Is PET-CT always needed for breast cancer staging?
For early breast cancer — Stage I or Stage II — PET-CT is not part of the standard staging workup. NCCN and ASCO guidelines do not recommend it for early disease: the scan adds cost and radiation exposure without changing treatment decisions for most patients.
PET-CT is appropriate when your disease is locally advanced (Stage III), when metastatic spread is suspected or confirmed, or when there is concern about recurrence. These are the situations where a whole-body metabolic picture is likely to change what happens next.
If you have been asked to have a PET-CT and are uncertain whether your stage warrants it, ask your oncologist what specific question the scan is expected to answer.
What happens on the day of a PET-CT scan?
Referral and preparation
Your oncologist sends a referral to a partner imaging centre. You will be given a date, a fasting window, and instructions about any medications to pause. Follow these precisely — a missed preparation step can affect the quality of the scan.
Blood sugar check on arrival
A glucose check confirms the tracer will distribute correctly through your body. If your blood sugar is elevated on the day, the scan may need to be rescheduled.
Tracer injection and rest
A small amount of radioactive FDG tracer is injected into a vein. You then rest quietly for around an hour while it travels through your body. You are not in the scanner during this time.
The scan
The scan itself takes 20 to 30 minutes. You lie still on a table that moves through the machine, which captures both metabolic activity and body structure in a single session. It is not painful.
Report and oncologist review
A nuclear medicine specialist reads the images and produces a written report, usually within 24 to 48 hours. Your oncologist reviews it alongside all your other results before discussing what it means for your treatment.

What does a PET-CT scan cost in Hyderabad?

What do the terms on a PET-CT report mean?
- FDG
- Fluorodeoxyglucose — a sugar molecule with a short-lived radioactive atom attached. Cancer cells absorb it faster than normal tissue, making them visible on the scan.
- SUV (Standardised Uptake Value)
- A number that measures how much tracer a region absorbed. There is no single value that means cancer or not-cancer; your oncologist interprets it alongside all your other findings.
- Staging
- Establishing how far cancer has spread at the time of diagnosis, across four stages. Your treatment plan is built around the stage.
- Restaging
- Re-assessing the extent of disease after treatment, or when recurrence is suspected. This is one of the most established roles for PET-CT in breast cancer.
- Metabolically active
- Tissue using glucose at a higher rate than expected. Active cancer cells are usually metabolically active — but so can infections and inflamed areas, which is why a radiologist interprets the scan in full clinical context.

What are the alternatives to PET-CT for staging early breast cancer?
For early-stage breast cancer, a CT scan of the chest, abdomen and pelvis combined with a bone scan is the standard staging workup. These cover the most common sites of spread at lower cost and a lower radiation dose. MRI of the breast is added in specific situations — for example, when tumour extent before surgery is uncertain, or when dense tissue limits mammography. These are not inferior options; they are the right tools for the stage of disease.
Can PET-CT show whether chemotherapy is working?
Yes, and this is one of its most valuable uses in locally advanced and metastatic breast cancer. A scan before and after a course of chemotherapy can show whether cancer cells have become metabolically less active — a sign that treatment is working. Your oncologist will tell you whether interim response assessment is part of your plan and how often it will be repeated.
Is there radiation involved, and is it safe?
Yes — PET-CT involves radiation from both the FDG tracer and the CT component. Radiological bodies including IAEA consider the dose acceptable when the scan is clinically justified. This is not a reason to avoid it when it is genuinely needed. It is a reason not to repeat it unnecessarily, which is another argument for being clear about the clinical indication before booking.

Frequently asked questions
My doctor said I need a PET-CT. Does that mean my cancer is advanced?
Not necessarily. Oncologists request PET-CT for several reasons: to confirm a finding on another scan, to plan radiotherapy, or to assess whether treatment has worked. A PET-CT request on its own does not tell you your stage. Ask your oncologist what they are looking for, and what a positive or negative result would mean for your treatment plan.
How is PET-CT different from a regular CT scan?
A CT scan shows the structure of your organs and tissues. PET-CT adds a metabolic layer, showing which areas are using glucose at an unusually high rate. Cancer cells are often highly active, so they appear bright. The combination tells your oncologist both where a lesion is and how active it is — information a CT scan alone cannot give.
Does health insurance cover PET-CT for breast cancer in India?
Most policies cover it when the scan is clinically indicated and the oncologist has documented why it is needed. The key step is pre-authorisation — get the insurer's approval before the scan, not afterwards. Bring the referral letter and your oncologist's clinical note when you apply. If the insurer refuses, ask your oncologist to provide a more detailed written justification.
How long does a PET-CT appointment take?
Plan for two to three hours at the imaging centre. The tracer injection is followed by about an hour of rest, then 20 to 30 minutes in the scanner, plus arrival, registration and a blood sugar check. You will also need to fast beforehand — confirm the exact fasting window with the centre when you book.
Can I have a PET-CT at a CION centre?
CION does not own PET-CT scanners. Your oncologist will refer you to an established partner imaging centre, and CION coordinates the referral and follow-up. The scan itself happens at the imaging centre; the report interpretation and your treatment planning happen through your CION oncologist at your next appointment.
A bright spot showed up on my PET scan. Does it mean the cancer has spread?
Not necessarily. The FDG tracer is absorbed by any tissue with high metabolic activity — including infections, inflamed joints, and recent surgical sites. A bright area is a finding to be interpreted, not a diagnosis. Your oncologist and the nuclear medicine specialist will look at the location, the SUV value, and the CT findings together before drawing any conclusion.
