Why does F-18 NaF PET detect more than a conventional bone scan?

Both tracers travel to areas of active bone turnover, but F-18 NaF is taken up more avidly by bone and clears from surrounding soft tissue faster. This leaves a cleaner, higher-contrast signal. PET cameras also capture images at higher spatial resolution than the SPECT cameras used in conventional bone scans, so smaller areas of bone involvement become visible.

Whether detecting a lesion earlier changes your treatment plan depends on your cancer type, stage, and what decision your team is making at the time of the scan. Your oncologist, not the scan alone, decides whether the upgrade is relevant for you.

F-18 NaF does not detect soft tissue disease. If your team needs to assess both bone and soft tissue in the same appointment, FDG PET/CT or PSMA PET/CT may give more complete information.
