How do you apply for pre-authorisation for PSMA PET-CT?
Check your policy schedule
Look in your policy documents for PET-CT, nuclear medicine, or diagnostic imaging. PSMA PET-CT is unlikely to be listed by name, but you need to know what category your insurer uses before you apply.
Get a written referral from your oncologist
The letter must explain why PSMA PET-CT is clinically necessary and why standard FDG PET cannot answer the same question. This distinction is what separates an approvable claim from a refused one.
Apply for pre-authorisation before booking
Submit the referral, your diagnosis reports, and biopsy results to your insurer or TPA. Do not book the scan first. Prior approval is what the claim hinges on.
Ask for the approval in writing, with the procedure code
Verbal approval from a call centre does not protect you. Ask for written confirmation and check that the procedure code on the approval matches what your imaging centre will invoice.
Confirm the imaging centre is on the network
Some insurers only reimburse scans at empanelled facilities. Check this before you book, because changing centres after the scan is difficult.
Collect itemised receipts on the day
Ask for the scan report, the tracer cost, and any radiologist fee as separate line items. Some claims require this breakdown, and it is much harder to obtain after you have left.
Will a private health insurance policy pay for PSMA PET-CT?
Private insurance may pay, but it is not straightforward. PSMA PET-CT uses a specialised tracer that most Indian insurance schedules do not list by name. Whether your claim succeeds depends on whether your insurer accepts PSMA PET-CT as falling within the general PET-CT category your policy covers.
Specialised tracers are refused more often than standard FDG PET because they appear as newer, unlisted procedures to the insurer's review team. This is an administrative classification, not a clinical judgement — and it can be challenged with the right documentation.
Pre-authorisation before the scan is the single most important step. A strong oncologist letter that explains what clinical question the scan will answer, and why FDG PET cannot substitute, shifts the claim from a routine refusal to a reviewable case.

Does Aarogyasri or AB-PMJAY cover PSMA PET-CT?
PSMA PET-CT is not currently listed in the Aarogyasri or AB-PMJAY procedure packages. Both schemes cover a defined set of procedures, and PSMA-specific tracers are not among them as of the current package versions.
If you are covered under Aarogyasri in Telangana or the equivalent AP scheme, you may be able to access standard FDG PET-CT for certain cancer types through the scheme. PSMA PET-CT would need to be arranged and paid for separately.
CGHS has published rates for PET-CT, but the rate schedule does not consistently distinguish between tracer types in a way that covers PSMA. Check with your CGHS dispensary directly, because the schedule is updated periodically and the position can change.
What can you do if your claim is refused?
Ask for the refusal in writing, with the specific reason and the policy clause cited. A vague refusal is not a final answer, and you cannot file a meaningful grievance without knowing the stated reason.
Your oncologist can write a stronger medical necessity letter for the grievance — one that explains in clinical terms why PSMA PET-CT was selected, what ASCO or ESMO guidance says about it, and what clinical decision it was intended to support.
If the internal grievance is not resolved, you can file a complaint with IRDAI through their Bima Bharosa portal. Document every step — dates, names, and what was said — because that record is what the regulator will ask to see.

Standard imaging — CT and bone scan — misses a substantial proportion of prostate cancer recurrences that PSMA PET-CT detects at early, treatable stages. ASCO and ESMO guidance now recommends PSMA PET-CT where it is available. Indian insurance schedules have not yet been updated to reflect this clinical consensus, which is why the mismatch between what oncologists order and what insurers approve happens so often.

My insurer approved FDG PET before. Does that mean PSMA PET-CT will be approved too?
Not automatically. FDG and PSMA are different tracers classified under different procedure codes, and approval of one does not extend to the other. You need to apply for pre-authorisation specifically for PSMA PET-CT, with an oncologist letter explaining the clinical reason it was chosen over FDG. Some insurers have approved PSMA PET-CT under a general PET-CT category once clinical necessity was established in writing — but it requires a separate application, not an assumption.
Can I pay upfront and claim reimbursement after the scan?
Reimbursement claims are possible, but the chance of approval is lower without prior authorisation. Some insurers refuse reimbursement for procedures not pre-approved, even when the clinical indication is clear. If you are considering paying upfront, contact your insurer in writing before the scan and document the response. That written record is your protection if the reimbursement is later disputed.
What should the oncologist's referral letter say?
The letter should explain why PSMA PET-CT was requested, why FDG PET or conventional imaging could not answer the same clinical question, and what treatment decision the scan result will inform. It should note that the recommendation is consistent with ASCO or ESMO guidance on prostate cancer imaging. The more specific the clinical reasoning, the harder the claim is to refuse on administrative grounds. A generic letter saying 'PET scan required' is not sufficient.
Is there a standard procedure code for PSMA PET-CT in India?
PSMA PET-CT does not yet have a single standardised procedure code that all Indian insurers use consistently. Ask your imaging centre what code they will use on the invoice, and ask your insurer what code they expect on the claim form — then confirm the two match before the scan. A mismatch in codes is a common reason for refusal that has nothing to do with clinical merit, and it is much harder to correct after the scan is done.
What does PSMA PET-CT cost if insurance does not pay?
CION coordinates PSMA PET-CT through partner imaging centres at an indicative cost of around ₹23,999 as of 2025 — confirm the current figure when booking, as availability and tracer supply are confirmed at that point. No guarantee of insurance coverage can be given before written pre-authorisation is obtained. If cost is a concern, discuss the timing of the scan with your oncologist.
