Must you pause breastfeeding after a PET-CT scan?
Yes. This is not a precaution you can weigh up — it is a safety requirement.
PET-CT uses a radioactive tracer called FDG. After it is injected, a small amount passes into your breast milk. Giving that milk to your baby exposes them to radioactivity they do not need.
Tell the nuclear medicine team that you are breastfeeding before your appointment. They will confirm the exact pause duration for the tracer being used and help you plan around it.
How long do you need to pause — and can you hold your baby?
IAEA and SNMMI guidance recommends pausing breastfeeding for approximately 12 hours after an FDG-PET-CT scan.
The FDG tracer has a short radioactive half-life. This means the radioactivity in your body falls significantly within hours, which is why the pause is measured in hours rather than days.
You can hold, cuddle, and carry your baby throughout this period. The restriction is on feeding — not on closeness. Skin-to-skin contact during the pause is fine.
What do these terms mean?
- FDG (fluorodeoxyglucose)
- The radioactive tracer injected before a PET scan. It is a form of glucose that the body — and tumours — absorb, making metabolically active areas visible on the scan. It is the FDG in your blood and milk that creates the temporary feeding restriction, not the CT portion of the scan.
- Radiotracer
- A substance that carries a small radioactive signal so a scanner can track where it goes in the body. The dose is calculated to be as low as is needed to produce a clear image.
- Half-life
- The time it takes for half of a radioactive substance to break down. FDG uses a form of fluorine with a short half-life, so the radioactivity falls quickly — which is why the breastfeeding pause is hours, not days or weeks.
- Expressed milk
- Milk pumped or hand-expressed before the scan and stored safely. Milk expressed before your tracer injection is completely safe — the radioactive substance has not yet entered your body.

How to prepare before your PET-CT appointment
- Tell the nuclear medicine team you are breastfeeding when you book, and again on the day.
- Express and freeze at least 12 hours' worth of milk in the days before your scan.
- Arrange a carer to feed your baby during the pause window after the scan.
- Bring bottles and expressed milk if your baby will be with you after the appointment.
- During the pause, keep expressing and discard that milk — this protects your supply.
- After 12 hours, resume breastfeeding as normal.
Is PET-CT the right scan for breast cancer?
PET-CT is not recommended for early-stage breast cancer. NCCN and ESMO guidelines do not support its routine use in early disease, and it does not replace standard imaging such as mammography, ultrasound, or MRI.
Breast tissue that is actively producing milk also absorbs FDG naturally, which can make PET images harder to interpret — another reason your team needs to know your feeding status before the scan is booked.
PET-CT is used in breast cancer for specific situations: staging locally advanced or metastatic disease, and assessing how well treatment is working. If your oncologist has referred you, they have a clinical reason and can explain what the scan is expected to show.
Did you know?
Lactating breast tissue absorbs FDG — the same tracer tumours absorb — because it is metabolically active.
This can cause intense uptake in both breasts on a PET image, making it difficult to distinguish normal from abnormal tissue. It is one reason PET-CT is generally reserved for specific clinical needs in breastfeeding women, not used as a routine scan.

Can I express milk during the 12-hour pause and save it for later?
No. Milk expressed during the 12-hour pause contains FDG and must be discarded, not stored. It is still worth expressing during the pause to protect your supply — but that milk cannot be given to your baby. Only milk expressed before the tracer was injected, or after the 12-hour pause is complete, is safe to use.
What if my baby refuses formula or a bottle?
This is worth planning for well before your appointment, not on the day. If your baby is not used to a bottle, introduce one gently in the days beforehand. A familiar carer offering a bottle in a calm setting is more likely to work than trying it for the first time during a stressful pause window. Speak to your lactation consultant or midwife for practical strategies — this situation comes up often, and there are approaches that help.
Is it safe to hold or cuddle my baby after the scan?
Yes. The restriction is specifically on breastfeeding — not on physical closeness. You can hold, carry, and cuddle your baby normally throughout the pause window. The concern is the FDG in your milk being ingested, not proximity to you. The low level of radioactivity from your body falls quickly after injection, and skin-to-skin contact is fine.
Will the pause affect my milk supply?
The scan itself does not affect your supply. What can affect it is stopping expressing during the pause. If you express and discard milk throughout the 12-hour window, you keep the signal to your body to continue producing. Most women who resume feeding promptly after the pause do not notice a lasting change in supply.
Do I actually need a PET-CT scan for breast cancer?
Not necessarily, and not in early-stage disease. NCCN and ESMO guidelines do not recommend PET-CT as a routine scan for early breast cancer. It is used in specific situations — staging locally advanced or metastatic disease, or assessing treatment response — where the information changes clinical decisions. If you have been referred, ask your oncologist what the scan is expected to show and how it will affect your treatment plan.
How much milk should I express in advance, and how do I store it safely?
Aim to have at least enough to cover your baby's feeds for the full 12-hour pause. Building up a frozen supply in the days before your scan gives you time without pressure. Milk expressed before your tracer injection is completely safe to store. For guidance on how long expressed milk keeps in the fridge or freezer and how to thaw it safely, follow current WHO guidance or ask your lactation consultant — recommendations are updated periodically and your consultant will have the current advice.
