Alpelisib cost in India: monthly and full-course price

What shapes the monthly and full-course cost of alpelisib in India?
Alpelisib is a patented targeted tablet, and like most newer cancer medicines it is expensive. This page does not quote prices, because they change over time, differ between suppliers and schemes, and are easily misunderstood. Instead it explains what makes up the total so you can ask for a clear estimate. Unlike medicines dosed by body weight, alpelisib is taken at a fixed daily dose, so the monthly tablet cost is fairly predictable at a given strength. However, if your dose is lowered because of side effects, a different tablet strength or pack may be used, which can change the monthly amount. The full-course cost depends on how long you stay on treatment, and there is no fixed length: it continues for as long as it controls the cancer and side effects are manageable. Alpelisib is always paired with fulvestrant injections, which add their own monthly cost, although fulvestrant is an older medicine with more options available. Supportive care also adds up: blood sugar tests and home glucometer strips, diabetes medicines, antihistamines, creams and mouthwashes, clinic visits, blood tests and scans every few months. Availability and supply routes in India have changed over time, so ask your oncologist about the current position. Insurance, government and employer schemes, and manufacturer patient support programmes differ widely in what they cover.

The benefit is real but limited
For PIK3CA-mutated cancers, alpelisib kept the cancer controlled for several months longer on average. It does not make metastatic cancer go away.

Some people stop within months, so the full-course cost varies widely.

Other targeted, hormone or chemotherapy options may suit you if alpelisib is out of reach.

Is fulvestrant expensive too?
Fulvestrant is an older medicine, and more versions are generally available, which can make it less costly than alpelisib. It is still given regularly by injection, with clinic charges. Ask for its cost to be included in your overall estimate.
What if we cannot afford alpelisib?
Tell your oncologist openly. Other options, such as everolimus with exemestane, other hormone treatments, chemotherapy or clinical trials, may be suitable. Choosing a treatment your family can sustain is a sensible decision, and your care continues whatever you choose.
Can I stop and restart to save money?
Stopping and restarting without medical advice is not recommended. The cancer may grow during gaps, and restarting can bring back early side effects such as high sugar. If cost is becoming difficult, discuss it with your oncologist before making changes.
Is there a patient support programme?
Manufacturers sometimes run programmes for eligible patients, and cancer charities may help too. Details and eligibility change, so ask the hospital social worker or financial counsellor what is currently available and what documents you need.
Are there hidden costs I should plan for?
Families often forget glucometer strips, diabetes medicines, travel for injections and scans, time off work and dietary changes. Listing these alongside the medical estimate gives a more realistic picture and helps you plan support.
