FAPI PET-CT: — The Tracer That Finds What FDG Misses

FAPI PET-CT uses a radiotracer that targets the tissue surrounding a tumour rather than the cancer cell itself. In certain cancers — pancreatic, biliary, gastric — this gives a clearer picture than FDG, which relies on glucose uptake that these tumours often do not show.

Which cancers see the clearest benefit from FAPI PET-CT?

FAPI tends to give the strongest advantage in cancers where the tumour is surrounded by dense, activated fibroblast tissue. Gastric, pancreatic, and biliary tract cancers — including cholangiocarcinoma — fall into this group, and FDG often returns a weak or equivocal signal in all three.

Peritoneal spread — cancer seeded along the lining of the abdomen — is another area where FAPI has shown additional sensitivity in published studies. Some ovarian cancers and soft tissue sarcomas also show strong FAPI uptake.

Your oncologist will tell you whether your cancer type has enough published evidence to justify a FAPI scan and, crucially, whether the result would change your treatment plan. A scan that adds information without changing management is a cost without a benefit.

What do these terms mean?

FAPI
Fibroblast Activation Protein Inhibitor — the radioactive molecule injected before the scan. It is designed to seek out and bind to FAP, a protein found on cancer-associated fibroblasts.
FAP
Fibroblast Activation Protein — a protein found in large quantities on fibroblasts that cancer has activated, but not significantly in most healthy adult tissue.
Cancer-associated fibroblasts
Cells in the tissue surrounding a tumour that cancer has recruited and reprogrammed to support its growth. FAPI targets these cells rather than the cancer cell itself.
Tumour stroma
The non-cancerous tissue around and within a tumour — including fibroblasts, blood vessels, and immune cells. FAPI targets part of this environment.
Radiotracer
A small molecule tagged with a short-lived radioactive atom. It travels to its target in the body, and the scanner detects the emitted radiation to build an image.

What should you ask before requesting a FAPI PET-CT scan?

  • Ask your oncologist whether your cancer type has published evidence supporting FAPI imaging
  • Ask whether the result would change your treatment plan — if not, the scan may not be necessary
  • Tell your team all the medications and supplements you are currently taking
  • Ask about preparation — FAPI generally does not require fasting, unlike FDG
  • Confirm when and how results will be communicated to you and your treating team
  • Ask the imaging centre to confirm the cost before your appointment

In pancreatic and biliary cancers, FDG often returns a weak signal because these tumours tend not to be highly glucose-hungry. FAPI targets the fibroblast network surrounding the tumour instead — and in published research, this has shown a clearer picture of disease extent in a meaningful proportion of cases where FDG was insufficient.

What does FAPI stand for and how does the tracer work?

FAPI stands for Fibroblast Activation Protein Inhibitor — it is the radioactive molecule injected before the scan. In many cancers, the tissue surrounding the tumour is rich in cancer-associated fibroblasts that express a protein called FAP. FAPI binds to FAP selectively. Because FAP is present in large amounts on these activated fibroblasts but not significantly in most normal adult tissue, the tracer builds up where the tumour is and gives a low background signal elsewhere. The PET scanner then detects the radiation from the tracer to build an image of where it has accumulated.

Which cancers is FAPI PET-CT most useful for?

FAPI PET-CT tends to give the clearest advantage in cancers where FDG uptake is typically low — most often gastric, pancreatic, biliary tract, peritoneal, and some ovarian cancers, as well as soft tissue sarcomas. It has also been studied in head and neck and breast cancers. How much it helps depends on both the cancer type and your individual tumour. Your oncologist will tell you whether your specific cancer is one where published evidence supports a FAPI scan and whether the result is likely to change your management.

Do I need to fast before a FAPI PET-CT scan?

You generally do not need to fast before a FAPI PET-CT, which is a practical difference from FDG. FDG requires fasting to keep blood glucose stable because glucose competes with the tracer for uptake. FAPI works via a different mechanism — binding to a protein, not glucose transport — so fasting is typically not required. The preparation instructions will be given to you by the imaging centre when your scan is booked. Follow those instructions specifically, because protocols can vary between centres.

Can FAPI PET-CT replace FDG PET-CT?

FAPI does not replace FDG — they answer different questions. FDG remains superior for cancers that are highly metabolically active, including many lymphomas, melanomas and certain lung cancers. FAPI adds value where FDG returns a weak or equivocal signal. Your oncologist may suggest both in some circumstances, or FAPI alone where FDG has already been done and the result was not clear enough. The decision is about which imaging question needs to be answered, not about which tracer is universally better.

Is FAPI PET-CT available at CION? What does it cost?

Is the evidence for FAPI PET-CT strong enough to trust?

The evidence is growing and promising, but it is not yet at the same level of maturity as FDG for most cancers. Multiple prospective studies and published research, along with guidance from the European Association of Nuclear Medicine, support FAPI's utility in specific cancer types. It is used clinically at specialist centres in India and internationally. What the evidence does not yet establish clearly is whether detecting additional disease on FAPI changes long-term outcomes — that question is still being studied.
