Myth: Annual PET Scans — Are Not a Safety Net

The belief that a yearly PET scan will protect you from cancer is understandable — and it is not supported by evidence. No major cancer guideline recommends it for healthy people, and the scan carries harms that are easy to overlook when you are frightened.

Are annual PET-CT scan packages actually a good idea?

A PET scan every year will catch my cancer before it spreads.

No international guideline recommends annual PET-CT for people without symptoms or a known cancer. The scan was developed to stage known disease and assess treatment response. NCCN, ASCO and ESMO have not endorsed it as a population screening tool, and it has not been shown to reduce cancer deaths in that setting.

There is no harm in scanning just to be sure.

Each PET-CT delivers ionising radiation from both the radioactive tracer and the CT component. False positives — findings that look suspicious but are not cancer — are common. Each false alarm starts a chain of further scans and often a biopsy, which carries real risks. That harm is not trivial.

If I can afford it, why not do it?

Whether you can pay for a scan and whether it will help you are separate questions. A scan that produces a false alarm can lead to procedures carrying real risks, and time spent investigating a harmless finding is time not spent on care that is actually indicated for you.

I had cancer — I should scan every year to catch it coming back.

Surveillance after treatment follows schedules specific to your cancer type, stage and treatment received. Those schedules reflect when recurrences are detectable and when finding them changes the outcome. Scanning more often than your oncologist recommends does not improve results and adds radiation exposure and the risk of false findings.

What harm does annual PET-CT scanning actually cause?

Each PET-CT scan involves ionising radiation — from the radioactive glucose tracer and from the CT component. Annual scanning means annual radiation exposure with nothing proven to offset that risk over time.

False positives are the more immediate problem. When a scan finds something suspicious, further imaging and often a biopsy follow. Biopsies carry real risks: infection, bleeding, and occasionally serious complications depending on where the tissue comes from. All of this can follow a finding that turns out to be harmless.

Overdiagnosis is the third harm: finding abnormalities that would never have grown into a cancer causing harm, but that look identical on a scan to one that would. Treating an overdiagnosed finding is not caution — it is harm that would not have happened without the scan.

What does evidence-based cancer screening actually look like?

Evidence-based screening means tests shown, in large clinical studies, to reduce the chance of dying from a specific cancer. In India, these include cervical cancer screening through HPV testing and cytology, mammography for breast cancer detection, and oral cancer screening for people who use tobacco — one of the highest-risk groups in India.

If you have had cancer, ask your oncologist for your surveillance schedule in writing and follow it. Contact your team if something changes between appointments — not to book a scan, but to describe what you have noticed so they can advise.

A new symptom — unexplained weight loss, a lump, persistent pain, or blood where there should not be blood — needs reporting the day it concerns you. Do not wait for an annual scan date.

What should you actually do if you are worried about cancer?

  • Ask your doctor which cancer screening programmes apply to you by age, sex and risk factors — they vary, and not all apply to everyone.
  • If you use tobacco, attend an oral cancer check-up — this is among the few evidence-based screening steps for high-risk people in India.
  • If a close relative has had a cancer linked to a hereditary pattern, ask about genetic counselling rather than whole-body scanning.
  • If you have had cancer before, follow your oncologist's surveillance schedule and keep every appointment.
  • Report any new symptom to your doctor the day it worries you — do not wait for a scan date.
  • If a clinic has offered you an annual scan package, ask your oncologist whether it is clinically indicated for you before booking.

Frequently asked questions

Why do clinics offer annual PET-CT packages if they are not recommended?

Commercial scan packages are a business product — their existence does not mean the scan is clinically appropriate for you. A PET-CT scanner is expensive equipment, and packages help cover that cost. The question is not whether a scan is available but whether it is indicated for your specific age, history and risk factors. CION does not offer annual whole-body PET-CT screening packages because the clinical evidence does not support them.

Can a normal PET-CT result give me peace of mind?

A normal result is not proof that cancer is absent. PET-CT does not detect all cancers reliably, and a new cancer can develop in the weeks or months after a normal scan. If anxiety about cancer is what is driving the request, a clinical risk assessment — looking at your family history, any symptoms and relevant markers — gives more meaningful information than a scan used outside its intended purpose.

Are there situations where regular follow-up scanning is actually recommended?

Yes. After treatment for certain cancers, periodic imaging is part of evidence-based surveillance. Recommended schedules vary by cancer type — some lymphomas, testicular cancers and selected lung cancers include CT or PET-CT at defined intervals for a defined period. Those schedules are not annual by default and are not indefinite. Ask your oncologist what applies to your situation and for how long.

I carry a BRCA variant — should I have annual whole-body scans?

No. BRCA variants raise the risk of breast and ovarian cancer, and surveillance for carriers is specific and evidence-based — but does not include annual PET-CT. NCCN guidelines for BRCA carriers typically include annual breast MRI and mammography from an age specific to your variant, alongside regular gynaecological assessment. A genetic counsellor or familial cancer specialist can map out what is appropriate for you.

What is PET-CT actually used for?

PET-CT has clear, evidence-supported uses: staging a cancer after a new diagnosis, assessing how well treatment is working, detecting recurrence when there is clinical reason to suspect it, and helping plan radiotherapy. It is a valuable tool in those situations. The concern is using it outside those settings, where the chance of a useful finding does not outweigh the radiation exposure and the risk of false alarms.
