Are PET-CT and radiotherapy the same kind of radiation?

No. Both involve radiation, but they use it in completely different ways and for completely different purposes.

A PET-CT works in two parts. A small amount of radioactive tracer — a glucose-like substance tagged with a short-lived radioactive atom — is injected into your vein. As it travels through your body, a scanner detects the signals it gives off. A CT scan is taken at the same time to provide an anatomical map. Together, they show your oncologist where cancer cells are most active.

The tracer is not directed at your tumour. It spreads through the body briefly, gives off a detectable signal, and then decays. Nothing in the scan is designed to damage or destroy tissue.

Why is radiotherapy a treatment but a PET-CT is only a scan?

The difference is in dose and intent. Radiotherapy delivers a concentrated, high-energy beam aimed precisely at a tumour, in doses calibrated to damage the DNA of cancer cells and stop them dividing.

The dose in radiotherapy is intentionally many times higher than anything used in a diagnostic scan — because that level of energy is what it takes to affect cancer cells.

A PET-CT uses a dose set to produce images, not to affect tissue. You can have multiple PET-CT scans during your care — to stage the cancer, monitor response, or help plan a procedure — and each one is a diagnostic step, not a round of treatment.

What to remember about radiation and your care

  • PET-CT and radiotherapy are different procedures — ask your team to explain each one separately so you know exactly what to expect.
  • After a PET-CT: IAEA guidance advises limiting close, prolonged contact with young children and pregnant women for a few hours while the tracer is still active in your body.
  • After external beam radiotherapy: no similar precautions are needed. You are not radioactive when you leave the treatment room.
  • After brachytherapy (internal radiotherapy using a sealed source): your team will give you specific guidance — these precautions differ from external beam and from a scan.
  • Drinking water after a PET-CT helps your body clear the tracer through your kidneys. Your team will usually ask you to hydrate well after the scan.
  • If you receive written instructions after any scan or treatment session, follow those — they are specific to your situation and take precedence over general advice.

The radioactive tracer used in most PET scans is designed to decay rapidly. Within a day of your scan, the radioactivity has largely cleared from your body through natural decay and through your kidneys and bladder.

Frequently asked questions

Does a PET-CT scan give me any radiation treatment?

No. A PET-CT is a diagnostic tool, not a treatment. The radioactive tracer is used to produce images of where cancer cells are active in your body — it is not aimed at your tumour and is not intended to, or capable of, destroying cancer cells. Radiotherapy is a completely separate procedure, planned and delivered on different equipment, and serves an entirely different purpose.

Will the radiation from my PET-CT affect my family?

For most adults in your household, no specific precautions are needed. IAEA guidance suggests limiting close, prolonged contact with young children and pregnant women for a few hours after the scan, while the tracer is still active. By the following day the radioactivity has largely cleared from your body. Your imaging centre will give you written instructions — follow those, and call them if you have questions specific to your situation.

How long does the radioactivity from a PET-CT last in my body?

The tracer used in most PET scans decays rapidly. Within a day of the scan, the radioactivity has largely cleared through natural decay and through your kidneys and bladder. Drinking plenty of water after the scan helps your body clear it more quickly, which is why your team will usually ask you to stay well hydrated. You do not need to do anything special beyond that.

Is the dose from a PET-CT scan dangerous?

The dose from a PET-CT is set at a level designed and regulated for diagnostic use — not for treatment. ICRP and IAEA classify diagnostic imaging radiation separately from therapeutic radiation because the doses are far lower. No diagnostic test is entirely without risk at a population level, but both bodies consistently judge the clinical benefit of accurate imaging to outweigh the small risk from the scan dose. If you are having frequent scans, it is reasonable to ask your oncologist how many are planned and why each one is necessary.

Why do I need both a PET-CT and radiotherapy — are they not the same thing?

They serve opposite purposes. The PET-CT tells your team exactly where the cancer is and how active it is — it is the map. Radiotherapy uses that map to aim the treatment precisely. Having both is not a duplication; it is how accurate, targeted treatment works. Many people also have a PET-CT after radiotherapy to check whether the treatment has worked, which again is a diagnostic step, not more treatment.

After radiotherapy, am I radioactive?

After external beam radiotherapy — the most common type — no. The machine produces a beam that passes through your body during treatment; it does not leave any radioactive material inside you, and you are not a source of radiation when you leave the room. If you are having brachytherapy, which places a sealed radioactive source inside or near the tumour, your team will give you specific guidance about close contact with others. Ask your team which type of radiotherapy you are receiving so you know which advice applies.

Can having many PET-CT scans increase my cancer risk?

Any exposure to ionising radiation carries a very small theoretical risk, and ICRP acknowledges this. At the dose levels used in diagnostic scanning, that risk is considered very low and is consistently judged by ICRP and IAEA to be outweighed by the clinical benefit of the information the scan provides. If you are concerned about the number of scans you are having, ask your oncologist how many are planned, why each one is needed, and whether any could be replaced by an alternative that uses less or no radiation.

What is the difference between nuclear medicine and radiotherapy?

Nuclear medicine — which includes PET-CT and other imaging procedures — uses small amounts of radioactive substances to create images of what is happening inside your body. Radiotherapy uses high-energy radiation specifically to damage and destroy cancer cells. Some nuclear medicine treatments, such as radioiodine for thyroid cancer, use radioactive substances therapeutically rather than just for imaging, which is where the lines can seem blurry. If your team mentions a radioactive treatment, ask them to explain whether it is nuclear medicine therapy, brachytherapy, or external beam radiotherapy — each is managed differently and carries different precautions.
