PET-CT With a Pacemaker, — Stent or Metal Implant

Many patients are told they cannot have MRI because of a pacemaker or metal implant — and they assume the same restriction applies to PET-CT. It usually does not. PET-CT and MRI are physically different scans, and the metal rules that apply to one do not transfer to the other.

A pacemaker, stent, joint replacement or other metal implant is not a barrier to having a PET-CT scan. Unlike MRI, PET-CT does not use a magnetic field. Metal cannot be moved or heated by the scan. Tell your imaging team what you have and they will note it — but the implant alone is very rarely a reason to cancel.

What do these terms mean?

PET scan
A scan that uses a small amount of radioactive sugar injected into a vein. It shows where cells in the body are unusually active.
CT scan
A scan that uses X-rays to produce detailed cross-sectional images of the body's internal structures.
PET-CT
Both scans combined in a single session. The PET shows metabolic activity and the CT shows anatomy. Together they help locate and stage cancer precisely.
Radiotracer
The mildly radioactive substance injected before the PET scan. It leaves the body within hours, mainly through urine.
Imaging artefact
A shadow, streak or distortion on a scan image caused by something — like dense metal — that interferes with the X-ray signal. It affects appearance, not safety.
MRI
A completely different scan that uses a very powerful magnetic field. This is why metal restrictions for MRI are strict — and why those restrictions do not apply to PET-CT.

What to tell the team before your scan

  • Name every implant you have — pacemaker, ICD, stent, joint replacement, surgical clips, rods or screws — and the year it was fitted.
  • Bring your pacemaker or cardiac device card if you have one. The make and model helps the team document the device and account for artefact on the images.
  • Mention removable metal — jewellery, piercings, partial dentures — so the team can advise you before you arrive.
  • Tell the team about any metal fragments from old injuries, even if you were told years ago they were not a concern.
  • Tell them if you are pregnant or may be pregnant. PET-CT uses ionising radiation and is not performed during pregnancy.
  • If you are breastfeeding, ask the team how long to pause feeds after the radiotracer injection — the guidance depends on the specific tracer used.

Does metal affect the quality of your images?

The CT part of a PET-CT uses X-rays. Dense metal absorbs X-rays differently from soft tissue, which can create a shadow or streak around an implant on the CT images. Radiologists call this beam-hardening artefact.

For most oncology scans, the area of clinical interest is far enough from the implant that artefact has no practical effect on the result. Where the implant is close to the area being assessed, your radiologist will document this explicitly in the report.

The PET part of the scan — the part that shows where cells are unusually active — is not meaningfully affected by metal. If the key finding is on PET, artefact on CT rarely changes what the scan can show.

Your referring doctor and the imaging centre will confirm whether the scan is appropriate for your specific situation before you proceed.

Questions about specific implants

I have a pacemaker. Is PET-CT safe for me?

Yes, in almost all cases. A pacemaker is an electronic device, not a ferromagnetic object, and PET-CT does not generate a magnetic field at any point. The scan does not produce the kind of energy that interferes with pacemaker circuits. Tell your imaging team the make and model if you have it — this allows them to document the device and account for any artefact near the chest on the CT images. Your cardiologist's clearance is not routinely required for PET-CT the way it is for MRI.

I have a coronary stent. Will the scan be accurate in that area?

Coronary stents are small and produce minimal artefact on the CT portion of a PET-CT. The PET component, which shows metabolic activity, is not significantly affected by metal. Your radiologist will note the stent when reading the scan. If the scan is being done specifically to assess the heart or coronary arteries, mention this to your oncologist beforehand — cardiac PET uses different protocols and tracers from the standard oncology scan.

I had a hip or knee replacement. Will the metal cause problems?

Large joint replacements do create beam-hardening artefact on the CT portion — a shadow or streak spreading around the implant. For most oncology scans, the area of clinical interest is far enough from the joint that this makes no practical difference to the result. Where the joint is in or near the area being assessed, your radiologist will document the artefact and take it into account. If the joint is directly relevant to what the scan is looking for, discuss this with your oncologist before booking.

I have dental implants or a metal crown.

Dental metal — implants, crowns, bridges, metal braces — creates a small, localised artefact in the head and neck region on CT. For PET-CT scans of the chest, abdomen or pelvis, this does not matter at all. For head and neck scans, your radiologist will account for it and document it in the report. If you have removable dental hardware, some centres ask you to take it out before the scan — the team will advise you when you book.

I have surgical clips from a previous operation.

Surgical clips are very common after abdominal, thoracic or gynaecological operations and are routinely seen on PET-CT without issue. They are small and create only a minor, localised artefact on CT. Your radiologist will note them when interpreting the images. The main reason to mention them is so the report accurately documents what is hardware and what is not — this matters most when the clips are in or near the area being assessed for cancer.

I was told I cannot have MRI because of my implant. Does that mean PET-CT is ruled out too?

No. MRI restrictions exist because of its powerful magnetic field, which can move, heat or interfere with certain implants and devices. PET-CT uses no magnetic field at any stage of the scan. If your cardiologist or implant surgeon said to avoid MRI, that advice applies only to MRI. You should still tell your PET-CT team about the device before the scan — not because it creates a safety risk, but so they can document it and interpret the images accurately.

Is PET-CT safe with a pacemaker?

The amount involved is small, the energy level low, and it is cleared from the body within hours through urine. If you have specific concerns about your device, your imaging team can explain what the tracer is and how it works before you proceed.

What if I have several metal implants from different operations?

Tell the team about each one individually — name the implant, where it is in the body, and when it was fitted. Multiple implants do not make PET-CT unsafe, but they may mean artefact appears in more than one area on the CT images. Your radiologist will document all of them when writing the report. If any implant is in or near the region being scanned, mention this specifically to your oncologist beforehand so the team knows what to expect when interpreting the images.

Is PET-CT safe during pregnancy?

No. PET-CT uses ionising radiation — from both the CT component and the radiotracer — and is not performed during pregnancy. If there is any possibility you are pregnant, tell the team before the scan. They will not proceed until pregnancy has been excluded.
