Most Stage I–II
Lumpectomy / Breast-Conserving Surgery
Removes the tumour and a margin of surrounding tissue while preserving the breast — the standard of care for most Stage I–II breast cancers where the tumour-to-breast ratio allows adequate margins and an acceptable cosmetic result.
Larger tumours, BCS feasible
Oncoplastic Surgery
Combines cancer resection with immediate breast reshaping — allows wider excision margins than standard lumpectomy while achieving a better cosmetic outcome; appropriate when a larger portion of the breast requires removal but the breast can be conserved.
Lumpectomy not appropriate
Simple Mastectomy
Removes the entire breast — recommended when lumpectomy is not clinically appropriate (e.g., multiple tumours, large tumour-to-breast ratio, inability to achieve clear margins). Chest muscle is preserved. Lymph nodes are not removed unless sentinel biopsy is indicated.
When reconstruction is planned
Skin-Sparing / Nipple-Sparing Mastectomy
Preserves the skin envelope and, where oncologically safe, the nipple-areola complex — creating a better foundation for immediate reconstruction; assessed case-by-case by the tumour board based on tumour location and margin safety.
Lymph node involvement
Modified Radical Mastectomy (MRM)
Removes the breast, overlying skin, and axillary lymph nodes — recommended when lymph nodes are involved. Chest muscle is preserved (unlike the older radical mastectomy). The standard surgical procedure when axillary clearance is required.
Immediate or delayed
Breast Reconstruction
Immediate reconstruction (at the same time as mastectomy) or delayed reconstruction — using implant, tissue flap, or nipple-sparing techniques, depending on anatomy, oncology plan and whether post-mastectomy radiation is planned; discussed during your surgical consultation.

Is breast-conserving surgery always possible?
Lumpectomy is the standard of care for most Stage I–II breast cancers where the tumour-to-breast ratio allows adequate margins and an acceptable cosmetic outcome. It is not possible when the tumour is too large relative to breast size, when margins cannot be achieved, or when multiple tumours are present. Your surgical oncologist will assess your reports and scans to determine which approach is appropriate for your case.
What is the difference between MRM and a simple mastectomy?
A simple mastectomy removes the entire breast. A modified radical mastectomy (MRM) removes the breast, overlying skin, and the axillary lymph nodes — recommended when lymph nodes are involved. Both preserve the chest muscle (unlike the older radical mastectomy). Your surgical oncologist will recommend the appropriate procedure based on your reports and lymph node involvement.
Can I have reconstruction at the same time as mastectomy?
Immediate reconstruction (at the same time as mastectomy) is possible and is often discussed during your surgical consultation. The type of reconstruction — implant, tissue flap, or nipple-sparing — depends on your anatomy, oncology plan, and whether post-mastectomy radiation is planned. The tumour board considers all factors before a recommendation is made. Delayed reconstruction (done months after mastectomy) is also an option.
What is recovery like after breast cancer surgery?
Lumpectomy is typically day care to one overnight; light activity resumes in 1–2 weeks. Mastectomy and MRM require 1–3 nights in hospital with a drain in place for 1–2 weeks post-discharge. Your surgical team will give you written post-operative instructions and a follow-up schedule before you leave. Post-surgical adjuvant therapy (chemotherapy, radiation, hormone therapy) is planned by the tumour board and explained before discharge.
Can I get a second opinion on my surgery recommendation?
Yes. CION actively encourages second opinions. Bring your reports from any hospital — our tumour board will review them and give you a written opinion on whether surgery is indicated, which procedure is appropriate, and what the expected cost is at CION. There is no fee for this written second opinion. You are under no obligation to proceed with treatment at CION.
