CGHS covers trastuzumab for central government employees, pensioners, and their dependants. The treating doctor applies for prior permission from the CGHS Additional Director before the first infusion. Cycles that begin in an emergency can be self-financed and reimbursed.
Aarogyasri in Telangana and Andhra Pradesh covers certain cancer treatment packages for families with qualifying ration cards. The hospital's social worker confirms whether your diagnosis and income category fall within the current approved package list.
ECHS covers trastuzumab for ex-servicemen and their dependants at empanelled hospitals, with prior approval from the ECHS polyclinic before treatment begins. ESI-registered employees earning within the wage ceiling can access treatment at ESIC hospitals or empanelled centres on referral from an ESIC dispensary doctor.

Does private health insurance cover trastuzumab?
Most comprehensive health insurance policies in India classify trastuzumab as a covered cancer treatment. Whether yours does depends on whether the policy has a cancer-specific exclusion, a sub-limit on targeted therapy, or a waiting period for malignancies that has not yet elapsed.
Read your policy schedule for the section on cancer or critical illness cover, then contact your insurer's pre-authorisation team to confirm targeted therapy is included. Do this before the first cycle, not at the billing counter.
Policies purchased after a diagnosis may exclude that condition as a pre-existing disease. If the policy is recent, check both the waiting period and the pre-existing disease clause before assuming cover applies.

How do I find out if my hospital is empanelled with PM-JAY?
The PM-JAY portal maintains a searchable list of empanelled hospitals by state and district. You can also ask at the hospital's reception or its Ayushman Bharat help desk — empanelled hospitals are required to display their PM-JAY registration. If the hospital where you are being treated is not empanelled, PM-JAY will not cover care there. In that situation you would need to consider a transfer to an empanelled centre or explore a different funding route.
Will the insurer pay the hospital directly or do I pay and claim later?
If your hospital is on your insurer's cashless network, the insurer settles the bill directly with the hospital after pre-authorisation is approved. If the hospital is not on the cashless network, you pay first and submit a reimbursement claim with all original bills, prescriptions, and discharge summaries. Confirm the cashless status before your first infusion — doing it at the billing counter is too late to change the arrangement.
Can I use both PM-JAY and private insurance for the same treatment?
You cannot claim the same expense twice — doing so is insurance fraud. However, if PM-JAY covers a treatment under its package rate and there is an uncovered balance, your insurer may be able to pay the shortfall depending on your policy terms. The sequencing has to be handled correctly, so discuss this with both the hospital's claims coordinator and your insurer before treatment begins, not after the bills arrive.
What should I do if my insurer rejects the pre-authorisation?
Ask for the rejection reason in writing. Common reasons include the hospital not being on the cashless network, the treatment not matching an approved package, or a waiting period that has not elapsed. Most rejections can be appealed — your oncologist can write a medical necessity letter if the ground for rejection is clinical. If the appeal is unsuccessful, ask the hospital's social work team about government schemes, manufacturer assistance programmes, or instalment arrangements with the billing department.
Does PM-JAY pay for the biosimilar or the original trastuzumab?
PM-JAY pays for approved treatment packages rather than named drugs. Within an approved package, the oncologist selects the appropriate medicine, and this is often a CDSCO-approved biosimilar because it meets the same clinical standard. If you have a clinical reason for a specific version, raise it with your oncologist, who can advise whether it can be accommodated within the package structure.
If I lose my job during treatment, does my CGHS or ESI cover stop immediately?
CGHS coverage for pensioners continues after retirement and is not employment-dependent. For serving employees, it continues while they remain in service. ESI has a continuation provision — if you have contributed for the minimum qualifying period, benefits may continue for a limited time after employment ends. The exact period depends on your contribution history, so contact your CGHS regional office or ESIC branch directly for the rules that apply to your situation.
