Most Indian health insurance policies cover chemotherapy given in hospital. Day-care sessions — where treatment takes a few hours — are covered under most current policies but some older ones exclude them. Does day-care chemotherapy count as hospitalisation?

Yes — under most policies issued or renewed in recent years. IRDAI has directed that day-care procedures, including chemotherapy given without an overnight stay, be treated as hospitalisation for health insurance claim purposes.

Older policies, or those with explicit minimum-stay clauses, may still require 24 hours of admission before a claim is valid. If your policy is more than five years old, check the day-care procedures list in your policy document — do not assume you are covered.

The hospital billing team can clarify what documentation your insurer typically requires. The verification of your own coverage, however, must happen directly with your insurer before treatment begins — not after.

Why are oral chemotherapy tablets so often not covered?

Oral chemotherapy is often excluded because standard health insurance is built around hospitalisation. When you are admitted to hospital, the insurer pays the hospital or reimburses a bill. Oral tablets work differently — you collect them from a pharmacy and take them at home.

Base policies classify pharmacy medicines as outpatient expenses and either exclude them entirely or apply an annual limit too low to cover what oral chemotherapy can cost across several months.

If oral therapy is part of your treatment plan, ask your insurer whether your policy covers it and to what limit. Many newer policies offer an OPD rider or pharmacy benefit that covers this — but it must be in place before the prescription is written, not after the claim is submitted.

What to check with your insurer before chemotherapy starts

  • Ask whether day-care chemotherapy is listed in your policy's approved day-care procedures.
  • Ask whether oral chemotherapy tablets are covered, and to what annual limit.
  • Confirm the pre-authorisation process and how many days in advance you need to apply.
  • Check for a co-pay clause — a percentage you pay out of pocket regardless of coverage.
  • Check your room rent sub-limit — a capped room rent can reduce the entire inpatient bill proportionately.
  • Ask whether supportive medicines — anti-nausea tablets, blood tests, growth factor injections — are included in the claim.
  • Get confirmation from your insurer in writing, noting the date and the name of the agent you spoke with.

Frequently asked questions

Does IRDAI require insurers to cover chemotherapy?

IRDAI guidelines require that hospitalisation for the treatment of any illness, including cancer, be covered under standard indemnity health policies. Day-care procedures — those that previously required overnight admission but can now be completed in a few hours — must also be covered under most policies. However, IRDAI does not specify a uniform list of covered procedures, and policy terms still vary. Your specific policy document is the authority on what is covered, not general IRDAI guidance alone.

What is pre-authorisation and how do I get it for chemotherapy?

Pre-authorisation is your insurer's written approval for a planned treatment before it starts. Without it, your claim is very likely to be rejected even if the treatment would otherwise be covered. Your oncologist's team prepares a letter explaining your diagnosis and the planned treatment plan. You or the hospital submits this to the insurer — usually three to five days before the first session. Ask your insurer at this point whether a single approval can cover all planned cycles, or whether each cycle needs separate pre-auth.

If my first cycle was approved, do I need a fresh approval for each subsequent cycle?

Usually yes. Most insurers treat each chemotherapy cycle as a separate claim requiring its own pre-authorisation. Some will approve the full course under a single pre-auth if your oncologist's letter specifies the total number of planned sessions. Ask your insurer at the start whether course-wide approval is possible — this can save significant paperwork over six cycles.

Is Ayushman Bharat or a state government scheme the same as private insurance?

No, and the process is different. Ayushman Bharat PM-JAY and state schemes such as Aarogyasri cover specific procedures at empanelled hospitals, and chemotherapy is generally within scope. These schemes do not use pre-authorisation in the same way private policies do. The hospital's insurance desk at an empanelled centre guides you through the scheme-specific process. Whether your preferred hospital is empanelled under your scheme is the first thing to verify.

Can I claim from two insurance policies at the same time?

Yes. This is called a contribution claim. Your primary insurer pays first, and the secondary insurer covers any remaining balance up to its own limit. The total recovered cannot exceed the actual cost of treatment. You must notify both insurers before the claim — not after. Tell your treating hospital which policies you hold; the billing team can help coordinate the documentation, though each claim runs through its own insurer separately.

What can I do if my chemotherapy claim is rejected?

Request the rejection in writing with the specific reason stated. Common reasons include missing pre-authorisation, a procedure coded incorrectly, or a clause the insurer says excludes the claim. If the reason is a billing code error, ask the hospital to resubmit with the corrected code. If you believe the rejection is wrong, raise a formal grievance with the insurer first. If that is unresolved, escalate to IRDAI's Bima Bharosa grievance portal. Keep all documents — approval letters, bills, discharge summaries, and correspondence — for the appeal.
