PET-CT is not done during pregnancy. The radioactive tracer crosses the placenta and exposes the developing baby to ionising radiation. There are safer imaging options that give your team the information they need.

Is PET-CT safe during pregnancy?
PET-CT is not performed during pregnancy. It uses ionising radiation from both the CT component and the radioactive tracer injected before the scan, and both reach the developing baby.
The tracer — a radioactive form of glucose — crosses the placenta and concentrates in fetal tissue. This makes the scan both unsafe and uninterpretable: tracer in normal fetal organs obscures the areas your radiologist needs to assess.
If you are pregnant or think you might be, tell your oncology team before any scan is arranged. This changes the imaging plan entirely.
What do these terms mean?
- Radiotracer (FDG)
- A mildly radioactive form of glucose injected before the scan. It concentrates in active cells — including cancer cells and, during pregnancy, fetal tissue.
- Ionising radiation
- The type of radiation that carries enough energy to damage DNA inside cells. Both the CT component and the radiotracer produce it.
- Placental transfer
- The way substances in the mother's blood pass into the baby's circulation. The radiotracer reaches the baby this way.
- Fetal radiosensitivity
- A developing baby's cells divide rapidly, making them more sensitive to radiation damage than adult cells — throughout pregnancy, not only in the first trimester.
What are the risks to my baby from PET-CT?
A developing baby's cells divide rapidly, which makes them more sensitive to ionising radiation than adult cells — at any stage of pregnancy, not only during the first trimester.
ICRP and WHO guidance both classify the developing fetus as a population requiring specific protection from ionising radiation. This is why PET-CT is considered contraindicated throughout pregnancy, not only in the early weeks.
The scan would also give unreliable results. Fetal tissue absorbs the radiotracer and appears active on the images, making it very difficult to distinguish normal fetal uptake from disease. The radiation exposure would not be balanced by reliable diagnostic information.

What scan is used instead of PET-CT during pregnancy?
MRI without contrast is the most commonly used alternative. It does not use ionising radiation and provides detailed images of soft tissue, including the pelvis and abdomen.
Ultrasound can be added where useful, particularly for pelvic assessment. Neither carries a radiation risk, and both can be used at any stage of pregnancy.
Your oncologist and radiologist will decide which combination gives your team the information they need. No imaging decision is made without considering both the cancer and the pregnancy.
Did you know?
There is no trimester of pregnancy when ionising radiation becomes safe for the developing baby. Fetal radiosensitivity persists from conception through full term — the brain and nervous system remain vulnerable long after early organ formation is complete.
This is why there is no safe timing for PET-CT in pregnancy. Waiting until the second or third trimester does not reduce the contraindication.

What if your situation is more specific?
I had a PET-CT scan before I knew I was pregnant. What should I do?
Contact your oncology team and obstetrician as soon as possible. They will arrange a medical physics dose assessment — a calculation of how much radiation the fetus actually received from that specific scan. In many cases the dose is lower than feared. The figure depends on your exact scan, equipment, and gestational age at the time. Do not try to estimate the risk from general information online; your team's dose assessment is the accurate and necessary starting point.
Can PET-CT be done after the first trimester, once organ development is complete?
No. The developing brain and nervous system remain sensitive to ionising radiation throughout pregnancy, not only during early organ formation. The scan also directly irradiates the mother's abdomen regardless of gestational age. ICRP guidance advises against PET-CT at every stage of pregnancy, not just in the first three months. MRI is appropriate and effective throughout and does not carry this limitation.
Will MRI give the same information as PET-CT for cervical cancer?
For most decisions made during pregnancy, yes. MRI shows the primary tumour, its local extent, and pelvic lymph node size well. It does not show metabolic tissue activity the way PET-CT does, and may not identify very small nodal deposits as reliably. Your oncologist will tell you specifically whether any gap in that information changes the treatment decision being made right now.
I am being treated for cervical cancer while pregnant. How will my cancer be assessed?
This is managed by a multidisciplinary team: your oncologist, a maternal-fetal medicine specialist, and a radiologist familiar with pregnancy imaging. MRI of the pelvis and abdomen is the main imaging tool. Treatment decisions — including those that involve the pregnancy — are made jointly by the team and discussed with you. Your oncologist will coordinate the right specialist support for your situation.
Do I need to stop breastfeeding after a PET-CT scan?
Yes, for a period after the scan. The radiotracer passes into breast milk, and a baby who feeds during this time would receive additional radiation. Your team will tell you exactly how long to pause — it depends on the specific tracer and dose used. Express and discard breast milk during the pause to maintain your supply. Do not resume before your team confirms it is safe to do so.
Is there any situation where PET-CT might still be considered during pregnancy?
In rare exceptional circumstances, if PET-CT would urgently change a treatment decision and no alternative imaging can provide that information, the fetal risk may be weighed against the risk of delaying treatment. This decision involves the oncologist, obstetrician, a medical physicist calculating the estimated fetal dose, and the patient. It is reviewed at a multidisciplinary meeting before any decision is made — not by a single clinician acting alone.

PET-CT can be arranged after delivery when it is clinically needed. There is no fixed waiting period after birth itself — timing depends on your clinical situation and how urgently the information is required. If you are breastfeeding, your team will advise a specific pause after the scan before you resume. Discuss timing at your first post-delivery appointment with your oncologist.
