Why would your oncologist order both a PET-CT and a separate diagnostic CT?
A PET-CT shows where in the body cancer cells are metabolically active. It is particularly useful for detecting spread and assessing response to treatment across the whole body in a single session.
A diagnostic CT gives detailed anatomical pictures of a specific region — the chest, abdomen, or pelvis — usually with contrast that highlights blood vessels and soft tissue boundaries. This level of detail matters for surgical planning, for measuring a tumour precisely, or for characterising something the PET scan flagged but did not fully resolve.
The two scans answer different questions. There are situations — particularly in staging or pre-surgical assessment — where a team genuinely needs both, and ordering both is not an oversight.
There are also situations where a PET-CT is not the right first test at all. For certain cancers and clinical questions, a contrast CT or MRI gives more relevant information at lower cost and lower radiation exposure. Your oncologist will explain which applies to you.

What is attenuation correction — and what do the other terms on your report mean?
- Attenuation correction (AC)
- A process that adjusts PET images to account for the way different body tissues absorb the gamma rays the tracer emits. The CT component of a PET-CT is done primarily to supply the data needed for this correction.
- Low-dose CT
- A CT scan using considerably less radiation than a standard diagnostic CT. It is adequate for attenuation correction and approximate anatomical localisation, but not for detailed diagnostic reporting.
- Contrast-enhanced CT (CECT)
- A CT scan where iodine-based dye is injected into a vein immediately before scanning. The contrast makes blood vessels, tumour margins, and organ boundaries much clearer. Most diagnostic CTs use contrast.
- Diagnostic CT
- A dedicated CT scan ordered to answer a specific clinical question. A radiologist reads and reports it fully as a standalone study. It is a separate procedure from a PET-CT.
- FDG PET-CT
- A combined scan using a mildly radioactive glucose tracer (FDG) to highlight metabolically active areas, fused with a CT for anatomical context. The CT component is typically the low-dose variety unless a specific protocol is ordered.
Does the CT inside a PET-CT cost extra — and why are there two charges?
The CT inside a PET-CT is part of the PET-CT package. It is not billed as a separate line item.
A diagnostic CT is a separate procedure and carries its own charge. If your oncologist orders one alongside a PET-CT, it will appear as a distinct item on your bill — and that is correct, not a billing error.
If you see two charges and are unsure which is which, ask the billing team to name each procedure. You are entitled to a clear explanation of every item on a medical bill before you pay.

Can the CT inside my PET-CT replace my diagnostic CT?
In most cases, no. The CT component of a PET-CT is a low-dose scan optimised for image correction, not for detailed anatomical reporting. A radiologist does not typically issue a standalone diagnostic report on it. If your oncologist has separately ordered a diagnostic CT, they need information the PET-CT's internal scan cannot reliably provide — most often detailed soft tissue characterisation or precise tumour measurement with contrast dye.
Will I receive contrast dye during a PET-CT?
Usually not. Standard PET-CT does not include intravenous contrast because contrast can alter how the tracer distributes in your body, affecting the scan's accuracy. Some centres offer a combined session where a diagnostic-quality contrast CT is done alongside the PET-CT, but this is a specifically ordered variation, not the default. If you are unsure whether contrast is part of your scheduled scan, ask the imaging team before your appointment so you can prepare correctly.
Why did my oncologist order both a PET-CT and a CT with contrast?
Because they answer different questions. A PET-CT maps metabolic activity across the whole body — useful for detecting spread and assessing treatment response. A contrast CT of a specific region gives structural detail that the PET-CT's internal low-dose scan cannot match. For surgical planning, for characterising a lesion found on PET, or for measuring a tumour precisely, the contrast CT adds information the PET-CT alone does not provide.
How much extra radiation does a diagnostic CT add if I am already having a PET-CT?
A PET-CT involves radiation from both the radioactive tracer and the low-dose CT. A diagnostic CT adds its own dose on top. The combined exposure is not trivial, and your oncologist weighs that against the clinical benefit before ordering both. Radiation dose decisions in oncology follow guidance from international bodies including the IAEA and ICRP. If you have concerns, ask your team to explain specifically what decision the diagnostic CT is informing in your situation.
Can I ask my oncologist to skip the diagnostic CT and rely on the PET-CT alone?
You can always ask why a test has been ordered and what it will change in your management — that is a reasonable question and your team should answer it clearly. However, if the diagnostic CT has been requested for a specific purpose such as surgical planning, declining it may leave your team without information needed to treat you safely. Ask what decision the CT is informing before deciding whether to decline it.
If both scans are done on the same day, will I receive two separate bills?
Yes, if both are ordered. A PET-CT and a diagnostic CT are separately coded procedures, each with its own charge even when performed in the same session or at the same centre. Two separate line items on your bill is correct, not an error. If the amounts look different from what you were quoted, ask the billing team to walk through each item with you before paying.
