What do these insurance terms mean?
- DOTATATE PET-CT and DOTANOC PET-CT
- Specialist nuclear medicine scans used to locate neuroendocrine tumours. A radioactive tracer is injected and travels to tumour cells, which then appear on the scan. DOTANOC and DOTATATE are two different tracers used for the same purpose.
- Prior authorisation
- Written approval from your insurer or TPA obtained before you have the scan. Without it, cashless claims are refused. Applying after the scan has already happened will not convert to cashless settlement, though a reimbursement claim is still possible.
- Reimbursement claim
- You pay for the scan yourself and then apply to your insurer to recover the cost. You will need original receipts, the scan report, and the same supporting documents as a prior authorisation application.
- TPA (Third-Party Administrator)
- The company your insurer uses to process claims. Prior authorisation requests go to the TPA first, not the insurer directly. Your policy documents and insurance card will show which TPA handles your plan.
- IRDAI
- Insurance Regulatory and Development Authority of India — the body that regulates insurers. You can file a complaint with IRDAI through the Bima Bharosa portal if a claim is wrongly refused after an internal appeal.
How do you apply for prior authorisation before your scan?
Get a specific referral letter from your oncologist
The letter should name DOTATATE or DOTANOC PET-CT explicitly, state your diagnosis, and explain why a standard CT or MRI cannot answer the same clinical question. A vague letter asking for 'PET-CT' is more likely to be queried by the TPA.
Contact your TPA before booking the scan
Call the helpline number on your policy card. Ask whether DOTATATE PET-CT is covered under your plan, what documents they need, and how many days the review will take. Get a case reference number in writing.
Submit the pre-authorisation documents
Send the referral letter, your pathology report confirming the diagnosis, any previous scan reports, and your policy details. Ask for confirmation of receipt and a timeline for the decision.

Which schemes and policies cover DOTATATE PET-CT?

What documents make a claim more likely to succeed?
The referral letter is the most important document. It should name DOTATATE PET-CT or DOTANOC PET-CT by name, state your confirmed diagnosis, and explain why a routine CT or MRI is not sufficient for your case. A letter that explains the clinical reasoning is much harder to refuse than one that simply requests imaging.
Support the referral with your biopsy or pathology report confirming the tumour type, previous scan reports showing the disease, and discharge summaries if you have been admitted. A treatment history summary is also useful if you have already started treatment.
Ask your oncologist to print the letter on clinic letterhead with their signature, registration number, and date. Unsigned or undated letters are returned by TPAs, and chasing a correction costs you days you may not have to spare.
What can you do if your claim is refused?
Ask the TPA or insurer to give the reason for refusal in writing. You are entitled to this. A written reason tells you exactly what your appeal needs to address — a missing document, a coding issue, or a policy exclusion.
Once you have the reason, ask your oncologist to write a supplementary letter addressing the specific objection. Then file a formal internal appeal with the insurer. Most policies allow at least one internal appeal before external escalation.
If the internal appeal also fails, file a complaint with IRDAI through the Bima Bharosa portal or approach the regional insurance ombudsman. Consumer courts accept health insurance disputes as well. Keep copies of every submission, every response, and every date you made contact — this record is what you will need if the dispute continues.
