Albumin and protein levels
Albumin is the main protein in the blood, made by the liver. It helps hold fluid inside blood vessels and carries medicines and hormones. It often falls during cancer and treatment, but not simply from poor eating: inflammation, infection, liver and kidney changes and fluid shifts all lower it. Good nutrition helps recovery, but diet alone may not raise it.

What does low albumin mean during chemotherapy?
Albumin is the main protein in the liquid part of the blood. It is made by the liver, and it does several jobs: it holds fluid inside the blood vessels, carries medicines, hormones and minerals such as calcium around the body, and helps tissues heal. On a blood report it usually appears in the liver panel alongside total protein and globulin. A low albumin is common during cancer and its treatment, and many families assume it simply means the patient is not eating enough protein.
In reality, poor eating is only one of several causes, and often not the main one. When the body is fighting cancer, infection or inflammation, the liver deliberately makes less albumin and more of other proteins, so albumin falls even when eating is good. Liver problems, protein lost through the kidneys or the gut, fluid shifts from drips, and collections of fluid in the tummy or around the lungs all lower it too. This is why protein powders and extra eggs alone may not raise the number, even though good nutrition still matters for strength and recovery. Low albumin can cause swelling
With less albumin to hold fluid in the blood, fluid can leak into tissues, causing swelling of the feet, legs or tummy.
It affects some test results
Because albumin carries calcium, a low albumin can make calcium look lower than it really is. Your team adjusts for this.
It can affect medicines
Some medicines are carried by albumin, so their effects can be stronger when it is low.
Tell your team about poor appetite, weight loss, swelling, diarrhoea or frothy urine, which can all be linked with low albumin.Possible causes
Why albumin can fall during cancer and treatment
- Inflammation and illness
- Cancer, infection and inflammation signal the liver to make less albumin. This is often the main cause and does not correct with diet alone.
- Poor eating and weight loss
- Nausea, mouth sores, taste changes and low appetite reduce protein intake over time.
- Liver changes
- Liver disease, cancer in the liver or medicine effects can reduce albumin production.
- Protein loss through the kidneys
- Some medicines and kidney conditions let protein leak into the urine, which may look frothy.
- Protein loss through the gut
- Severe diarrhoea and some bowel conditions cause protein to be lost.
- Fluid shifts
- Drips dilute the blood, and fluid collecting in the tummy or chest draws albumin out of the bloodstream.

Supporting nutrition during treatment
Helpful for strength, even if albumin does not change quickly.
Eat protein at every meal
Dal, pulses, eggs, milk, curd, paneer, fish, chicken, soya and nuts are good sources. Small amounts often are easier than big meals.

Eat little and often
Five or six small meals and snacks are often easier than three large meals when appetite is poor.
Ask about supplements
A dietitian can suggest oral nutrition supplements if eating is difficult. Ask before buying protein powders.

Treat symptoms that stop eating
Report nausea, mouth sores, constipation and taste changes, which can often be eased.
Stay gently active
Light activity helps maintain muscle and appetite, as energy allows.
Rapidly increasing swelling of the legs, tummy or face · breathlessness, especially when lying flat · passing much less urine · severe diarrhoea that will not settle · yellowing of the eyes or skin · confusion or extreme drowsiness · being unable to eat or drink for more than a day. Being straight with you
What this page cannot tell you
It cannot tell you why your albumin is low or whether it is a concern. The cause may be inflammation, nutrition, liver, kidney or gut problems, or fluid shifts, and often several together. Your team interprets it with your other tests and symptoms.
It also cannot tell you whether you need albumin infusions or supplements. Albumin drips are used only in specific situations and do not fix the underlying cause. Your team decides.
Protein powders are not a quick fix
They can help people who cannot eat enough, but they may not raise albumin when inflammation is the cause. Some also contain added sugars or herbs.
Kidney and liver conditions change advice
People with some kidney or liver problems may need careful protein amounts. A dietitian can guide this.
Low albumin and swelling
Raising the legs and gentle movement may ease swelling, but new or worsening swelling should still be reported.
Weight can be misleading
Fluid build-up can hide muscle loss on the scales. Tell your team if clothes feel looser even though weight seems steady.
Albumin and calcium
A low albumin can make calcium results look low. Your team may use a corrected calcium value.
Medicine effects
Some medicines, including certain pain and seizure medicines, may need care when albumin is low.
Traditional nutrition myths
Avoiding foods such as eggs, milk or fish during treatment because of beliefs about heat or cold can reduce protein intake. Ask your team what is safe.
Recovery takes time
As inflammation settles and eating improves, albumin often rises slowly over weeks.

Specialist nutrition advice is one of the most useful supports during treatment.
Mouth and swallowing problems
Mouth sores, dry mouth and difficulty swallowing make protein foods hard to eat. Soft options such as dal, khichdi, curd, eggs, paneer and smoothies can help, and your team can treat the mouth problems themselves so eating becomes easier.
Weekly weight checks at home
Weighing yourself once a week on the same scale, at the same time, helps show whether you are losing weight. Tell your team about steady loss or sudden gain.
Fluid in the tummy or chest
When fluid collects in the tummy or around the lungs, draining it can remove protein too, and albumin may fall further afterwards. Your team takes this into account and may check levels again after a drainage procedure.
What to do next
Report poor appetite, weight loss, swelling, diarrhoea or frothy urine, eat protein little and often, ask to see a dietitian before buying supplements, and let your team interpret albumin results and decide whether any treatment is needed.
