Why is generic abiraterone so much cheaper?
Indian generic manufacturers produce abiraterone under the Drug Pricing Control Order, which sets a ceiling on what they can charge. The National Pharmaceutical Pricing Authority publishes and enforces these ceiling prices.
The active ingredient is identical to the originator version. What differs is the brand name, the manufacturer, and sometimes the pack configuration. Several Indian companies including Cipla, Natco and MSN make abiraterone and sell it under their own brand names.
The originator price reflects the cost of the original clinical trials. Once generic competition enters a market, prices fall sharply — and India has had generic abiraterone available for several years.

How can you bring your monthly cost down?
Ask your oncologist whether your hospital is empanelled under Ayushman Bharat before you arrange a private purchase. If your family holds an Ayushman Bharat card or a Below Poverty Line ration card, the scheme may cover your medicine at no cost to you.
State governments run their own cancer medicine schemes alongside the central programme. Your hospital's medical social worker is the right person to check both your central and state scheme eligibility — this is a service CION centres offer as part of day care.
Several Indian manufacturers operate patient-assistance programmes for abiraterone, where the medicine is provided free or at very low cost to patients who qualify. Your oncologist can apply on your behalf. The criteria are income-based, and the process usually takes one to two weeks.

Why is the originator so much more expensive than Indian generics?
The originator company funded the clinical trials that established abiraterone's efficacy and safety, and the price reflects that investment. Once a medicine's patent protection expires or is successfully challenged, other manufacturers can produce it at the cost of manufacturing alone, which is far lower. India has had a competitive generic market for abiraterone for several years, which has driven prices down substantially for patients here.
