Stage 4 breast cancer — also called metastatic breast cancer — means the cancer has spread from the breast to another part of the body. Common sites include the bones, liver, lungs, and brain. This stage is also called advanced or secondary breast cancer.
Hearing the words "stage 4" is one of the hardest moments a person can face. We want you to hear this clearly: stage 4 breast cancer is treatable. HER2-positive breast cancers respond well to targeted therapy — medicines that go after a specific protein on cancer cells. This approach is more precise than chemotherapy and can be very effective even at stage 4. Your HER2 status is confirmed from your biopsy report.
Chemotherapy travels through the bloodstream to reach cancer cells anywhere in the body — making it particularly useful for widely spread disease. For triple-negative breast cancer (where hormone therapy and HER2 targeting are not options), immunotherapy is a newer approach that helps your own immune system find and attack cancer cells. Radiation is used at stage 4 to treat specific metastatic sites — for example, a bone lesion causing pain, or a brain metastasis. Knowing what to ask
Not all stage 4 breast cancers behave the same way. These six factors shape which treatments work best for you — and why two patients with the same diagnosis may receive very different plans.
Your biopsy report states whether the cancer cells are positive or negative for oestrogen receptors (ER) and progesterone receptors (PR). If positive, the cancer is driven at least partly by hormones — and hormone-blocking therapy is a very effective first-line option with generally fewer side effects than chemotherapy. If both receptors are negative, hormone-blocking therapy will not help, and the plan shifts to other approaches. This is one of the first things your oncologist confirms before recommending any treatment at stage 4.
HER2 is a protein that promotes cell growth. If your cancer overexpresses HER2 (HER2-positive), targeted therapy directed at this protein can be highly effective — even at stage 4. HER2-positive metastatic breast cancer has seen some of the biggest improvements in outcomes over the last decade, thanks to advances in targeted treatment. Knowing your HER2 status from your biopsy or liquid biopsy is therefore essential before any treatment plan is finalised.
Where the cancer has spread matters. Bone metastases are common and can be managed with systemic therapy plus bone-protective medicines; pain is often well controlled. Liver metastases may need faster-acting systemic therapy. Brain metastases may require radiation to that specific site in addition to systemic treatment. Your oncology team uses PET-CT or CT scans to map all sites before planning, so each one is accounted for — not just the most obvious lesion.
If you have had previous treatment for early-stage breast cancer — surgery, chemotherapy, or radiation — your oncologist needs to know exactly what was used and how your body responded. Some treatments cannot be repeated because of cumulative side effects on the heart or other organs. Others may be reintroduced if enough time has passed. Your treatment history also tells the team which cancer cells may have developed resistance, helping them choose therapies that are more likely to work at this stage.
Some treatments — particularly chemotherapy — place demands on the liver, kidneys, and bone marrow. Blood tests before and during treatment check that these organs are handling the therapy well. If there is any compromise, the plan is adjusted to protect your organs while still targeting the cancer as effectively as possible. This is not a barrier to treatment — it is good oncology practice. Your functional health (how well you can carry out daily activities) is also considered when choosing between equally effective options.
