Why is there no cough or pain if I have a serious infection?

When chemotherapy lowers your white blood cell count, your body cannot produce the normal signs of infection — no cough, no pain, no redness.

Your immune system uses white blood cells to attack infection and cause the swelling and redness that signal where the problem is. When those cells are depleted, infection arrives silently.

This is why a fever may be the only warning you get. It is also why any fever during chemotherapy is treated as a medical emergency, regardless of how otherwise well you feel.

Do not wait for other symptoms to appear before going to hospital. They may never come, even as the infection becomes serious.

Does the temperature number matter if I feel otherwise okay?

Any fever in a person on chemotherapy is treated as potentially serious, regardless of the exact reading. The number matters less than the fact that you are on chemotherapy and your temperature has risen. Feeling otherwise okay does not make it safe to wait — neutropenic fever can worsen rapidly even when you feel only mildly unwell. Go to hospital tonight and let the team assess your blood counts.

Can I wait until the clinic opens in the morning?

No. NCCN and ASCO guidance is clear that fever during chemotherapy needs same-night hospital assessment. The hours between now and when the clinic opens are exactly when the risk of deterioration is highest. If you are uncertain, go to the nearest emergency department rather than waiting for a callback in the morning.

What will happen when I arrive at the emergency department?

You will be assessed quickly once the triage team knows you are on chemotherapy and have a fever — say this immediately when you arrive. The team will take blood tests to check your white blood cell count and look for signs of infection. Depending on the results, you may be admitted for monitoring and treatment. Bring your chemotherapy drug card or treatment summary if you have one.

They seem fine apart from the fever. Is it really that urgent?

Yes. Feeling well is exactly what makes neutropenic fever so dangerous — the normal signs that tell you how serious an infection is are suppressed. ASCO and NCCN guidance treats fever during active chemotherapy as a same-night emergency even when the person appears otherwise well. The absence of other symptoms is not reassurance; it is part of what makes this different from an ordinary fever.
