How does F-DOPA show what is happening in Parkinson's disease?

In Parkinson's disease and related conditions, the nerve cells that produce dopamine — a chemical essential for smooth movement — reduce in number over time. F-DOPA PET-CT measures how well those cells are still working by tracking uptake of a radiolabelled dopamine precursor across the brain.

This distinguishes Parkinson's disease from essential tremor or drug-induced parkinsonism, which share similar symptoms but do not involve dopamine cell loss. EANM and SNMMI both recognise presynaptic dopaminergic imaging as an established tool for this clinical question.

The scan does not diagnose Parkinson's disease on its own. Your neurologist uses it alongside your symptoms, examination findings, and response to medication to form a complete clinical picture.

When does an oncologist request F-DOPA rather than a standard scan?

Paragangliomas and phaeochromocytomas are rare tumours of nerve-related tissue that often fail to show on a standard FDG PET-CT. Some are also missed by a DOTATATE scan. F-DOPA finds these tumours because they take up the radiotracer through the same pathway they use to produce hormones.

EANM guidelines identify F-DOPA as an established imaging tool for paraganglioma and phaeochromocytoma, with EANM guidance specifically highlighting its role in head and neck paragangliomas where other tracers may not perform as well.

Its role in other neuroendocrine tumours — such as midgut carcinoids or gastroenteropancreatic NETs — is more variable and considered emerging rather than established. Your oncologist will confirm whether F-DOPA is appropriate for your specific tumour type, or whether a different tracer is better supported by current evidence.

Terms your doctor may use

F-DOPA (fluorodopa)
A radioactive form of levodopa — the substance the brain converts into dopamine. Injected into a vein before the scan and tracked as relevant cells or tumours take it up.
FDG (fluorodeoxyglucose)
The most common PET tracer. It shows glucose uptake. Some tumours, including many paragangliomas, do not use enough glucose to appear clearly on FDG — which is when F-DOPA becomes the better choice.
Paraganglioma
A tumour arising from nerve-related tissue outside the adrenal gland — commonly in the head, neck, chest, or abdomen. It can produce hormones that cause high blood pressure, sweating, and palpitations.
Phaeochromocytoma
The same type of tumour when it grows inside the adrenal gland, which sits above each kidney. It behaves similarly to paraganglioma and often requires the same imaging approach.
Parkinsonian syndrome
An umbrella term for conditions that share Parkinson's symptoms — tremor, slowness, stiffness — but may have different causes and different responses to treatment. F-DOPA helps distinguish between some of them.
DOTATATE scan
Another specialised PET tracer that targets receptors many neuroendocrine tumours carry. When a tumour does not carry enough of these receptors, the DOTATATE scan may be negative — one situation where F-DOPA can add information.

How is F-DOPA PET-CT different from a standard FDG PET-CT?

FDG shows glucose uptake, which most cancers and areas of inflammation share. F-DOPA follows a completely different pathway — it enters cells that handle dopamine, either in the brain's movement centres or in neuroendocrine tumours. This means it can detect changes that show nothing on an FDG scan. The two tracers answer different clinical questions, and your doctor chooses based on what they are looking for, not on which is newer or more advanced.

I have a tremor — will this scan confirm whether it is Parkinson's disease?

The scan gives your neurologist important information but does not diagnose Parkinson's disease on its own. Essential tremor — the most common type of tremor in adults — does not involve dopamine cell loss, so the scan is typically normal in those cases. That normal result is clinically useful because it supports a diagnosis of essential tremor rather than Parkinson's. Your neurologist will interpret the result alongside your full clinical picture, not in isolation.

What preparation do I need before an F-DOPA PET-CT?

You will usually be asked to eat a low-protein meal in the hours before the scan, because dietary protein competes with the tracer for uptake. Some medicines — particularly levodopa and other dopaminergic drugs used for Parkinson's — may need to be paused. The partner imaging centre will give you personalised instructions when your appointment is confirmed. Follow them carefully, as preparation directly affects image quality.

How long does the scan take?

Allow roughly 90 minutes to two hours at the imaging centre in total. The radiotracer needs time to be absorbed before imaging begins, so there is a waiting period after the injection. The scan itself is similar to a standard PET-CT: you lie still on the table while the scanner moves around you. You can usually eat and drink normally once the scan is complete.

Is F-DOPA PET-CT used for brain tumours?

There is published research on F-DOPA and related amino acid PET tracers in brain tumour grading and recurrence detection. This is an area of active investigation and is not yet routine in most guidelines. We do not yet have enough evidence to recommend it as a standard tool for brain tumour assessment. If your neuro-oncologist considers it relevant for your situation, they will advise whether it is appropriate and available at your nearest partner centre.
