Why is there such a large price range for the same drug?
Imatinib has been manufactured in India since a landmark 2013 Supreme Court ruling upheld the government's right to refuse extended patent protection for the drug. That decision opened the market to multiple Indian manufacturers, and the National Pharmaceutical Pricing Authority now caps the price Indian companies can charge.
The originator brand is imported and is not subject to that same ceiling, which is why it sits at a very different price point. Most oncologists in India prescribe the regulated Indian generic, and the two are considered therapeutically equivalent by the CDSCO.
Government outlets go further still. Jan Aushadhi pharmacies are funded by the central government specifically to make essential medicines, including cancer drugs, available at close to manufacturing cost.
How do I apply for free or reduced-cost imatinib?

Your oncologist or a team member will know which scheme is active at your treating centre and can initiate a referral. Do not wait until after your first month of paying full price.
Meet the hospital social worker or patient navigator
Most large cancer centres have a social welfare desk. They hold current application forms and track which schemes are open — eligibility rules change, and they follow the updates.
Gather your documents before the appointment
You will typically need your pathology or bone marrow report confirming the diagnosis, a prescription from your oncologist, an income certificate or BPL card, and Aadhaar. Bring originals and several photocopies.
Submit the application and note the reference number
Ask for written acknowledgement and a clear timeline. If approval takes weeks, ask your team whether an interim reduced-price supply is possible while the paperwork is processed.
Follow up if you do not hear within the stated time
The hospital social worker is your main point of contact. Schemes can have funding gaps — if one route closes, ask immediately whether an alternative is open.

Yes. Indian-manufactured generic imatinib contains the same active molecule at the same dose as the originator. The CDSCO approves generics on the basis of bioequivalence data, which means the drug behaves the same way in the body. Your oncologist is very likely already prescribing the Indian generic; if you are unsure, ask them to confirm what is on your prescription.
Can I get imatinib free of charge in India?
Some patients can, through state government cancer drug supply schemes or the central government's Jan Aushadhi programme. Eligibility usually depends on income, BPL status, or the category of cancer centre where you are being treated. Free supply is not automatic — you need to apply with documentation. Ask your oncology team or the hospital social worker to check what is available at your centre.
Does Ayushman Bharat PM-JAY cover imatinib?
PM-JAY covers the cost of hospitalisation for listed conditions, but it does not typically cover outpatient oral medicines like imatinib taken at home. Some states have extended drug coverage schemes that go beyond PM-JAY — your team can confirm whether your state has one and whether you qualify. Do not assume PM-JAY covers your monthly imatinib supply without checking directly.
What is the NPPA ceiling price and how does it affect what I pay?
The National Pharmaceutical Pricing Authority sets the maximum price that Indian manufacturers can legally charge for essential medicines, including imatinib. This ceiling limits what private pharmacies can sell Indian-made imatinib for, which is why generic prices are far lower than the imported originator brand. Jan Aushadhi outlets are subsidised further, below even the NPPA ceiling, because the government absorbs part of the cost.
How long does it take to get approved for a free drug scheme?
It varies. Some state schemes process applications within a few weeks; others take longer, particularly when documentation needs verification. Start the process at the same appointment where imatinib is prescribed, not after your first month of paying full price. Your hospital social worker can give you a realistic timeline for the schemes at your centre and may be able to arrange a reduced-price interim supply while you wait.
Can I buy imatinib online to reduce cost?
