Why is a biopsy always required even after a clear PET-CT?
PET-CT measures metabolic activity — it highlights tissue that is more active than the area around it. It cannot tell you whether that activity is cancer, tuberculosis, a fungal infection, or another inflammatory process.
In Hyderabad and across Telangana, TB is common enough that any lung mass showing high uptake on PET must be treated as possibly TB until a biopsy proves otherwise. Treating lung cancer in someone who has active TB, or missing active TB entirely, causes serious harm.
A biopsy is always required before a lung cancer diagnosis is confirmed and before treatment begins. No scan result, however clear it looks, is sufficient on its own.
What determines how much the staging workup costs in total?
Does health insurance cover PET-CT for lung cancer staging?
Most major health insurance policies in India cover PET-CT as a diagnostic investigation for cancer staging. Cashless claims need pre-authorisation — your oncologist's letter stating the clinical reason is usually what the insurer requires. Reimbursement-based policies require you to pay first and claim with receipts. Check your policy for diagnostic sub-limits, because some policies cap total diagnostic reimbursement separately from the overall insured sum. Confirm coverage before you book rather than after the scan is done.
Why does PET-CT cost more than a regular CT scan?
A CT scan uses X-rays and a contrast dye on standard equipment. PET-CT uses a radioactive tracer called FDG, which must be produced in a cyclotron facility, transported within hours of production, and injected before the scan. The scan takes longer than a standard CT and requires a nuclear medicine physician to report it alongside the radiologist. These additional steps — the tracer, the specialised equipment, and the dual specialist reporting — are what the higher price reflects.
Why does the quoted price differ so much between centres?
FDG tracer has a short shelf-life and is produced to order, so centres that run fewer scans per day may pass on a higher per-patient cost. Centres with NABH accreditation or in-house nuclear medicine reporting generally charge more than those that outsource their reporting. Location also matters — centres in central Hyderabad tend to charge more than those further out. None of these differences tells you which centre will give you the better quality report; ask your oncologist which partner centres they use and trust, not just which one has the lowest listed price.
Can we do the PET-CT first and the biopsy afterwards?
Usually yes, and many teams prefer this sequence. PET-CT maps all the sites where disease may have spread and helps your team choose the safest and most informative place to take the biopsy from. If the PET shows involvement in a lymph node that is easier to reach than the lung mass itself, that is often the better biopsy site. What does not change is the order of decisions: PET-CT informs the plan, biopsy confirms the diagnosis, and treatment starts only after both are complete.
How does molecular testing fit into the staging cost?
Once a biopsy is confirmed as lung cancer, the tissue sample is sent for molecular testing — checking for mutations such as EGFR, ALK, ROS1, and KRAS, and for PD-L1 expression. These results determine whether targeted therapy or immunotherapy applies to your case, and treatment decisions cannot be made without them. The cost varies by the panel used and the laboratory.
