Does a positive PET scan always mean cancer?

A normal PET is reassuring, but it does not rule cancer out. Some tumours are too small, too slow-growing, or the wrong cell type to appear strongly. Your team reads the scan alongside your symptoms, blood tests, and clinical history.

A PET scan can tell you what type of cancer you have.

PET shows metabolic activity, not cell identity. The type of cancer — which determines your entire treatment plan — can only be identified from a biopsy and pathology report.

What else makes a PET scan light up?

Infection, inflammation, surgical healing, and normal organ activity all appear bright on a PET scan, for the same reason cancer does — they absorb more glucose than the surrounding tissue. The scan cannot tell one from another.

In India, where tuberculosis is widespread, TB-affected lymph nodes are one of the most common causes of a positive PET that turns out to be non-cancerous on biopsy. Fungal infections, sarcoidosis, recent vaccination sites, and granulomas can all produce the same picture.

This is the scan working as it should. Identifying areas of high metabolic activity is exactly what it is designed to do. Determining whether that activity is cancer is the job of the biopsy that follows.

What do the words in your PET scan report mean?

FDG uptake / SUV
A measure of how much glucose a tissue is absorbing. A high value is suspicious, not diagnostic — infections and inflammatory conditions produce equally high readings. Your oncologist interprets this number in the context of your full clinical picture.
False positive
A scan finding that looks abnormal but is confirmed to be non-cancerous by biopsy. False positives are well-documented in PET imaging, which is why NCCN and ASCO require biopsy confirmation before any cancer treatment begins.
Biopsy
Removal of a small tissue sample for examination under a microscope. It is the only test that confirms whether cells are cancerous and, if so, what type. No treatment decision should be made without one.
Pathology report
The laboratory result from examining your biopsy sample. It identifies the cell type, confirms whether cancer is present, and guides every treatment decision that follows.
PET-CT
A combined scan pairing the metabolic picture from PET with the anatomical detail from CT. It locates any bright area precisely but still cannot distinguish cancer from infection — that requires a tissue sample.

Frequently asked questions

How often does a PET scan give a false positive for cancer?

The rate varies considerably by cancer type, body region, and patient population. In India, where TB and granulomatous infections are common, false positives from infectious causes are particularly well documented. NCCN and ASCO guidance consistently requires biopsy confirmation of any suspicious PET finding before treatment, because false positives are frequent enough to change management. A single percentage figure would not apply to your specific finding — ask your oncologist what the likely cause is and what investigation comes next.

My scan report says 'suspicious uptake' — does that mean I have cancer?

'Suspicious uptake' means the radiologist found an area of increased activity that warrants further investigation. It is not a cancer diagnosis. The next step is usually a biopsy, or in some situations a repeat scan after a short interval if your team suspects an infection that may resolve on its own. Ask your oncologist what the specific finding means and what test comes next — that conversation will give you a real answer where the report alone cannot.

My oncologist wants a biopsy after my positive PET — is that really necessary?

Yes, and it is standard practice under NCCN and ASCO guidelines. A positive PET alone is not sufficient to start cancer treatment. The biopsy confirms what the scan cannot: whether the cells are cancerous and, if so, what type. That information determines the treatment — the drugs, the radiation approach, the surgical plan. Proceeding without a biopsy carries a real risk of treating the wrong condition entirely.

Can a PET scan miss cancer?

Yes. Some cancers do not absorb glucose strongly enough to appear clearly — certain lymphoma subtypes, lobular breast cancer, and many slow-growing cancers can produce a near-normal scan. A normal PET is useful information, but your team will weigh it alongside your symptoms, blood results, and clinical examination. If suspicion remains after a normal scan, your oncologist may still recommend further investigation.

We are waiting for biopsy results after a positive PET — what should we expect?

Biopsy results typically take one to two weeks, depending on the tests requested and the laboratory. Some samples need additional staining or molecular analysis and take longer. While you wait, avoid making decisions based on the scan report alone — it describes what is active, not what it is. Your oncologist will discuss the pathology report with you as soon as it is ready, and that conversation is the one that determines next steps.
