What does PET-CT show in mesothelioma?
Mesothelioma grows along the pleural lining of the lung rather than forming a single lump. PET-CT picks up areas of high metabolic activity — tissue that is using energy faster than normal, which includes actively growing cancer.
The scan is most useful for showing whether the disease has spread outside the chest: to lymph nodes, into the abdomen, or to distant sites. That information is what determines whether surgery is a realistic option and which systemic treatments are most appropriate.
PET-CT is also used after treatment to assess whether active areas have settled. A follow-up scan that shows reduced activity tells your team the approach is having an effect.
What happens during a PET-CT scan?
Prepare the day before
Avoid strenuous exercise the day before the scan. Your team will tell you whether to adjust any medications.
Fast on the day
You will be asked not to eat for several hours before the appointment. Plain water is usually allowed. Your centre will confirm the exact instructions.
Receive the tracer injection
A small amount of radioactive glucose (FDG) is injected into a vein. It is a low-radiation dose and leaves the body within hours.
Rest quietly for about an hour
You wait in a calm environment while the tracer travels through your body. Staying still and relaxed gives a cleaner result.
Lie still for the scan
The scan itself takes roughly 20 to 30 minutes. You lie on a flat bed that moves slowly through the scanner. Breathing normally is fine.
Wait for the report
A radiologist reviews the images and sends a report to your oncologist. Ask your team when to expect it.

Where does PET-CT have limits in mesothelioma?
Mesothelioma spreads as a thin sheet along the pleural surface, not as discrete lumps. PET-CT can miss small or flat deposits that do not generate enough metabolic signal to stand out.
In India, tuberculosis affecting the pleura is a genuine diagnostic challenge. Pleural TB can produce FDG-avid lesions that look similar to mesothelioma on imaging. This is one of the central reasons biopsy and tissue analysis are required before any treatment decision — imaging alone cannot distinguish between the two.
PET-CT also cannot tell you which subtype of mesothelioma is present. The subtype affects prognosis and treatment options, and only a biopsy with specialist pathology analysis answers that question.
A scan that appears clear does not rule out disease. If your clinical team still has concerns after a normal result, further investigation is appropriate.
Questions families ask about PET-CT and mesothelioma
Can the PET-CT tell us whether surgery is possible?
It is one of the most important pieces of information for that decision, but not the only one. Surgeons also consider your lung function, your general fitness, and the detailed findings on CT. PET-CT helps by showing whether disease has spread to sites that would make surgery inappropriate — distant lymph nodes, the other side of the chest, or the abdomen. If the scan shows no distant spread, the surgical question remains open and your team will discuss it further.
We already had a CT scan. Why do we need a PET-CT as well?
A CT scan provides detailed anatomical images — the shape and size of structures. A PET scan adds a metabolic layer by showing which areas are using energy in a way that suggests active disease. The two are complementary rather than duplicates. A CT may show thickening along the pleura without being able to say whether it is active; the PET component helps answer that. Combining both in a single session gives more information than either scan gives alone.
The scan shows areas lighting up — does that mean the cancer has spread?
Not necessarily. Increased FDG uptake shows metabolic activity, which can come from active cancer but also from inflammation, infection, or healing tissue. Lymph nodes that appear active on PET-CT are not automatically confirmed as involved by cancer — that requires further assessment, sometimes a biopsy of the node itself. Your oncologist will interpret the result in the context of everything else known about your situation, not from the scan image alone.
Could the scan be showing TB rather than mesothelioma?
Yes, and this is a genuine concern in India, where TB is common and can affect the pleura in ways that look very similar to mesothelioma on both CT and PET-CT. Pleural TB can produce thickening, fluid, and FDG-avid lesions. This is one of the central reasons that biopsy — not imaging — is what confirms a mesothelioma diagnosis. Tell your team about any past TB, any TB exposure, or any symptoms that might point to infection. That information shapes how the results are interpreted.
Will the PET-CT be repeated during or after treatment?
Often yes. A repeat scan after a course of treatment gives your team an objective measure of how the disease has responded. Areas that were active before and are quiet now suggest a positive response. Areas that remain active or have increased suggest a different approach may be needed. The timing and frequency of repeat scans depends on what treatment you are receiving and what your team is measuring — your oncologist will tell you when a follow-up scan is planned.

Usually not. A plain CT of the chest is typically the first imaging step when mesothelioma is suspected, because it gives a detailed picture of the pleura, any fluid, and the surrounding structures. PET-CT is usually ordered at the staging stage — once a biopsy has confirmed or is close to confirming the diagnosis. Your oncologist will tell you the sequence of investigations planned for your situation.
What is the difference between PET-CT and a plain CT for mesothelioma?
A plain CT shows anatomy in detail — the shape, size, and position of structures. PET-CT adds a metabolic layer using a radioactive tracer that accumulates in areas of high activity. In mesothelioma, the combination helps identify not just where disease is visible, but where it is actively growing — and whether it has spread to sites outside the chest that the CT alone might not clearly characterise.
How long does the whole PET-CT appointment take?
Allow most of a morning or afternoon. The tracer injection and the resting period before scanning take roughly an hour, and the scan itself takes around 20 to 30 minutes. You may also spend time at the centre for registration and preparation. Your imaging centre will confirm the exact timing when you book.
Can the scan give a false-normal result in mesothelioma?
Yes. Because mesothelioma often spreads as a thin sheet along the pleural surface rather than as a distinct mass, small areas of disease may not generate enough metabolic signal to appear clearly. A normal or quiet scan does not rule out mesothelioma. If your clinical team has strong reasons to suspect the disease — or if symptoms persist — further investigation including biopsy remains appropriate even after a scan that appears unremarkable.
