Olaparib is for patients whose cancer carries a BRCA1 or BRCA2 mutation — relevant in ovarian, breast, prostate and pancreatic cancer. A genetic test on your blood or tumour tissue confirms the mutation before treatment starts. Without a confirmed qualifying mutation, olaparib is not indicated.

What test result do you need before olaparib is considered?

A BRCA1 or BRCA2 mutation, confirmed by genetic testing, is the key requirement in most settings where olaparib is used. Your oncologist will order testing on a blood sample, on tumour tissue, or sometimes both.

The mutation can be germline — meaning it was inherited from a parent and present in every cell of your body from birth — or somatic, meaning it developed only in the cancer cells. Both types may qualify. A germline result also has implications for blood relatives, who may want to consider their own testing.

In prostate cancer, eligibility is slightly broader. Mutations in other genes that help repair damaged DNA — grouped under the term HRR — can also qualify alongside BRCA. Your oncologist will tell you which genes were included in your panel and what the result means for your options.

Am I likely to be eligible for olaparib?

  • You have ovarian, fallopian tube or peritoneal cancer with a confirmed BRCA1 or BRCA2 mutation
  • You have HER2-negative metastatic breast cancer with a BRCA1 or BRCA2 mutation
  • You have prostate cancer with a BRCA or other HRR gene mutation that has stopped responding to hormone therapy
  • You have pancreatic cancer with a BRCA1 or BRCA2 mutation and the disease has not progressed on platinum chemotherapy
  • Your genetic test — blood or tumour tissue — shows a qualifying mutation
  • Your oncologist has assessed your general fitness alongside the gene result

What do BRCA, HRR and PARP inhibitor mean?

BRCA1 and BRCA2
Genes that normally instruct your cells to repair damaged DNA. A mutation in either means that repair process is faulty — and olaparib works by exploiting exactly that weakness in cancer cells.
Germline mutation
A mutation inherited from a parent and present in every cell of your body from birth. A blood test detects it. Because it is heritable, close blood relatives may want to consider genetic counselling.
Somatic mutation
A mutation that developed only in the cancer cells and was not inherited. Detected by testing tumour tissue. It can qualify you for olaparib but does not affect your relatives' inherited risk.
HRR (homologous recombination repair)
A family of genes that work together to repair broken DNA strands. BRCA1 and BRCA2 are the most well known, but several others in the group can also predict a response to olaparib, particularly in prostate cancer.
PARP inhibitor
The class of drug olaparib belongs to. PARP is an enzyme cancer cells use as a backup repair tool. Blocking it in cells that already have a broken BRCA pathway leaves the cancer unable to repair its DNA — the cell cannot survive.

My BRCA test came back negative. Can I still have olaparib?

In most cases, a negative BRCA result means olaparib is not indicated. The main exception is prostate cancer, where mutations in other HRR genes — beyond BRCA alone — can also qualify. If you have prostate cancer and your test covered only BRCA, ask your oncologist whether a broader HRR gene panel was included. For ovarian, breast and pancreatic cancer, a negative BRCA result generally rules olaparib out, and your oncologist will explain which treatment options remain appropriate for your situation.

What is the difference between a blood BRCA test and a tumour tissue test?

A blood test detects mutations present in every cell of your body — ones inherited from a parent. If positive, your close blood relatives may carry the same mutation. A tumour tissue test looks only at the cancer cells and can detect mutations that developed in the tumour itself without being inherited. Both types of mutation can qualify you for olaparib. Your oncologist will advise which test is right for your situation; sometimes both are done.

Does my family need testing if my BRCA result is positive?

If your result is germline — found by a blood test, meaning it was inherited — close relatives such as parents, siblings and children may want to consider genetic counselling and testing. This does not mean they have cancer, but a confirmed BRCA mutation raises the risk of certain cancers, and there are surveillance and risk-reduction options available to carriers. If your result is somatic — found only in the tumour tissue — it was not inherited and your relatives do not need testing for this reason.

How long does BRCA testing take in India?

Turnaround time varies by laboratory and whether the sample needs to go to a specialist reference centre. A germline BRCA blood test typically takes one to three weeks. A somatic tumour tissue test can take a similar amount of time, depending on the quality and quantity of tissue available from your biopsy. If the original sample is insufficient, a repeat biopsy may be needed before testing can proceed. Ask your team when the sample was sent and when to expect the result, so you are not waiting without a timeline.

Is olaparib approved and available in India?

Yes. Olaparib has received approval from CDSCO, India's central drug regulator, for eligible patients in approved indications. It is taken as an oral tablet at home twice daily, so it does not require an infusion visit. Cost is a significant practical consideration; ask your oncologist about current pricing, any available patient access programmes, and whether your health insurance covers it. Any cost figure you find online is likely to be outdated — always confirm pricing directly with the dispensing pharmacy or your treating centre at the time of prescription.

What happens if I am not eligible for olaparib?

Not being eligible does not mean you are out of treatment options — it means a different treatment fits your cancer's biology better. Your oncologist will discuss alternatives suited to your cancer type and stage, which may include chemotherapy, other targeted therapies, hormonal treatment, or a clinical trial. It is reasonable to ask two specific questions: why was olaparib ruled out, and what is the recommended alternative expected to achieve.
