What does a cardiac PET viability scan actually show?
Some heart muscle that appears permanently damaged on an echocardiogram or stress test is actually still alive. It is not contracting because it is not receiving enough blood — a state cardiologists call hibernating myocardium. A viability scan distinguishes this living but dormant muscle from tissue that has genuinely scarred over.
The scan uses a glucose tracer. Living heart muscle cells take up glucose even when blood flow is reduced. Scar tissue does not. The resulting images show your cardiologist which regions are still metabolically active and which are not.
This is a cardiac imaging study, requested specifically to answer a question about your heart. It does not assess for cancer and should not be understood as one.
Why does your cardiologist need this result before bypass or stenting?
Bypass surgery and stenting both work by restoring blood flow to heart muscle. That is valuable when the muscle is still alive and capable of recovering. When the muscle has scarred, restoring blood flow to that area does not improve function — it exposes you to a significant procedure without the expected benefit.
ACC/AHA and ESC guidelines identify viability assessment as an important step before revascularisation in patients where the benefit is uncertain, because the scan can separate those likely to recover function from those who are not.
If viable muscle is found in the affected region, revascularisation is more likely to improve your heart function and symptoms. If scar is found, your cardiologist can consider other management approaches. Either result moves the plan forward on solid ground.

- Fast for as long as your appointment letter specifiesDo not eat or drink anything other than plain water during the fasting window. The exact duration will be stated in your written instructions.
- If you have diabetes, your preparation is different — read your instructions carefullyBlood sugar control before this scan directly affects image quality. If anything is unclear, call the scanning team before the day, not on arrival.
- Tell the team all your medications at the time of bookingParticularly any diabetes medicines. Some may need to be timed differently. Do not stop or change any heart medication without your cardiologist's agreement.

Is this the same PET scan used for cancer?
The machine and the glucose tracer are the same. What is different is the question being asked. In oncology, the scan looks for tissue that is abnormally metabolically active — a pattern cancer cells often show. In cardiac viability testing, the same glucose uptake is measured to find heart muscle that is still alive despite reduced blood flow. The technology is identical; the clinical purpose is entirely different. You are not having a cancer scan.
What happens if the scan shows my heart muscle has scarred?
Your cardiologist will have a clearer basis for the decision ahead. If the region of concern is significantly scarred, bypass surgery or stenting is unlikely to restore function in that area, and your team can consider other ways to manage your heart condition. Knowing this before a major procedure is protective, not discouraging — it steers the plan toward what is actually likely to help you.
How long will the cardiac viability scan take?
Cardiac viability studies involve rest imaging and typically take longer than a standard scan. Your appointment letter will give you a time estimate. Plan to be at the centre for longer than you might expect. The time is spent waiting for the tracer to distribute and then lying still for the scan — the scan itself is not painful.
Can I manage my blood sugar on my own on the day if I have diabetes?
Do not manage it on your own assumptions — follow your specific instructions exactly. Blood sugar levels before this scan affect the quality of the images your cardiologist will rely on to make a treatment decision. If your instructions seem unclear, or if your blood sugar is difficult to control on the morning of the scan, call the scanning centre before arriving. Arriving with poorly controlled blood sugar may mean the scan needs to be rescheduled.

The imaging team provides a report to your referring cardiologist. It is your cardiologist — the specialist who knows your full history — who will explain what the results mean for your treatment. Before you leave your next appointment, ask when to expect the report and when your follow-up will be. Do not leave without knowing the next step.
