Why does the bladder glow so brightly on a PET scan?
The tracer used in PET scanning — called FDG — is a form of glucose that cancer cells absorb more actively than normal cells. After the injection, your kidneys filter it from the blood into urine, just as they would any other substance your body needs to remove.
As the scan progresses, that radioactive urine collects in the bladder. The bladder itself is not metabolically active, but the concentrated tracer inside it creates an intense signal that can appear as bright as any tumour on the image.
This happens in every patient. It is not a sign that something is wrong with your scan or your body.
What does your team do to reduce the bladder signal?
Drink water as instructed before your appointment
Water dilutes the tracer in your urine and encourages the kidneys to keep flushing it through, reducing how concentrated the bladder signal becomes.
Empty your bladder just before the scan begins
Voiding immediately before lying on the scanner removes most of the concentrated tracer that has built up, giving the radiologist a clearer view of pelvic structures from the start.
Delayed pelvic images are taken after a second void
For gynaecological cancers, the pelvis is often imaged a second time after you have voided again partway through the appointment. The gap allows more tracer to clear from the bladder.
CT images are read alongside the PET
The CT component of PET-CT shows anatomy without tracer interference. The radiologist uses CT to identify structures in areas where the PET signal is too intense to assess on PET alone.
Can bladder activity actually hide cancer near the pelvis?
It can make nearby structures harder to see — and that is a limitation the radiology team accounts for rather than ignores.
Lymph nodes and soft tissue immediately adjacent to the bladder are the most affected. A small node pressed close to the bladder wall may not be clearly visible on PET alone, which is exactly why the CT images and delayed acquisitions are part of the standard protocol for pelvic cancers.
If your report notes that pelvic assessment was limited by bladder activity, that is an honest finding. Your oncologist will factor it into the overall clinical picture alongside your examination and other investigations.

Does the bright bladder mean my scan result could be wrong?
PET-CT is a guide to where disease is likely to be, not a definitive diagnosis on its own. Any finding that will affect your treatment needs to be confirmed — usually by biopsy — before a major decision is made.
The bladder limitation is one of several reasons PET results are read by experienced radiologists alongside your full clinical history. A bright area near the bladder that could be disease or could be concentrated tracer is a reason for further assessment, not an immediate answer.
If you are pregnant or think you might be, tell your team before the scan is booked. FDG PET-CT is not performed during pregnancy.
Terms you may see in your PET-CT report
- FDG
- Fluorodeoxyglucose — the radioactive tracer injected before a PET scan. It behaves like glucose in the body and is excreted in urine, which is why the bladder and kidneys always show activity.
- Physiological uptake
- Normal tracer activity in organs that are not diseased. The bladder, kidneys, brain and heart all show physiological uptake on every scan. The radiologist distinguishes this from abnormal uptake.
- Delayed imaging
- A second set of PET images taken after the patient has voided again, usually focused on the pelvis. The additional time allows more tracer to clear from the bladder.
- SUV
- Standardised uptake value — a number describing how intensely an area absorbs the tracer. High SUVs in the bladder are expected on every scan and do not indicate cancer there.
- Tracer artefact
- A bright area on PET caused by concentrated tracer rather than disease. Radiologists are trained to identify these and distinguish them from genuine abnormal uptake.
Did you know?
For pelvic cancers such as cervical and endometrial cancer, the proximity of tumour sites to the bladder is one of the main technical challenges of PET-CT staging.
Delayed pelvic acquisition after voiding is a recognised technique in international imaging guidelines for improving diagnostic accuracy in this region.

Is the bright bladder on my PET scan something to worry about?
No. The bladder lights up on every PET scan because the tracer is excreted in urine and collects there. It is not a sign of bladder cancer or any other abnormality. Your radiologist expects it and is trained to read around it. If the scan had found something genuinely concerning in or near the bladder, the report would describe it specifically.
What does it mean if my report says pelvic assessment was limited?
It means the bladder signal made one or more nearby areas harder to assess with certainty on PET alone. This is an honest acknowledgement of a known limitation, not a failed scan. Your oncologist will read it alongside your CT images, examination findings and any biopsy results to build the full picture. In some cases a further investigation — MRI or a repeat PET with a modified protocol — may be recommended.
Why do I have to empty my bladder just before the scan?
Voiding immediately before the scan removes most of the concentrated tracer that has built up in the bladder. This reduces the bright signal and makes structures nearby easier to see. For pelvic cancers you may be asked to void a second time partway through the appointment so that delayed images of the pelvis can be taken with a lighter signal. Following the preparation instructions exactly is one of the most straightforward ways to improve image quality.
Could a small lymph node near the bladder be missed on the scan?
A very small node immediately adjacent to the bladder can be harder to assess on PET alone — this is a known limitation that radiologists account for. The CT component of PET-CT shows anatomy clearly regardless of tracer activity, and is read alongside the PET for exactly this reason. If there is genuine uncertainty about a specific area, your team will recommend further imaging or assessment rather than leaving the question open.
Can I have a PET scan if I am pregnant?
No. FDG PET-CT is not performed during pregnancy. The radioactive tracer crosses the placenta and the radiation involved is not acceptable for a developing baby. If there is any possibility you are pregnant, tell your team before the scan is booked. Alternative imaging methods can be discussed depending on what your oncologist needs to assess.
Will the radiologist's report mention the bladder every time?
Usually not if the bladder activity is straightforward and has not affected the assessment. If it has made a specific pelvic area harder to evaluate, the report will say so clearly. Reading 'physiological bladder activity' in a report is routine. Reading 'pelvic assessment limited due to bladder activity' in a particular region means the radiologist found genuine uncertainty there and is being transparent about it — that is useful clinical information, not bad news about the scan.
