The Deauville score is a five-point scale that nuclear medicine physicians use to report how active lymphoma looks on a PET-CT scan. Scores 1 and 2 are negative. Score 3 is equivocal. Scores 4 and 5 indicate uptake above what is expected for healthy liver tissue and may suggest active disease.

Why does your lymphoma PET-CT report have a number from 1 to 5?
Radiologists use the Deauville scale because it gives your treating team — and any centre you consult later — a consistent way to describe whether lymphoma appears active on the scan. A number from 1 to 5 is precise where phrases like 'appears improved' are not.
PET-CT scans detect glucose uptake. Lymphoma cells often take up more glucose than surrounding tissue, which shows as bright areas. The Deauville score describes how bright those areas are, using two reference organs as fixed comparison points.
Those references are the mediastinal blood pool — the large blood vessels in the chest — and the liver. Both are used because their glucose uptake is stable and predictable across patients, making them reliable anchors for the scale.
What does each Deauville score mean?
- Score 1
- No FDG uptake detected anywhere above background. The scan shows no activity that could represent lymphoma.
- Score 2
- Uptake is present but at or below the level of the mediastinal blood pool — the lower of the two reference points. Clearly negative.
- Score 3
- Uptake is above the mediastinal blood pool but at or below the liver. This is the equivocal zone. Whether score 3 is treated as negative or positive depends on when the scan was done in your treatment course. Your haemato-oncologist places it in context.
- Score 4
- Uptake is moderately higher than the liver. This is described as positive and may indicate active lymphoma. The location of the uptake and the comparison to your previous scan both matter.
- Score 5
- Uptake is markedly higher than the liver, or new sites of uptake have appeared that were not present on the previous scan. This is the most concerning category on the scale.
- Score X
- New areas of uptake have appeared but are considered unlikely to be lymphoma — for example, an infection or area of inflammation. These are noted separately and not scored against lymphoma activity.

Did you know?
Before the Deauville scale was adopted internationally, there was no agreed standard for reporting lymphoma PET-CT findings. Two physicians reading the same scan could reach opposite conclusions about whether the disease was responding.
The scale was created at an international expert consensus workshop to solve exactly this problem — so that a result means the same thing wherever in the world it is read.

Questions families ask about the Deauville score
Why are there two reference organs instead of one?
Two references create a range rather than a single cutoff, which makes the scale more useful. The mediastinal blood pool is the low anchor — any uptake at or below it is clearly not concerning. The liver is the higher anchor that separates scores 3, 4 and 5 from each other. Using both means the scale can describe a spectrum from clearly normal to clearly abnormal, with an equivocal zone in between, rather than a simple yes-or-no that would miss the middle ground.
Does score 3 mean the same thing at interim PET and at end-of-treatment PET?
Not exactly. At an interim scan — taken partway through treatment — many guidelines treat score 3 as consistent with a good response for certain lymphomas, particularly Hodgkin lymphoma. At end-of-treatment assessment, whether score 3 is acceptable or concerning depends on the lymphoma type and the clinical guidance being applied. The number is the same at both time points; the clinical weight it carries differs. Your haemato-oncologist will tell you how to read it in your specific situation.
What does a score of 4 or 5 mean for my treatment?
It means the radiologist has identified uptake above the threshold for active disease. What that means for your treatment is a separate question that only your haemato-oncologist can answer, because it depends on where the uptake is, how it compares to your previous scan, what lymphoma type you have, and what treatment you are on. A score of 4 or 5 is not an automatic instruction to change treatment. Please speak to your team before drawing any conclusions from the number alone.
Can the Deauville score be wrong?
PET-CT is an imaging tool, not a biopsy. FDG uptake can be elevated by infections, inflammation, healing tissue after chemotherapy, and normal variation — not only by active lymphoma. Score X exists precisely because some bright areas are clearly unrelated to the disease. For borderline results, your team may recommend a repeat scan or a biopsy rather than acting on a single result alone. The Deauville score is a standardised description of what the scan shows, not a direct measurement of lymphoma cells.
Who assigns the Deauville score — and can I ask for a second read?
The score is assigned by a nuclear medicine physician or radiologist with specialist training in PET-CT interpretation. For borderline cases — particularly score 3 at a critical decision point, or an unexpected score 4 or 5 — it is entirely reasonable to ask your haemato-oncologist whether a second read from another specialist centre is appropriate. This is standard practice in lymphoma care and is not an unusual or difficult request to make.
What is complete metabolic response, and how does it relate to the Deauville score?
Complete metabolic response, abbreviated CMR, is a standardised term from the Lugano Classification — the international framework used to assess lymphoma treatment response. A CMR is generally defined as a Deauville score of 1 or 2, and in some contexts a score of 3, depending on the lymphoma type and the guidance being applied. Your haemato-oncologist will tell you which response category your scan falls into and what that means for your ongoing care.
