What does PSMA PET-CT look for?
PSMA stands for Prostate-Specific Membrane Antigen. It is a protein that prostate cancer cells produce in large amounts. The tracer used in this scan is designed to find and attach to that protein, making deposits visible on imaging.
The standard PET tracer, FDG, tracks glucose consumption. Prostate cancer cells do not always consume glucose at a detectably higher rate than the healthy tissue around them. This means small deposits in lymph nodes or bone can be invisible on an FDG scan, even when they are clinically significant.
PSMA PET-CT closes that gap. It is a tool designed specifically for prostate cancer, not a general-purpose cancer scan.
How is PSMA PET-CT different from a standard PET scan?
The difference is the tracer. A standard PET scan uses FDG, which tracks how cells consume glucose. That works well for many cancers — lymphoma, lung cancer, head and neck tumours — because those cells burn glucose at a higher rate than their surroundings. Prostate cancer usually does not.
PSMA PET-CT uses a tracer that binds directly to a surface protein on prostate cancer cells. It is not measuring metabolic activity. It is finding a specific molecular target. NCCN and ESMO guidance recognises it as more sensitive than conventional imaging for detecting the spread of prostate cancer, including in sites that appear normal on CT or standard PET.
The practical difference is also cost and availability. PSMA tracers are more expensive to produce than FDG and require specific preparation. That is why the scan costs more and is coordinated through specialist imaging centres rather than being available everywhere.

- Your PSA has risen after surgery or radiotherapy and your team needs to locate where the recurrence is.

What happens on the day of a PSMA PET-CT?
The tracer is injected through a vein. You then rest quietly for around an hour while it travels through the body and binds to cancer cells. The scan itself takes approximately 20 to 30 minutes. Unlike a standard FDG PET scan, there is no fasting requirement — you can eat and drink normally before arriving.
You will be asked to drink water and to empty your bladder before the scan begins. The tracer clears through the kidneys, and a full bladder can obscure the pelvis on the images.

Can PSMA PET-CT detect cancers other than prostate cancer?
In standard clinical practice in India, PSMA PET-CT is ordered for prostate cancer. A small number of other cancers — including certain salivary gland tumours — can also express PSMA protein, and the scan is used in those situations in some centres. If your oncologist is recommending it for a diagnosis other than prostate cancer, they will explain the specific reason it applies to your case.
My father already had a standard PET-CT. Why is a PSMA PET-CT being ordered as well?
The two scans are not duplicates — they answer different questions. The earlier FDG PET was appropriate for what it was assessing at the time. PSMA PET-CT is now being ordered because your team wants greater detail about where prostate cancer specifically is, or has spread to. Using both scans in the right sequence is standard practice. It is not a sign that something was missed earlier.
What does it mean if the PSMA PET-CT shows no deposits?
A scan that shows no PSMA-avid deposits is an encouraging result. It does not guarantee there is no cancer present. Very small deposits below the scan's detection limit, and a minority of prostate cancers that express PSMA at low levels, may not appear. Your oncologist will interpret the result alongside your PSA level, biopsy details, and clinical history — never from scan findings alone.
How long does the whole appointment take?
Plan for around two to three hours at the imaging centre. Most of that time is the resting period after the tracer injection, not the scan itself. The scan typically takes under half an hour. You can eat and drink normally before arriving, which is different from the fasting that a standard FDG PET scan requires. Bring something to read or listen to during the waiting period.
Is the radiation from PSMA PET-CT harmful?
The radiation exposure from a PSMA PET-CT is in a range comparable to other nuclear medicine investigations. For patients for whom the scan is clinically indicated, the diagnostic benefit is considered to outweigh the exposure. If you have concerns, ask the nuclear medicine specialist at the imaging centre to explain the estimated dose for your scan. The tracer clears through the kidneys and is largely out of the body within 24 hours.
How much does PSMA PET-CT cost, and is it covered by insurance?
PSMA tracers are more expensive to produce than FDG, which is why this scan is priced higher than a standard PET-CT.
