What happens to your sample in the first hour?

  1. Sample removed

    Your biopsy sample is taken and immediately separated from the procedure site. From this moment, time matters for the tissue.

  2. Placed in fixative

    For most solid tissue biopsies, the sample goes straight into a small container of formalin. This chemical stops the cells from breaking down — a process called fixation.

  3. Container sealed and labelled

    The container is sealed and marked with your name, identification number, and the date and time of collection. This label stays with your sample through every step that follows.

  4. Request form completed

    A form describing your clinical history and what the pathologist needs to assess accompanies the sample. This guides the laboratory on which tests to run.

  5. Transported to the laboratory

    The sealed container is sent to a specialist pathology laboratory — often a partner facility — and travels at the correct temperature for the sample type.

  6. Received and logged at the lab

    On arrival, the laboratory checks the sample is intact, confirms the labelling matches the request form, and assigns your sample a unique case number. Processing then begins.

Frequently asked questions

Why does formalin matter — and will it affect the test results?

Formalin preserves tissue by locking its proteins in place, which is what allows the pathologist to cut very thin slices and examine the cells days later. Without it, the tissue would begin to break down within minutes of removal. The length of time a sample spends in formalin before processing also matters — both too little and too much fixation can affect how reliably certain biomarker tests read. This is why the exact time of collection is recorded on the container alongside your name.

What if my sample needs to be sent to another city for specialist testing?

Some tests — certain molecular panels or second-opinion reports — are performed at laboratories in other cities. The sample travels as a processed paraffin block or as glass slides rather than as fresh tissue, which means it can be shipped safely without a new biopsy. Your treating team coordinates this referral. Ask your oncologist if any part of your testing is going elsewhere, so you know which results may take longer to return.

How long is my biopsy sample kept after testing is finished?

Pathology departments retain biopsy material for a prolonged period after testing — in most Indian institutions, the processed paraffin block is kept for years rather than months. This is intentional. If your treatment changes, if a second opinion is needed, or if new biomarker tests become relevant later, the original block can be retrieved and retested without another procedure. Ask the pathology department at your treating hospital to confirm their specific retention policy.

Can a biopsy sample ever be lost or mixed up with someone else's?

Laboratories follow strict chain-of-custody steps specifically to prevent this. Every container is labelled at the point of collection and cross-checked against the request form when it arrives at the laboratory. Accreditation standards from bodies such as the College of American Pathologists require these checks as a condition of accreditation. If you ever receive a result that seems inconsistent with your clinical picture, it is reasonable to ask your oncologist to verify that the sample identity was confirmed.

What is a frozen section — is it different from a routine biopsy?

A frozen section is used during surgery when a result is needed within minutes — for example, to check whether a tumour margin is clear before closing. Instead of formalin, the tissue is rapidly frozen and sliced immediately for the pathologist to read. The turnaround is fast but the image quality is lower than a standard fixed sample, so it is reserved for specific surgical decisions. Most diagnostic biopsies — the ones taken before treatment is planned — use standard formalin fixation, not freezing.
