How to take ibrutinib

Taking Ibrutinib: — Food, Timing and Missed Doses

Ibrutinib is taken once daily at the same time each day. The rules are simple, but a few — grapefruit, missed doses, certain medicines — matter enough to know before you begin.

Medically reviewed by Dr. T. Raghavender Reddy, Medical Oncologist, MBBS · DM (Medical Oncology) · MD (Radiation Oncology) · Last reviewed August 2026

  • Once daily, same time every day — Consistent timing keeps ibrutinib levels steady in your bloodstream.
  • Food does not matter — but grapefruit does — Take it with or without a meal, but avoid grapefruit juice entirely.
  • Swallow whole — Opening, crushing or chewing the capsules changes how the drug is absorbed.
  • Missed dose: same day take it, next day skip it — Never take two doses to make up for one missed. The rule is simple and safe.

Take ibrutinib once daily at the same time each day, with or without food. Swallow the capsules whole — never crush or chew them. Avoid grapefruit juice. If you miss a dose, take it on the same day as soon as you remember. If you only realise the next day, skip it and continue as usual.

Can you take ibrutinib with or without food?

Yes — ibrutinib can be taken with or without food. What matters is picking a consistent time each day and keeping to it.

Grapefruit and grapefruit juice are the exception. They affect how your body processes ibrutinib and can raise the drug level in your blood higher than intended. Avoid them entirely, not just around the time of your dose.

Seville oranges, used in some bitter marmalades, have the same effect. If you are unsure about a fruit or drink, your pharmacist is the right person to ask.

How do I take ibrutinib each day?

  1. Take at the same time every day

Pick a time that fits your routine — morning, afternoon or evening — and stay with it. Consistent timing keeps drug levels stable.

  2. Swallow the capsules whole with a full glass of water

Do not open, break, crush or chew the capsules. Use water to swallow them, not grapefruit juice or citrus drinks.

  3. Do not double up if you miss a dose

If you remember on the same day, take the dose then. If you only remember the following day, skip the missed dose and continue at your usual time.

  4. If you vomit shortly after taking your dose

Do not take another capsule on your own. Call your oncology team and ask what to do — they will tell you whether to retake that day's dose.

What should you tell your team before starting ibrutinib?

  • Tell your oncologist every medicine you take — prescription, over-the-counter, vitamins and herbal remedies
  • Mention St. John's Wort specifically if you use it — it reduces how well ibrutinib works
  • Tell your team if you take blood thinners — ibrutinib affects platelets and increases bleeding risk
  • Tell your dentist or surgeon you are on ibrutinib before any procedure, including tooth extractions
  • Store ibrutinib at room temperature, away from moisture and direct heat

Common questions

Frequently asked questions

Do I need to take ibrutinib with food?

No. Ibrutinib can be taken with or without food — absorption is similar either way. What matters more is consistency: taking it at the same time each day keeps drug levels in your blood stable. If taking it with a meal helps you remember, that is a good reason to build that into your routine.

What happens if I take ibrutinib at a different time one day?

A single shift in timing is unlikely to cause a problem, but get back to your usual time the next day rather than shifting your whole schedule. If your chosen time is not working — it is hard to remember or clashes with something daily — speak to your team about settling on a better time before the inconsistency becomes a pattern.

Can I drink alcohol while on ibrutinib?

There is no absolute prohibition, but ibrutinib affects platelets and increases bleeding risk, and alcohol can add to that. Most oncology teams advise keeping alcohol to a minimum and being honest about how much you drink so they can factor it in. If you are also on blood thinners, the guidance will likely be more cautious — ask your oncologist directly.

Which medicines interact with ibrutinib?

Several medicines affect how ibrutinib is processed in the body. Strong antifungals, certain antibiotics, some heart medicines, and blood thinners are among the most common interactions. St. John's Wort reduces ibrutinib levels and should be stopped. This is why your full medicines list matters so much at the start — bring every packet to your first appointment, including supplements, so your oncologist and pharmacist can check each one.

How long will I need to take ibrutinib?

Ibrutinib is typically taken continuously — not in fixed cycles — for as long as it is controlling your disease and you are tolerating it. Unlike chemotherapy, there is no pre-set end date. Your oncologist will assess response at regular intervals. If treatment needs to change for any reason, that decision will be based on those assessments, not on a fixed duration.
