What do DOTANOC and DOTATATE actually target?

Both tracers carry a small radioactive label — Gallium-68 — attached to a molecule that seeks out somatostatin receptors on the surface of certain tumour cells. Neuroendocrine tumours carry these receptors in large numbers.

When you receive the tracer as an injection, it travels through the bloodstream and collects wherever somatostatin receptors are present. The PET scanner then images those areas of uptake.

DOTATATE binds primarily to one receptor subtype, SSTR2, with very high affinity. DOTANOC binds to three subtypes — SSTR2, SSTR3, and SSTR5 — which can matter in certain tumour types. Your oncologist specifies which tracer your referral requires; this is not a choice you need to make.

How is this different from a standard FDG PET scan?

FDG PET works by tracking glucose. Cancer cells that divide rapidly consume a lot of glucose, so they appear bright on an FDG scan. This works well for many common high-grade cancers.

Well-differentiated neuroendocrine tumours often grow slowly and consume glucose at a rate close to normal tissue. They can appear negative — or only weakly positive — on an FDG scan even when disease is present and widespread.

DOTANOC and DOTATATE bypass glucose entirely. They light up based on receptor expression, which is the biological feature these tumours reliably carry. This is why an oncologist who suspects or has confirmed a NET will refer for a somatostatin receptor PET rather than, or in addition to, an FDG scan.

Did you know?

Gallium-68 somatostatin receptor PET-CT has replaced OctreoScan — the previous standard, which used Indium-111 octreotide — in most international guidelines for imaging neuroendocrine disease.

What is the difference between DOTANOC and DOTATATE?

Both are Gallium-68 somatostatin receptor tracers that detect the same group of conditions. The technical difference is receptor binding: DOTATATE has very high affinity for one subtype, SSTR2, while DOTANOC binds three subtypes — SSTR2, SSTR3, and SSTR5. In practice, the choice is made by your oncologist based on your tumour type and referral; it is not something you decide. At CION partner imaging centres, both scans are available subject to tracer supply, which is confirmed at the time of booking.

Is the tracer always available when I book?

Not automatically. Gallium-68 tracers have a short half-life and are produced in small batches. Supply is confirmed at the time of booking — your scan date is set only once that confirmation is in place. Last-minute changes do happen if a batch is delayed. If you are travelling from outside the city specifically for this scan, ask the team to confirm explicitly that tracer supply is secured before you make travel or accommodation arrangements.

Do I need to fast before a DOTANOC or DOTATATE scan?

Preparation for somatostatin receptor PET is different from FDG PET. FDG PET requires strict fasting because blood sugar affects tracer uptake. That constraint does not apply in the same way here, but preparation requirements can still vary depending on your specific situation and whether other tests are being done at the same visit. Ask the imaging centre at the time of booking and follow their instructions exactly — do not assume the same rules apply as for any FDG scan you may have had before.

How long does the scan take, and what happens on the day?

The tracer is given as a small intravenous injection. You then wait for it to distribute through the body before the scan begins — this waiting period is typically around one hour. The scan itself takes less than an hour in most cases. Total time at the centre, including waiting, is usually two to three hours. You can eat and drink normally during the waiting period unless the team tells you otherwise. The imaging team will confirm the exact timeline at booking.

What does it cost, and is it covered by insurance?

DOTANOC and DOTATATE PET-CT are coordinated through CION partner imaging centres at an indicative cost of around ₹23,999 as of mid-2026 — confirm the current figure at the time of booking, as costs change. Whether your health insurer covers it depends on your specific policy. Coverage for Gallium-68 nuclear medicine imaging varies widely. My doctor mentioned PRRT. Is this scan connected to that treatment?

Directly, yes. Peptide Receptor Radionuclide Therapy (PRRT) is a treatment for neuroendocrine tumours that uses the same somatostatin receptor system to deliver targeted radiation to tumour cells. Before PRRT can be considered, your team needs to confirm that your tumour shows adequate somatostatin receptor uptake on a Gallium-68 PET scan. A DOTANOC or DOTATATE PET is required as part of the PRRT workup, and the result directly affects whether that treatment is an option for you.
