A PET Scan Cannot — Spread or Worsen Cancer

A PET scan shows what is already happening inside your body. It cannot make cancer spread, move or grow. If a scan has ever seemed to coincide with bad news, the scan revealed that news — it did not create it.

What do people believe — and what is actually true?

The injection can make the cancer spread.

The radiotracer is a sugar-like molecule that cancer cells absorb slightly more of than normal cells — that difference is what the scanner reads. It cannot cause cells to divide faster, move through the body or seed new tumours. No biological mechanism exists for this, and there is no clinical evidence it occurs.

The scan disturbs cancer cells and sets them loose.

A PET scan is entirely passive. The scanner reads signals from outside your body. Nothing in the process touches, stimulates or dislodges cells. The scan observes — it does not intervene.

My cancer got worse right after my scan, so the scan caused it.

Cancer that is actively growing does so continuously, often for weeks or months before a scan is ever ordered. A scan captures a picture at a single moment in time. If progression was found, it had already been building — the scan did not start it.

The radiation from the scan stays in your body and keeps affecting the cancer.

The radiotracer used in PET scans has a short half-life — its radioactivity reduces rapidly on its own — and it is also eliminated through your urine over a few hours. There is no lasting radiation in your body after the scan is done.

What actually happens during a PET scan

  • A small amount of radiotracer — a sugar-like molecule — is injected into a vein.
  • You rest quietly for about an hour while the tracer moves through your body.
  • Cancer cells, which use more energy than most normal cells, absorb slightly more of it.
  • The scanner detects where the tracer has concentrated and builds an image from that.
  • The scanner emits nothing into your body — it only reads signals from outside.
  • The tracer leaves your body through your urine over the next few hours.

Why do some patients seem to get worse after a PET scan?

The scan does not cause the worsening — it reveals it. Cancer that is actively growing does so for weeks or months before the scan is ordered. The result arrives at a particular moment, but the process behind it started long before.

This is one of the most painful coincidences in cancer care. A family sees a loved one go for a scan, and then hears that the disease has progressed. The sequence feels like cause and effect. It is not.

A PET scan is ordered precisely because the team wants to know what the disease is doing. If it shows progression, that information is what changes the treatment plan — which is exactly the purpose of the scan.

Where does the belief that scans spread cancer come from?

The fear is understandable. The word 'radiation' is alarming, an injection you cannot see is unsettling, and a scan almost always comes before a difficult conversation. The mind looks for a cause, and the scan is the most recent visible event.

In many communities there is also a belief that cancer should not be 'disturbed' — that investigating it or scanning it can wake it up or set it moving. This is deeply felt, and the feeling behind it deserves respect even when the biology is different.

If you or your family carry this concern, tell your treating team. They will not dismiss it. Hearing directly from your doctor what happens in the scan room — in plain terms — often settles the question more fully than anything written can.

WHO and IAEA guidelines classify PET scans as safe diagnostic imaging. NCCN and ASCO recommend them for staging and response assessment in many cancers because the clinical value of accurate information outweighs the minimal risk from the scan itself.

The decision to order a scan is always a clinical one: the team asks whether the information it gives will change what they do next. If it will, the scan is worth doing.

Is the radiation in a PET scan dangerous?

The radioactivity used is small and chosen to give enough information to be clinically useful while staying within levels that regulatory bodies consider safe for diagnostic use. The tracer also leaves your body within hours, which limits total exposure. WHO and IAEA guidelines classify PET scans as safe. Your oncologist orders one when the information it provides is important enough to guide the next treatment decision — that assessment is always the starting point.

Why did my cancer appear to have spread after my PET scan?

Because the scan showed what was already there. A PET scan cannot create new disease — it detects existing disease, including deposits that may have been too small to see on earlier scans or not examined before. If cancer had grown, that growth had been happening before the scan was done. The timing of the scan and the timing of the news coinciding does not make the scan responsible.

Should I refuse a PET scan if I am worried it will make things worse?

Refusing a scan your oncologist has recommended means making treatment decisions without the information it would provide — and that is a real risk to your care. The scan itself carries no risk of worsening your cancer. If you have this concern, raise it directly with your oncologist before the scan. A conversation about why it has been ordered and what it will change in your treatment often resolves the worry in a way that written reassurance cannot.

How long does the radiotracer stay in my body?

The tracer used in most PET scans has a short half-life — its radioactivity reduces rapidly on its own — and your body also clears it through urine over a few hours. By the end of the day of your scan, the vast majority of the radioactivity is gone. Your team may ask you to drink plenty of water afterward and to avoid prolonged close contact with young children or pregnant people for a few hours as a routine precaution.
