Does PSMA PET-CT find prostate cancer spread earlier than a bone scan?
Yes. PSMA PET-CT identifies bone and lymph node deposits at smaller sizes than a bone scan and CT can. NCCN and ASCO both list it as a preferred staging scan for intermediate and high-risk prostate cancer.
A bone scan detects metastases indirectly — it shows areas of abnormal bone activity, which means it can miss smaller deposits and occasionally flag old injuries as suspicious. PSMA PET-CT binds directly to a protein found in high concentrations on prostate cancer cells, so it shows where the cancer is rather than its downstream effect on bone.
MRI is most useful for mapping the tumour within the prostate gland itself. It is not designed to show distant spread.
Do you still need a bone scan and MRI if you have had a PSMA PET-CT?
For most patients being staged for intermediate or high-risk prostate cancer, PSMA PET-CT replaces the bone scan and CT combination. You do not usually need all three.
A prostate MRI is still recommended before treatment planning for localised disease — it maps the tumour within the gland in detail that PSMA PET-CT is not designed for. If you have already had one for local staging, you will not normally need a repeat.
If PSMA PET-CT tracer cannot be confirmed for your location and date, the older pair may be ordered instead. Ask your oncologist which scans are planned for you and why.
What happens on the day of your PSMA PET-CT?
Booking and tracer confirmation
The PSMA tracer has a short shelf life and is produced in small batches. Your appointment is confirmed only once tracer availability is verified at the partner imaging centre for your specific date.
Preparation
You will receive specific instructions about fasting and any medicines to pause. Follow these exactly — incorrect preparation can affect image quality.
Tracer injection
A small amount of the PSMA tracer is injected into a vein. You then wait in a quiet room — usually around an hour — while it circulates before imaging can begin.
The scan
You lie still on the scanner table for roughly 20 to 30 minutes. The scanner is open, not enclosed like an MRI, and most people find it straightforward.
After the scan
You can eat, drink and travel normally. Drinking plenty of water through the rest of the day helps clear the tracer from your body.
Results
A nuclear medicine specialist and radiologist review the images. Your oncologist receives the report and discusses it with you at your next appointment.

What does each scan cost, and could PSMA actually save money?
PSMA PET-CT through CION's partner imaging centres is indicatively priced at around ₹23,999 as of the date this page was last reviewed. This covers a single whole-body scan.
A bone scan and a CT are each ordered and charged separately. When both were routinely ordered together — as they were before PSMA PET-CT became available — the combined cost could be comparable to a single PSMA scan, with the added inconvenience of two separate visits.
The reason to prefer PSMA PET-CT where it is indicated is accuracy, not cost. But knowing it may not cost more than the older pair is useful when your family is planning ahead.
Questions families ask when choosing between these scans
My father was told to get a bone scan. Can he ask for a PSMA PET-CT instead?
That is a reasonable question to raise with the oncologist. The right choice depends on the cancer stage and risk category, PSA level, and whether tracer supply can be confirmed for his location and date. In some cases a bone scan is still ordered alongside PSMA PET-CT — for example, in a clinical trial protocol. The conversation to have is: has PSMA PET-CT been considered, and if a bone scan is still preferred, why?
We are in a smaller city. Can we still get a PSMA PET-CT?
PSMA PET-CT is available through CION's partner imaging centres across Telangana and Andhra Pradesh, but it depends on the specific centre and tracer supply on your date. Availability is confirmed at booking — your CION team will check this and will tell you honestly if the scan cannot be fulfilled for your location rather than booking a slot that cannot be kept.
What if the PSMA PET-CT result comes back negative?
A negative result is generally reassuring, but no scan has a zero false-negative rate. Very small deposits, or cells that produce little PSMA protein, may not appear. Your oncologist interprets the result alongside your PSA level and the overall clinical picture — not as a standalone answer. If the result is unexpected given other information, a repeat scan or an alternative test may be recommended.
Is PSMA PET-CT also used to check whether treatment has worked?
Yes. It is used for restaging after surgery or radiation, and for biochemical recurrence — a rising PSA after definitive treatment with no symptoms on conventional imaging. At PSA levels where a bone scan and CT often show nothing, PSMA PET-CT can detect disease. Whether it is the right scan for your specific situation at your current PSA level is a question for your oncologist.
Can MRI show what PSMA PET-CT missed, or the other way around?
They answer different questions. MRI gives detailed images of soft tissue and maps the local extent of the tumour within the prostate gland. PSMA PET-CT gives a whole-body picture of where cancer cells are present but is not the primary tool for local detail within the gland. In some centres both are done, particularly when precise local staging is needed before surgery or radiation planning.

In studies of patients with intermediate and high-risk prostate cancer, PSMA PET-CT found lymph node or bone disease that conventional imaging had missed in a meaningful proportion of cases — enough to change the treatment plan entirely.
The choice of staging scan is not a technical detail. It determines whether the treatment you receive is matched to the disease you actually have.

Is the PSMA tracer safe?
The PSMA tracer is administered in a very small quantity and cleared from the body rapidly. It is not chemotherapy. The radiation exposure is in the range used for other diagnostic nuclear medicine scans and is considered acceptable for a diagnostic procedure under regulatory and clinical guidance. If you have kidney problems or take specific medications, tell your team before the scan so they can advise on any precautions needed.
Will my health insurance cover PSMA PET-CT?
Coverage varies by insurer and policy. Some insurers in India now cover PSMA PET-CT for prostate cancer staging, particularly where it is listed as indicated by NCCN or ASCO. A letter of medical necessity from your oncologist and the clinical indication documented in your file will support the claim. Check directly with your insurer, and ask your CION team what documentation they can provide.
What if my oncologist has recommended both a bone scan and a PSMA PET-CT?
There are situations where both are ordered — for example, in a clinical trial protocol, or where a specific clinical question is better answered by the bone scan. It is entirely reasonable to ask why both are needed rather than one replacing the other. A clear explanation is something you are entitled to expect, and your team should be able to give it.
How do I arrange a PSMA PET-CT through CION?
