Vulvar and vaginal cancers are rare, and good information about how they are staged is hard to find. PET-CT is part of the workup for both, and understanding what it can and cannot show will help you ask the right questions at your next appointment.

PET-CT is useful for both vulvar and vaginal cancer. It maps lymph node involvement and identifies spread to distant organs that a clinical examination cannot reliably confirm. Its main limit in the pelvis is the bladder: normal urinary tracer accumulation can obscure nearby nodes. Biopsy, not PET-CT, confirms the diagnosis.

Is PET-CT useful for vulvar or vaginal cancer?
Yes. PET-CT is used in the staging workup for both cancers, particularly when there is concern that disease may have spread beyond the primary site.
Both cancers spread first to lymph nodes — in the groin for vulvar cancer, and in the pelvis for vaginal cancer. PET-CT can detect involved nodes that feel normal on examination and are not clearly enlarged on a standard CT scan.
For vaginal cancer, NCCN guidance includes PET-CT as part of the initial staging assessment, particularly for women who are candidates for definitive radiation treatment. Finding spread early — or confirming that it has not occurred — changes the treatment plan in a meaningful way.

What are the limits of PET-CT in the pelvis?
The most important technical limit is the bladder. The tracer used in PET — FDG — is cleared by the kidneys into urine, so the bladder fills with intensely active signal.
This can make it harder to see lymph nodes that sit close to the bladder wall. Your team will ask you to empty your bladder immediately before the scan, which helps. Some ambiguity close to the bladder can still remain.
PET-CT also has a lower limit of detection. Microscopic disease and very small nodes with early involvement may show no activity even when cancer is present. A scan that appears clear does not guarantee that no spread has occurred.
Biopsy of clinically suspicious nodes remains part of the assessment regardless of what the PET-CT shows.
Who cannot have PET-CT, and what should you tell your team beforehand?
PET-CT uses a radioactive tracer and includes a CT component. It is not given during pregnancy.
Tell your team before the scan is booked if you are pregnant, think you might be, or are breastfeeding. The scan can be deferred or replaced with a suitable alternative.
You will also be asked to fast for several hours before the scan. The tracer competes with glucose, and eating beforehand affects image quality. Your team will give you specific instructions for your scan day.

Frequently asked questions
Can PET-CT see the tumour itself, not just the lymph nodes?
Usually yes, though the primary site is not always the most informative part of the scan for these cancers. The tumour at the vulva or vagina will generally show metabolic activity and can help your team understand how much local tissue is involved. The challenge is that the bladder — which sits very close — fills with high-signal tracer during the scan, and tissue immediately adjacent to it can be harder to interpret clearly. MRI is often used alongside PET-CT because it gives better local anatomical detail without the bladder interference.
What is the difference between PET-CT and MRI for these cancers?
They answer different questions. MRI gives better detail of the primary tumour — how deeply it has grown, whether it involves nearby structures such as the bladder or rectum. PET-CT gives a whole-body view and is more sensitive for detecting metabolically active lymph nodes and distant spread. Your team may request both for exactly this reason. If you have had only one, ask your oncologist whether the other is also needed for your situation.
Does a clear PET-CT scan mean the cancer has not spread?
No. A PET-CT that shows no abnormal activity means no spread was detected at this level of sensitivity — it does not exclude microscopic disease or involvement of very small nodes. The staging decision is made from the whole picture: your examination findings, your MRI, your PET-CT, and any biopsy results together. Your oncologist will explain what your specific result contributes to that assessment rather than reading it in isolation.
Can I have the scan during my period?
Yes. Menstruation does not affect PET-CT image quality or how the tracer is taken up. You do not need to delay the scan for this reason. If you have concerns about comfort or positioning on the day, let your team know beforehand and they will make adjustments.
Does PET-CT diagnose cancer or confirm what type it is?
No. PET-CT shows areas of high metabolic activity, which is common in cancer but also occurs in inflammation and some benign conditions. It cannot identify the cell type or confirm a diagnosis. That always requires a biopsy and a pathology report. PET-CT is ordered after the diagnosis is established, to determine how far the disease has spread and to guide treatment decisions.
How long does the scan take and will I feel anything?
The total appointment is usually around two hours, though most of that time is a waiting period after the tracer injection while it distributes through your body. The scan itself typically takes around twenty to thirty minutes. You will feel the tracer injection but the scan causes no pain or sensation. You will need to lie still during it. If lying flat for an extended period is difficult for you, tell your team before the appointment so they can plan accordingly.
