Being asked for two major scans in the same week is common when your team needs different types of information. PET-CT and MRI answer different questions — and having both does not mean one is unnecessary.

In what order should you have them?
Either order is clinically acceptable in most situations. Your team's decision will usually come down to which result is needed first and which appointment slot is available sooner.
If your MRI uses gadolinium contrast, some centres prefer to schedule it before PET. This is a logistical preference, not a firm clinical rule — gadolinium does not affect how the PET tracer distributes through the body.
The radiotracer from PET has a short half-life, measured in hours rather than days. By the time you have an MRI later the same day or the following morning, most of it has already decayed and it will not interfere with the MRI images.
If both scans are on the same day, MRI is often done first, followed by PET after the tracer has been given time to distribute. Ask your radiology team how they have sequenced it — the answer will be specific to your protocol and imaging centre.

What to prepare before having both scans
- Tell your team if you have diabetesPET requires blood sugar to be within a specific range. Your team will advise how to manage this around your usual medication and meal timings.
- Fast as instructed before PETYou will be told not to eat for several hours before the tracer injection. Water is usually allowed. Ask specifically what you can and cannot have, and for how long.
- Avoid strenuous exercise the day before PETMuscle activity raises glucose uptake in muscles, which can make the scan harder to read. Ask your team what counts as strenuous for this purpose.
- Tell MRI staff about all metal implants before bookingThis includes pacemakers, cochlear implants, joint replacements, and surgical clips. Some implants are MRI-compatible; others are not. Staff need this information before the appointment, not on the day.
- Ask if a kidney blood test is needed before gadolinium contrastGadolinium is filtered by the kidneys. If you have kidney problems, your team may check your kidney function before deciding whether to use contrast for MRI.
- Tell both teams about previous reactions to contrast dyeA reaction to one type of contrast does not mean you will react to the other — gadolinium and FDG are different substances — but both teams should know your history.

Terms you may hear when both scans are discussed
- FDG
- Fluorodeoxyglucose — a radioactive form of glucose injected before PET scanning. Cancer cells absorb it faster than most normal cells, which is why they appear brighter on the scan.
- Radiotracer
- The short-lived radioactive substance used in PET. It travels through the body and is detected by the scanner. Most of it decays within hours, which is why radiation exposure from PET is brief.
- Half-life
- The time it takes for half of a radioactive substance to decay. The tracers used in PET have half-lives measured in hours, so residual radiation after the scan is minimal by the following day.
- Gadolinium
- A contrast agent given by injection during some MRI scans to make blood vessels and tumours easier to see. It is not radioactive and is a completely different substance from PET tracers.
- Uptake time
- The waiting period between the FDG injection and the start of the PET scan. The body needs this time to distribute the tracer evenly before imaging can begin.
- Staging
- Determining how far a cancer has spread — which organs or lymph nodes are involved. PET-CT is commonly used for staging because it surveys the whole body in a single scan.
When does your team need one scan and not the other?
PET-CT is not always the right scan. For the brain and spine, PET is less reliable because normal brain cells already consume large amounts of glucose, making it difficult to separate cancer from healthy tissue. MRI is the standard choice for the brain and spine.
For precise local staging — exactly how far a tumour extends into surrounding tissue in the pelvis, liver, or soft tissue — MRI gives detail that PET cannot match. PET may confirm a tumour is active; MRI shows exactly where its edges are and what structures are involved.
PET is used where the whole-body picture matters: finding spread to distant lymph nodes, lungs, bones, or other organs in one session. No standard MRI covers the whole body in the same way.
When both are requested together, it is because your team has two distinct questions. The cost and time involved reflect that, not duplication. If you are unsure why both are needed, it is a fair question to ask your oncologist before booking.

Frequently asked questions
Will the radiotracer still be in my body when I have the MRI?
If your MRI is later the same day, a small amount may still be present — but it will not affect the MRI images. MRI detects magnetic signals, not radioactivity, so the two do not interfere. By the following day, the tracer will have largely decayed due to its short half-life. You do not need to wait a specific number of days between the two scans. Your team will tell you if they want any particular gap for your protocol.
Is it safe to have both scans in one week?
Yes. MRI uses no ionising radiation, so combining it with PET does not add to your radiation exposure from the MRI side. The radiation from PET comes from the tracer, which decays quickly. Having both in the same week is routine practice in oncology when whole-body staging and local anatomical detail are both needed. If you are concerned about your total radiation exposure, ask your oncologist — they can explain what each scan contributes and put it in context for your situation.
Does the gadolinium dye from MRI affect the PET scan?
No. Gadolinium works by altering magnetic signals in tissue and is cleared by the kidneys. It does not affect how FDG distributes through the body or how the PET scanner reads its signal. If your MRI with gadolinium is done the same day or the day before PET, the two scans remain independent of each other. Tell both teams you are having both scans so they can note it in their reports if they consider it relevant.
I already had a CT with iodine contrast for PET — do I also need gadolinium for MRI?
The CT part of PET-CT uses iodine-based contrast, which is a different substance from gadolinium used in MRI. They do not interact. Whether you need gadolinium for your MRI depends on what the scan is looking for — it is not automatically required because you already had iodine contrast. Your radiologist decides based on the clinical question. Tell the MRI team what contrast you already received so the information is in the record before they make that decision.
What if I am claustrophobic — which scan is harder to tolerate?
MRI is generally harder for people with claustrophobia. The bore is narrower and the scan runs longer, often with loud repetitive noise throughout. PET-CT scanners have a wider, more open design and the imaging portion itself is shorter once the tracer has been taken up. If claustrophobia is a concern, tell your team before booking either scan. Mild sedation is sometimes offered for MRI, and knowing this in advance means it can be arranged rather than discovered at the last moment.
Can my care team at CION help coordinate both scans?
