PET-CT vs PET-MRI: — Is the Newer Machine Better?

PET-MRI is newer and gives sharper images of soft tissue, but it is available at very few centres in India. For most patients and most cancers, PET-CT is the established standard — and being recommended it is not a compromise.

When might PET-MRI be worth raising with your oncologist?

  • Your cancer is in the brain, liver, bile ducts or pelvis, where MRI gives sharper soft-tissue images than CT
  • Your oncologist has said a specific area is unclear on PET-CT and MRI may help resolve it
  • You are being assessed for prostate cancer, where MRI is already part of the standard workup
  • You are a child or teenager, and reducing radiation exposure carries greater weight
  • You need repeated scans over time and cumulative radiation dose is a consideration your team has raised

Is PET-MRI actually available in India?

PET-MRI is available at only a small number of centres in India — mainly certain large teaching hospitals and a few private academic centres in the biggest cities. It is not widely available, and most cancer centres do not have access to it.

PET-CT, by contrast, is available at major cancer centres across India. CION coordinates PET-CT through partner imaging centres in Telangana and Andhra Pradesh.

If your team has recommended PET-CT, it is almost certainly because it is the appropriate scan — not because PET-MRI is unavailable and they are settling for less.

Does a better machine mean a better outcome for you?

Not necessarily. For the majority of cancer types — including lung, breast, lymphoma, colorectal and most head and neck cancers — PET-CT is the imaging standard recommended by NCCN, ASCO and ESMO. There is no established evidence that PET-MRI improves clinical outcomes for most patients in these categories.

PET-MRI has shown genuine advantages in specific situations: cancers close to the liver, certain brain tumours, prostate cancer, and cases where reducing radiation matters more than usual. Outside those situations, the additional detail it provides rarely changes the treatment decision.

The scan that answers your oncologist's clinical question is the right scan for you.

What do these scan terms mean?

PET (Positron Emission Tomography)
Shows how active cells are metabolically. Cancer cells often absorb more glucose than normal cells, so they light up on a PET scan.
CT (Computed Tomography)
Uses X-rays taken from multiple angles to build a three-dimensional picture of anatomy — bones, organs and masses.
MRI (Magnetic Resonance Imaging)
Uses magnetic fields and radio waves instead of X-rays. No ionising radiation. Gives especially good detail in soft tissue, the brain and the pelvis.
FDG tracer
A mildly radioactive form of glucose injected before the scan. It is taken up by active cells, making metabolically busy areas — including most tumours — visible on PET.
Attenuation correction
A technical step that uses the CT or MRI image to sharpen the PET signal. PET-MRI handles this differently from PET-CT, which is one reason it is more complex and takes longer.

What should you do if PET-CT was recommended for you?

Have the scan your oncologist has asked for. For the vast majority of patients and cancer types, PET-CT is the right answer, and there is no clinical reason to delay it while searching for a PET-MRI centre.

If you want to understand the reasoning, ask directly: 'Is there anything MRI would show that CT cannot for my cancer?' A clear answer is reasonable to expect.

Do not travel to another city for PET-MRI unless your oncologist has specifically said it would change your management plan. The delay, cost and stress are real, and for most patients the scan result would be the same either way.

What is PET-MRI and how is it different from PET-CT?

Both scans use the same PET technology to show metabolic activity in the body. The difference is what sits alongside it: PET-CT pairs PET with a CT scan, which uses X-rays to image anatomy, while PET-MRI pairs PET with MRI, which uses magnetic fields and gives sharper detail in soft tissue such as the brain, liver and pelvis. PET-MRI also delivers a lower radiation dose because the MRI component adds no ionising radiation. The trade-off is that it takes longer, is available at very few centres in India, and the evidence base for most cancer indications is still developing compared to PET-CT.

Is PET-MRI available in Hyderabad or at CION?

PET-MRI is not available at CION centres. CION uses PET-CT, coordinated through partner imaging centres in Telangana and Andhra Pradesh. PET-MRI is available at very few sites in India overall — mainly selected large academic or private hospitals in a handful of major cities. For the cancer types most commonly managed at CION, PET-CT is the clinically appropriate and guideline-recommended scan.

Is PET-MRI better than PET-CT for detecting cancer?

It depends on the cancer type and the clinical question your oncologist is trying to answer. For cancers in or near the liver, certain brain tumours, prostate cancer and rectal cancer, MRI provides better soft-tissue detail and PET-MRI may add genuine value. For most other cancer types — including lung, breast, lymphoma and most head and neck cancers — NCCN, ASCO and ESMO guidelines specify PET-CT as the standard, and there is no established evidence that PET-MRI improves clinical outcomes in those situations. Being recommended PET-CT for most cancers is not a compromise.

Why did my oncologist ask for PET-CT and not PET-MRI?

Almost certainly because PET-CT is the appropriate scan for your cancer type and the question your team is trying to answer. For the majority of cancer indications, international guidelines recommend PET-CT, not PET-MRI. Availability also plays a role — PET-MRI is not accessible at most centres — but even where it is available, most oncologists would still choose PET-CT for most patients because the evidence supports it. If you want to understand the reasoning, ask your team directly: they should be able to tell you what information they are looking for and why PET-CT answers it.

Is the radiation from PET-CT harmful?

PET-CT involves a radiation dose from both the radioactive tracer and the CT component. Your oncologist and radiology team weigh this against the clinical benefit before recommending any scan — and for a cancer investigation, that benefit is almost always clearly greater. If repeated scanning is needed over months or years, cumulative radiation dose is something your team will factor in, and it is a reasonable question to raise. PET-MRI delivers a lower dose, but it is only a practical alternative in specific situations and at the very few centres that have the machine.

Should I travel to another city to get a PET-MRI?

Only if your oncologist has specifically said it would change your treatment plan. For most patients, the scan result would be the same whether you had PET-CT locally or PET-MRI elsewhere — and the delay, cost and stress of travel are real. Ask at your next appointment: 'Is there anything MRI would show that CT cannot for my cancer?' If the answer is no, your local PET-CT is the right choice. If the answer is yes, your team will refer you appropriately rather than leaving you to arrange it yourself.
