoma belt or a soft wrap band, sold by stoma suppliers, spreads the weight of a filling bag and stops it swinging when you walk.

How do I drape a saree over a urostomy bag?
Empty the bag first
Start with a near-empty bag and a fresh seal if the change day is close. Check that the tap is closed and pointing the way you prefer, down or to the side.
Position the petticoat cord
Tie the cord above the stoma, or use a petticoat with a wide elastic band. Run a finger under the band to be sure it does not touch the wafer edge anywhere.
Tuck away from the stoma
Tuck the first end of the saree into the petticoat on the side away from the stoma. Keep the tuck loose over the bag so the fabric is not pulling the wafer.
Let the pleats fall over the bag
Make the pleats as usual and tuck them in at the centre. The fall of the pleats covers the bag completely. If the bag sits to the right, a slightly wider pleat set hides any outline.
Sit and bend once before you leave
Sit, stand, and bend to touch your knee. If anything pulls, loosen the tuck over the bag. A quick check now saves a change later.
On the supply list
Clothing aids you may hear about, in plain language
- Stoma belt
- A narrow elastic belt that clips to tabs on some bags and holds the wafer firmly against the skin. Useful under a saree or for heavy work.
- Wrap or ostomy band
- A wide soft tube of stretch fabric worn over the bag, like a short vest for the belly. It holds the bag flat and stops it swinging.
- Bag cover
- A cloth sleeve that slips over the bag so it does not touch bare skin or sweat against it. Easy to sew at home from old cotton.
- Stoma guard
- A small shield worn over the stoma under a belt or seatbelt, for people whose work or travel presses on that spot.
- Night drainage bag
- A large bag connected to the day bag's tap at bedtime, so you sleep through without emptying. Nothing to do with clothes, but it changes what you wear in bed.

Commonly believed
Four things families say about clothes and the bag
She will not. The saree is among the kindest garments to a stoma, because nothing in it is fitted across the belly. The only change is the height of the petticoat cord. Many women wear exactly what they wore before, from the first month.
Loose clothes let the bag swing and bulge as it fills. A bag held flat with a wrap band, under a normally fitted kurta or shirt, is far less visible. What hides a bag is support, not extra fabric.
A leak is a fit problem to solve, not a colour to plan around. People who dress for the leak they are expecting stay anxious. Fix the seal, carry one spare bag, and wear the colours you like.
He can wear a belt that sits above or below the stoma, a soft belt, or braces. What he cannot do is buckle a stiff belt across the stoma itself. Most men settle this in the first fortnight by moving the belt down a little.
Real life
What about weddings, festivals, the gym and bed?
All of these are possible. The trick each time is the same: empty before, support the bag, and carry one spare.
Weddings and long functions
Heavy silk sarees and sherwanis are fine. Plan where the nearest bathroom is, empty the bag before the ceremony, and keep a small pouch with one spare bag and a wipe in a handbag or a jacket pocket. A wrap band under a heavy outfit stops the bag pulling when you sit for hours.
Walking, gym and yoga
Walking needs nothing special. For the gym, a stoma belt or wrap band and a bag emptied beforehand are enough. Ask your surgeon before lifting heavy weights or doing strong abdominal exercises, because a hernia beside the stoma is a known problem after this operation.
Sleeping
Connect a night bag and wear loose cotton, with the tubing running out the side and down.
What this page cannot tell you
It cannot tell you which wafer or belt suits your body, or whether a bulge beside the stoma is a hernia. Both need a person looking, and a stoma nurse or your surgeon will get there faster than guesswork.
If the skin under a band or belt goes red and stays red, stop using it and show the stoma nurse.Questions we are asked
Common questions about clothes and a urostomy
Will people be able to see the bag under my clothes?
Not under ordinary clothes. A bag emptied at a third full sits flat, and a saree, kurta, shirt or salwar covers it completely. Nobody who does not already know will notice. What does show is a full, swinging bag, so emptying on time matters more than any particular garment.
Can I wear a blouse with a tight fit and a saree?
Yes. The blouse ends well above the stoma, so its fit does not matter. Some women choose a slightly longer blouse so that raising the arms does not lift it past the top of the wafer, but that is a preference, not a rule.
Which side should the bag tap face?
Whichever makes emptying easier for you. Most people point the tap down for daytime and use a night bag in bed. Under a saree, some women turn the tap slightly to the side so it does not catch on the petticoat. Try both for a week each.
Can I wear jeans or a fitted skirt?
Yes, with the waistband above or below the stoma rather than on it. High-waist jeans a size up, or a stretch skirt with a wide band, both work. A stiff waistband pressed on the wafer edge is the thing to avoid, because it lifts the seal when you sit.
Does a seatbelt or a scooter press on the stoma?
A car seatbelt should be worn, always. Slide it so the lap strap sits below the stoma, or use a small stoma guard. On a two-wheeler the waistband and the pillion grip are the pressure points, so empty the bag first and check that nothing sits on the stoma.
Can I wear a bag cover made at home?
Yes. A simple sleeve of soft cotton, open at the tap end, keeps the plastic off the skin and soaks up sweat. Wash it daily. Do not use anything with elastic tight enough to squeeze the bag flat, because a bag that cannot fill will push urine back under the seal.
What do I wear to the temple or for prayers?
Whatever you normally wear. Sitting cross-legged on the floor is fine once the wound has healed and the bag is emptied first. If bending forward repeatedly, a wrap band keeps the bag steady. There is no religious rule we know of that a urostomy breaks, and your surgeon can write a note if a family member asks.
Where in Hyderabad do I buy stoma belts and bands?
The surgical suppliers that stock urostomy bags usually carry belts and wrap bands from the same brands, and the larger ones deliver to the districts. Ask the stoma nurse at your centre for the current list. The CION helpline can also point you to stoma support and suppliers.
