Will Insurance or a Government Scheme — Pay for Bevacizumab?

Several government schemes and most private insurance policies can cover bevacizumab, but each requires pre-authorisation before treatment starts. Knowing which scheme you qualify for before your first cycle saves time and avoids an unexpected bill.

Several government schemes cover bevacizumab when it is prescribed for an approved indication. PM-JAY, CGHS, ESI, and the Aarogyasri schemes in Telangana and Andhra Pradesh all include cancer treatment packages that can include bevacizumab. Private insurance generally covers it too. In every case, pre-authorisation before the cycle starts is required.

What do these scheme names actually mean?

PM-JAY (Ayushman Bharat)
India's central government health assurance scheme for lower-income families. Eligibility is based on Socio-Economic Caste Census data. It covers hospitalisation and day-care procedures including cancer treatment packages, up to an annual limit per family.
Aarogyasri / YSR Aarogyasri
State health schemes in Telangana and Andhra Pradesh that cover cancer treatment for BPL and near-BPL families. They work alongside PM-JAY and in some cases provide top-up cover for treatments PM-JAY does not fully fund.
CGHS (Central Government Health Scheme)
Covers central government employees, pensioners, and their dependants. Treatment must be at an empanelled hospital. Bevacizumab is reimbursable when prescribed under an approved protocol.
ESI (Employees' State Insurance)
Covers formal-sector employees earning below the notified wage ceiling. Cancer treatment including chemotherapy and biological therapy is provided free at ESI hospitals and empanelled centres.
Pre-authorisation
Written approval from the scheme or insurer that a planned treatment is covered, obtained before treatment begins. Without it, the scheme may decline to pay even when the treatment was clinically appropriate.
Biosimilar
A version of bevacizumab made by a different manufacturer and approved by CDSCO as equivalent in quality, safety, and efficacy to the original. Biosimilars are substantially cheaper and are the version most schemes will fund.

What do I need to sort out before my bevacizumab cycle?

  • Check your PM-JAY enrolment status through the Ayushman Bharat portal or at any government hospital — have your Aadhaar number ready.
  • Government employees and pensioners: ask your department whether you hold active CGHS or ECHS coverage.
  • Ask your oncologist to write your diagnosis code and the indication for bevacizumab — schemes approve by diagnosis, not by drug name alone.
  • Confirm with CION's care coordinator that your treating centre is empanelled under your scheme — this is required for cashless treatment.
  • Submit the pre-authorisation request at least one week before your planned cycle.

Does private health insurance cover bevacizumab?

Most Indian private health insurance policies include biological therapy and targeted agents under their cancer treatment benefit, but coverage is rarely unconditional.

Your policy may have a sub-limit for chemotherapy or high-cost drugs that is lower than the cost of a full bevacizumab course. Read your policy's schedule of benefits, not just the headline sum insured.

Nearly all private insurers require pre-authorisation for each cycle. Your oncologist's team submits the request — ask whether the clinic has a dedicated insurance desk, because delays in authorisation can delay treatment.

If your insurer reimburses at the biosimilar rate but you receive the branded version, you may be responsible for the difference. Clarify this with your insurer before the first cycle.

Which scheme applies to me?

How do I find out if I am enrolled in PM-JAY?

Check through the Ayushman Bharat eligibility portal or ask at your nearest government hospital — they can look up your status using your Aadhaar number or ration card. Eligibility is based on the Socio-Economic Caste Census and is not linked to whether you have applied: many families are enrolled without knowing it. If your name appears, your care coordinator can help you activate the benefit at an empanelled centre.

Does PM-JAY cover the full cost of bevacizumab?

PM-JAY covers cancer treatment under defined packages, and the annual family limit applies across all treatment in that year. Whether bevacizumab fits within the package for your cancer type and stage is something your care coordinator can confirm by checking the scheme's package list. If the cost of a full course exceeds your remaining cover, the Aarogyasri top-up scheme in Telangana and Andhra Pradesh may bridge part or all of the gap. My employer offers group health insurance. Will it cover bevacizumab?

Group policies vary more than individual policies, and some have tighter sub-limits on high-cost drugs. Ask your HR team for the full policy schedule — not just the brochure — and share it with your care coordinator. The key things to check are whether biological therapy is explicitly included, what the per-hospitalisation and high-cost drug sub-limits are, and whether day-care chemotherapy is covered — because that is how bevacizumab is administered at CION.

What if I do not qualify for any scheme and cannot afford the cost?

Several pharmaceutical companies that manufacture bevacizumab biosimilars in India run patient assistance programmes for patients who cannot meet the full cost. Your oncologist can make a referral. CION's care coordination team can advise on available support at the time of your consultation. Cost alone is never a reason to delay telling your team — speak up early, not after a cycle has already been missed.

Will CGHS pay for bevacizumab at a private cancer centre?

CGHS reimburses treatment at empanelled private hospitals at notified rates. Bevacizumab is reimbursable when prescribed as part of an approved treatment protocol and when the treating hospital holds a valid CGHS empanelment. The reimbursement rate is set by CGHS and may differ from the hospital's billing rate — your care coordinator can tell you what the gap, if any, is likely to be. ECHS, the equivalent scheme for ex-servicemen, operates in a similar way.

I am already mid-treatment. Can I apply for coverage now?

PM-JAY and Aarogyasri: enrolment is prospective, but if you were already eligible when treatment started, ask at the scheme helpdesk whether a retrospective claim is possible. CGHS: claims for past cycles can sometimes be submitted with original bills and a referral note, depending on whether the hospital was empanelled at the time. Private insurance: most policies require pre-authorisation before treatment, and retrospective claims for high-cost drugs are commonly declined. Inclusion on the list does not automatically mean every scheme covers it — coverage still depends on whether your cancer type and the indication your oncologist has prescribed it for match the scheme's approved treatment packages. Your care coordinator can confirm this for your situation.

Are biosimilar versions of bevacizumab safe?

Yes. Biosimilars approved by CDSCO, India's drug regulator, are required to demonstrate equivalent quality, safety, and efficacy to the reference product before approval. Several bevacizumab biosimilars are approved and in clinical use in India. Your oncologist will confirm whether a biosimilar is appropriate for your specific treatment — they are not substituted without clinical review.

Does the Aarogyasri scheme in Telangana cover bevacizumab?

The Telangana Aarogyasri scheme covers specific cancer treatment packages for BPL families, and bevacizumab is included under packages for eligible cancer types. Coverage depends on your cancer type and the treatment protocol your oncologist has recommended. Your care coordinator can confirm whether your specific package is included. How long does pre-authorisation take?

For most government schemes, pre-authorisation takes two to five working days when the paperwork is complete. Private insurers vary — some issue approval quickly for standard protocols, others take longer for high-cost drugs. Build in at least one week before your planned cycle. Your oncologist's team should submit the request rather than you doing it yourself, as they know the clinical language and documentation the scheme expects.

What documents does the scheme or insurer ask for?

You will typically need your diagnosis in writing including the cancer type, stage, and diagnosis code; the oncologist's prescription or treatment plan; biopsy or pathology reports confirming the diagnosis; your scheme card or policy number; and your Aadhaar. Government scheme applications may also require an income certificate or BPL ration card. Ask your care coordinator for a checklist specific to your scheme before the first authorisation request.

What happens if the insurer only covers part of the cost?

You pay the difference between what the scheme or insurer reimburses and the actual treatment cost. That gap depends on whether the scheme reimburses at a biosimilar rate, what sub-limits apply, and whether the drug is billed separately from administration. Ask for a written cost estimate before your first cycle. If the shortfall is unmanageable, tell your care coordinator early — there may be assistance programmes or treatment options worth discussing.
