What documents do you need when changing chemotherapy hospitals?

  • Discharge summary or oncology summary letter from your previous hospital
  • Chemotherapy cycle records showing drug names, doses and dates of each cycle given
  • Blood test reports from at least your last two or three cycles
  • Recent scan reports and the physical disc or digital copy of the images
  • Biopsy or pathology report
  • Molecular or genetic test results, such as HER2, EGFR or PD-L1 testing
  • A complete list of all medicines, supplements and herbal or Ayurvedic preparations you are taking

Does switching hospitals delay your next chemotherapy cycle?

In most cases, switching does not add a meaningful delay. Your new centre will review your previous records before confirming the next cycle — that review takes one appointment, not several weeks.

The one situation that can cause a short delay is arriving without sufficient records. Blood test results from the last two or three cycles help the new team make a safe decision quickly. Bring your own copies rather than waiting for your previous hospital to send them.

If your next cycle is due within a few days, say so when you call to book. The new centre can arrange an urgent review appointment so your timing is not affected.

How do you make the handover to a new centre go smoothly?

The most important step is to collect your own copies of your records before you leave your previous centre. Most hospitals will provide these on request. You are entitled to them.

When you contact the new centre, mention that you are mid-chemotherapy and state which cycle you are on. This allows the team to prepare for a treatment review rather than an initial consultation — a different kind of appointment that needs different preparation.

Tell your new team about everything you are taking — including vitamins, herbal preparations and any Ayurvedic medicines. These can affect how chemotherapy behaves in your body, and knowing about them helps your team care for you safely.

Frequently asked questions

Will I have to restart my chemotherapy from cycle one if I switch centres?

No. Your chemotherapy does not restart when you change hospitals. Your new centre picks up from your current cycle and continues the plan your oncologist designed. They will review your cycle records and blood tests before giving the next dose — which is what any centre does between cycles regardless of where you have been treated.

What documents do I need to bring when I change hospitals?

The most important ones are your oncology summary or discharge letter, your chemotherapy cycle records showing drugs, doses and dates, blood test reports from your last two or three cycles, and your recent scan reports or imaging discs. Your biopsy and pathology report and any molecular test results are also helpful. A list of all your current medicines rounds out the picture. Bring your own copies — do not wait for them to be sent ahead.

Will switching hospitals delay my next chemotherapy cycle?

In most cases, no. Your new centre will review your records before confirming the next dose — that is a single appointment, not a lengthy process. The situation most likely to cause a short delay is arriving without complete records, which is why collecting your own copies before you leave your previous centre matters.

Will the new centre make me repeat expensive tests I have already had?

Routine retesting is not needed when previous results are available and recent. Your new oncologist will review what was already done. A fresh blood count before each cycle is standard practice at any centre, not something specific to switching. Scan reports and pathology from your previous hospital are accepted as valid. Retesting would only happen if a result is outdated or something in your condition has changed.

Do I need to ask my current hospital's permission before switching?

No. You do not need permission to move to another centre. It is helpful to let your team know — they can write a summary and prepare your records, which saves time at the new centre. If the relationship is difficult, you can simply request your records without further explanation. You are entitled to them.

Can I switch back to my previous hospital if I change my mind?

Yes. Nothing about switching is permanent. If your circumstances change, you feel more comfortable elsewhere, or you want to return to your original centre, you can move again. The same process applies each time: bring your current records with you. Your new centre continues from wherever you are in the treatment plan.
